Globally, prostate cancer is a disease of public health importance and it is most common among men between 60 to 70 years of age. Distant primaries involving supraclavicular nodes secondary to prostate cancer is very rare. This report is a case of an unusual presentation of prostate cancer manifesting as a huge neck mass in a 61-year-old man from Kumasi, Ghana, who had complete regression of the mass after androgen deprivation therapy. Carcinoma of the prostate should be considered among the differential diagnoses in any elderly male presenting with a neck mass.Journal of Medical and Biomedical Sciences (2016) 5(2), 1-4

Background: Recent studies have questioned the high risk for disease recurrence in cases of bladder neck involvement by prostate cancer (pT4 disease). Design: The study was based on 141 patients submitted to radical prostatectomy. PSA-recurrence was defined as 0.5 ng/ml or rising at three different examinations. Bladder neck invasion was correlated to Gleason score (Results: Bladder neck invasion was seen in 30/141 (21.27%) patients; 9 (6.38%) had also concomitant positive surgical margins in the sections. Extraprostatic extension was seen in 39/141 (27.65%) and seminal vesicle invasion in 18/141 (12.76%) patients. Patients with bladder neck invasion significantly correlated to Gleason score (P=0.04), preoperative PSA (P P=0.04). No correlation was found to age (P=0.44), race (P=0.12) and positive urethral or lateral margins (P=0.32). The PSA-recurrence relative risk in patients with bladder neck invasion was 0.17 (P=0.68), with extraprostatic extension 0.53 (P=0.47) and with seminal vesicle invasion 5.76 (P=0.02). Conclusions: Bladder neck involvement correlates with pathologic unfavorable findings on radical prostatectomy specimens as well as to preoperative PSA levels. However, the PSA-recurrence risk associated with bladder neck involvement (pT4) was similar to extraprostatic extension (pT3a) and substantially lower than seminal vesicle invasion (pT3b). The findings favor a need for downstaging of bladder neck involvement in the next version of the TNM system. The method proposed for tumor extent evaluation is simple and accessible to all pathologists working in routine pathology laboratories.

Prostate cancer is the second most common cancer in men worldwide and the commonest cause of mortality in men. It is the commonest diagnosed cancer in African men. The earliest and common sites of metastasis are the axial skeleton and regional lymph nodes. Initial presentation of the metastatic disease with cervical or supraclavicular lymphadenopathy is rarely reported. We report 3 cases of metastatic cancer of the prostate presenting initially to physicians with huge cervical and supraclavicular lymph nodes enlargement which on biopsy revealed metastatic adenocarcinoma. On further evaluation, there were non-bothersome storage symptoms, elevated prostate specific antigen and abnormal digital rectal examination. Transrectal ultrasound (TRUS)-guided biopsy of hypoechoic nodules revealed high-grade adenocarcinoma of the prostate on histopathological examination. The cervical and supraclavicular lymphadenopathy resolved after commencement of androgen deprivation therapy. We advocate for prostate cancer screening in African men above 50 years of age presenting with cervical or supraclavicular lymphadenopthy to primary care physicians even in the absence of lower urinary tract symptoms.

By drawing examples from a particular genre of literary work—life-histories—this paper aims to describe, how ‘resistance’, a pervasive feature of total institutions, occurs in a variety of ways in different total institutions. Working within the broad theoretical framework of Goffman’s Asylums (1961), this article will assess the extent and nature of resistance that is possible in each total institution, and the amount of agency each inmate is invested with. It will also explore as to ‘how’ the basic characteristics of total institutions create space for resistance and ‘why’ inmates resist. This will lead to an understating of the ways in which the ‘resisting self’ develops—whether it is a result of the inmates' interaction with other inmates/staff, or is a product of their personal experiences before entering the total institution. The discussion will also seek to establish that depending upon how, why, and against what inmates resist, several types of total institutions differ.

To transmit an accurate, reliable and flexible power without any distortion to consumer is a serious issue and challenging to industry especially for engineer. During transmission, a lot of power problem occur which affect the power system operation and efficiency. There are several issues regarding power quality of which harmonics affect the power system operation and their extenuation should be given preeminent importance. Among different types of harmonic filters, active power is examined as very much profitable and economical solution. This paper presents the simulation of active power filter for reduction of harmonics using mitigation techniques MATLAB/Simulink software.

The Codex Iustinianus was of much less importance than the Digest for the rebirth of medieval scientia iuris; however, the turning points that mark the evolution of medieval jurisprudence all have a precise resonance in the different approach to the study of the Codex and in particular to the Tres Libri Codicum, the final books of the legal compilation. In particular, we have focused on three turning points: the birth of medieval legal science and the role of Pepone, still obscure in many ways; the study of the Tres Libri from Vacario to Rolando da Lucca, with the definitive affirmation of the summae; and the Annotationes ai Tres Libri (1515) by Andrea Alciato, which mark the start of legal humanism.
