Tuberculosis of breast is a rare disease which is difficult to differentiate from carcinoma of breast. The involvement of breast can be primary or secondary to some focus in body. A case of secondary tuberculosis of right breast in a 21-year-old female from Kashmir, India, is being reported. Presentation was as a painless discharging sinus of right breast. A tubercular foci of rib was the affecting source of disease. No other evidence of tuberculosis was present in the body. Resection of involved rib segment, along with the discharging sinus, was performed. The patient had antitubercular therapy for 9 months, with no recurrence seen in followup.

Primary tuberculosis of the breast is a rare disease, despite the high prevalence of pulmonary and extra-pulmonary tuberculosis in disease endemic countries. The diagnosis of primary breast tuberculosis is challenging due to it’s non-specific clinical and imaging findings. Lesions of tuberculosis such as nodular mastitis, disseminated mastitis, and sclerosing lesions clinically mimic fibroadenoma, carcinoma, and fibrocystic changes, depending on the mode of presentation. We report a case of primary tuberculosis of breast, she presented with breast lump. The diagnosis was confirmed on histopathology after wide excision of the lump. The patient recovered uneventfully and was discharged on antitubercular therapy for 9 months with follow-up protocol. The primary tuberculosis of the breast presents with a diagnostic dilemma for the surgeons. A high index of suspicion for tuberculosis is needed in diseases endemic countries, especially in the Indian subcontinent.

A middle aged woman presented with a lump and discharging sinus in her breast. On first look, the lesion seemed to be a carcinoma of breast. The patient got enlarged axillary lymph nodes with fever in the evening, night sweating and loss of weight, indicating the lesion to be of tuberculous in origin. FNAC indicated the lesion to be tuberculous but open biopsy was advised by the pathologist for confirmation. Excision biopsy of the lesion confirmed the lesion as tuberculous. Treatment with anti-tuberculous drugs cured the disease. A lump in the breast with skin involvemennt and axillary lymph node enlargement may be considered a malignant tumour on first sight, but tuberculosis cannot be ruled out, especially if a sinus is present along with systemic manifestations of the disease. doi: 10.3329/taj.v21i1.3224 TAJ 2008; 21(1): 73-76

Background: Tuberculosis of the breast, which was once upon a time a rare disease, has become quite common especially in the developing world, where tuberculosis is still a major health care problem. The condition can mimic a variety of other conditions of the breast ranging from septic conditions to malignancy. Introduction: Awareness of the modes of presentation, diagnostic difficulties and treatment is essential to prevent misdiagnosis. Case descriptions: A series of 2 cases is presented to highlight the variability in presentation. Discussion: The pathology, clinical presentation, diagnostic modalities and their pitfalls along with therapeutic options are discussed. Conclusion: The differential diagnosis of tuberculosis of the breast should be kept in mind during the course of diagnosis of the entire spectrum of breast diseases ranging from sepsis to cancer.

Background: Tuberculosis of breast is quite rare in relation to all breast pathology that we encounter in our daily practice. Ever since it was first described in 1829 many reports have been published. There has been an increasing incidence since the resurgence of AIDS. Methods: This study was done to see the incidence, presentation and investigative modality of tuberculosis of breast in our hospital. Results: A total of 986 patients out of which 585 were fibrocystic breast disease, 238 were malignant, 84 cases presented as abscess, followed by 45 cases as galactocele. 34 cases were diagnosed as tuberculosis of breast. Presentation of tuberculosis of breast was lump 19cases. 9 cases presented as abscess of breast, 4 cases had discharging sinus out of which 2 had developed them after being operated elsewhere for benign breast lump and abscess. Core needle biopsy was done in 4 cases with 2 cases coming out to be positive. Incisional biopsy was done on 2 cases which had come with mutilated breasts and they were positive for tuberculosis. 2 patients underwent excision of small lump diagnosed as fibroadenoma on FNAC, which came out as tuberculosis on biopsy. FNAC of the axillary lymph node showed a positivity rate of 43%. 9 cases were negative. Conclusions: Thus, breast lesions has to be dealt with carefully keeping in mind tuberculosis of breast, as it is a potentially curable disease and we can avoid a lot of morbidity if we can diagnose and start early ATT.

Mammary tuberculosis is a rare clinical entity, often mimicking breast cancer or abscesses. A 26-year-old female presented with a painful left breast mass and an associated sinus tract with purulent discharge. Physical examination revealed a tender, erythematous, firm left breast mass with an associated sinus tract opening with discharge within the upper outer quadrant of her left breast with a clinically palpable left axillary lymphadenopathy. Ultrasound revealed nonspecific findings and mammography could not utilize due her young age. A surgical procedure was planned and the left breast mass was completely removed by an open surgical biopsy including left axillary lymphnodes and the hystopathological evaluation of the specimen revealed findings consistent with mammary tuberculosis whereas no other nidus for tuberculosis were found within the organs including lungs. Primary mammary tuberculosis should be considered in the differential diagnosis of any case of a painful breast mass that appears refractory to conventional therapy.

Aim: To evaluate the clinico-pathological profile of the patients with tuberculosis of the breast. Materials and Methods: The clinico-pathological data of the patients with tuberculosis of the breast, who were reviewed from June 2004 to June 2011 were retrospectively analyzed. Results: Sixteen patients (2.01%) had tuberculosis of the breast out of 796 patients with breast lesions. A breast lump was the presenting symptom in 6 patients and a lump with a sinus was the symptom in 5 patients. Fine needle aspiration (FNAC) and biopsy were the diagnostic methods which were used. All the patients were treated with antitubercular treatment (ATT). Three patients needed additional surgical procedures. Conclusion: Tuberculosis of the breast is a rare disease, which presents most commonly as a lump. As it mimics carcinoma of the breast, it can be a diagnostic dilemma. FNAC and biopsy are the mainstay of the diagnosis and imaging also offers little benefit. ATT is an effective line of management, along with a limited role of surgery.

This study examines an assemblage of 65 stone artefacts, predominantly whetstones. The remaining items include loom weights, smoothers, pestles and a stone ball. Whetstones are found in large numbers at medieval sites. These common household implements were widely used for sharpening knives, shears, axes, hooks, arrowheads and spearheads. There is an interesting group of weights made from flat, oval stones, each featuring two symmetrically spaced grooves. It is difficult to determine exactly what they were used for: they may have served either as loom weights or fishing net weights (sinkers). Their discovery at a coastal settlement, where fishing played a major role, suggests that the latter option is more likely. The stone ball is probably a gaming piece. Stone pestles and smoothers were standard household utensils that had a relatively wide range of uses.
