Hydatid disease is a parasitic infection caused by Echinococcus granulosus. Hydatid cysts can rupture either spontaneously or following trauma. Ruptured hydatid cyst of the liver is an infrequent clinic entity. Ultrasonography and computerized tomography are the main diagnostic tools, with 85% and 100% sensitivity, respectively. Treatment with surgery is mandatory. We report a case of spontaneous rupture of hydatid cyst of the liver in a 5-year-old boy who presented to the emergency department with a history of abdominal pain. The germinative membrane was removed from the cyst pouch, and the peritoneal cavity was lavaged with hypertonic saline solution, followed by omentoplasty. Albendazole was administered for three months postoperatively. The recovery course was uneventful. In endemic regions, ruptured hydatid cyst of the liver should be included in the differential diagnosis of abdominal pain.

Hydatid cyst rupture into abdomen is a serious complication in cystic hydatid disease of liver. Both microscopic or macroscopic rupture can occur and it is fatal without surgery. It is primarily caused by tapeworm (ECHINOCOCCUS GRANULOSUS) and occurs worldwide with an incidence of 200/100,000in endemic areas. This is a case of 28 yr. female presenting with sudden onset pain abdomen since 4 days. Abdominal ultrasonography demonstrates ruptured hepatic hydatid cyst with free peritoneal fluid. She was managed in emergency operation theatre due to her clinical condition and exploratory laparotomy with omentopexy and toileting were done after fluid resuscitation, antihistaminic and corticosteroid treatment. Postoperatively she received antihelminthic treatment with Albendazol. She made a good recovery following surgery.

Introduction: Hydatid cyst of the liver is usually asymptomatic,diagnosed accidentally or when there is complications such as infection or rupture.Rupture may occur spontaneously or related to trauma. Spontaneous rupture is more seen in endemic areas like Iraq and frequently seen in children. Method: Four cases with spontaneous rupture of liver hydatid cyst were reported at Mosul Pediatric Surgical Center at AL-KHANSA A Teaching Hospital. They were diagnosed by ultrasound, chest x-rays and C T Scan. They underwent surgical exploration and removal of the cysts. Post-operative medical treatment with Albendazole were used prophylactly in all of them. Results: There were three males and one female, their age ranged from 5.5 to 10 years. Their main presentations were abdominal pain, one of them had respiratory symptoms while the forth had allergic reactions. Ultra sound examination was the main tool of diagnosis , chest x-rays was useful in one and C T Scan was needed in two of the cases .Surgical approach to these patients were by laparotomy in three and right lower thoracotomy in one .Removal of the cyst and cleaning of the cavity with antiseptic solution. These four hydatid cysts ruptured at different sites two of them to peritoneal cavity, one to thoracic cavity and one to extra peritoneal space. Conclusion: Liver H C rupture is a serious complication, it comes in the differential diagnosis of acute abdomen in endemic area .It needs urgent management. It can rupture at any points of liver surface to peritoneal cavity. Rupture can extend to pleura, biliary passages and extra peritoneal space when there is adhesion and erosion of the ectocyst to these structures. Rupture H.C of the liver to pleura can be dealt through thoracotomy approach without opening the abdomen. Extra peritoneal rupture of liver hydatid cyst can be dealt extra peritoneally without opening the peritoneal cavity and without contaminating the peritoneal cavity.

Thirty-six patients with intrabiliary rupture of hepatic echinococcal cysts were managed between 1974 and 1993. Clinical findings, skin tests, serologic tests, and imaging techniques were used to establish the diagnosis. Twenty-five (69.4%) patients had pain, 24 (66.6%) jaundice, 22 (61.1%) fever, 20 (55.5%) chills, 10 (27.7%) malaise, and 7 (19.4%) other symptoms as the major causes of admission. All patients underwent choledochotomy and T-tube drainage. Treatment directed to the cyst was cystectomy and capittonage, cystectomy and drainage, and partial hepatectomy in 22, 12, and 2 patients, respectively. Omentoplasty was added to the treatment in 10 patients. Seven (19.4%) patients had complications. The period of hospitalization for patients with and without complications was 34.6 -+ 18.1 and 15.1 -+ 2.7, days, respectively. This study indicates that better results are obtained in patients with cystic lesions of the liver by avoiding percutaneous puncture or biopsy, the early use of ultrasonography and computed tomography, evacuation of the cyst together with its germinative membrane and the involved biliary tract under adequate care to avoid spillage into the peritoneal cavity, treating the remaining cavity according to its location, size, and the presence of infection, and decreasing the pressure in the biliary tract by T-tube drainage.

Now the grim reality is that like so many other aspects of science and technology all ostensibly benign, decent, therapeutic, and beneficial they also contain a dark, malevolent, destructive warlike side as well. Our brains are vulnerable daily within a complex electromagnetic—cyber—RF saturated environment and that vulnerability is critical to grasping our collective dilemma.
