When Sullivan drops the metal case with the hypo-gun and the samples inside, the case is securely closed. When Dean spots it, the case is sitting with the lid open.
After Wesley escapes from the interrogation room he's running down a hallway and just before he kicks open the door you can see a man (behind a glass door) down the hall in the distance. The two bad guys supposedly come down the same hall but you don't see the glass door, only a bulletin board, it's a different hallway. One of the bad guys shoots through the door and hits a power panel and the lights go out. Scene cuts and then you see the man behind the glass door again.
When Dean is running down the hall after shooting the guard and taking his weapon, he is not carrying the weapon, though it was in his hand when he flips the switch to secure the door. When he gets to the doors to let the patients out, he's holding the weapon again.
During Deans escape from the mental hospital, shots are fired yet the police in the building do not hear them.
When the bad guy shoots through the door and knocks out the power, the lights remain on the entry foyer of the mental hospital.
Caller ID shows up the way the name it is listed in the directory - last name, first name. But Amy's first name shows first on her Caller ID.
