The urachus or median umbilical ligament is a normal embryonic remnant of the primitive bladder dome and extends upward from the anterior dome of the bladder toward the umbilicus. The tubular urachus is normally changed into a fibrous band after birth. If this regression is incomplete, disorders of the urachus can appear. Umbilical-urachal sinus occurs when the umbilical end of the urachus fails to obliterate and persists as a fusiform outpouching structure just below the navel. Periumbilical pain and tenderness with a periodic discharge through the navel and a wet umbilicus are usually reported in patients with an umbilical-urachal sinus. We report the case of a 40-year-old, diabetic, and hypertensive male presenting with recurrent episodes of pus discharge from the umbilicus. Further, evaluation with a computed tomography confirmed the presence of urachal sinus. We discuss the management of the same, along with a brief review of the literature.

Urachus or the median umbilical ligament is a fibrous strand connecting umbilicus to bladder, representing embryologic remnant of cloaca and allantois. Urachal anomalies are infrequent in adult population. Moreover they have a different course in adults than pediatric age group in which they are more common, frequently involute and have a benign course. These remnants are prone to infection and development of malignancy. A proper diagnostic workup by clinical and imaging tools is required. We present a case report of a urachal sinus complicated with abscess in an adult with brief review of the literature.
Urachal sinus: An atypical case and review of the literature, 2023
Urachal sinus is a rare type of urachal abnormality. It happens because of blind focal dilation at the umbilical end and has increased risk of infection. We report the case of a 23-year-old female with abdominal pain and umbilical discharge. Ultrasound detected a possible infected urachal sinus which was initially treated with antibiotic therapy. Urachal sinus excision and laparoscopic bladder raffia was later performed with no recurrence at present. Diagnosis of this pathology is essential given that surgery is curative and avoids complications such as neoplastic transformation.

We report an infant who presented with a discharging single-lumen, tubular, and mucosa-lined umbilical mass and a large central abdominal mass, inferior to the umbilicus. At operation, the mass was discovered to be a prolapsed, open, and inflamed urachal sinus leading to a pyourachus. Primary total excision from dome of the bladder to the umbilicus led to smooth recovery with excellent results.

Patent urachus is an uncommon congenital anomaly caused by persistence of embryologic communication between bladder and umbilicus. The traditional surgical approach has been open for years but laparoscopic approach has become an effective and minimal invasive technique to treat urachal remnants. A 11 years old child presenting as umbilical discharge was diagnosed with patent urachus and treated by laparoscopic excision. There was no intra or postoperative complications. Patient was discharged on post-operative day 1. The urachus connects the fetal allantois at umbilicus to the dome of bladder. Normally it obliterates and gives rise to median umbilical ligament. Urachus is extraperitoneal and easily viewed during laparoscopic visualization of pelvis. Urachal remnants represent a rare congenital anomaly due to failure in obliteration process. With increased urbanization and industrialization, there is an emerging global epidemic of obesity which is a forerunner of metabolic syndrome. The most significant underlying cause of the metabolic syndrome is insulin resistance and its consequences. The metabolic syndrome causes lipogenesis and fat deposition. It is one of the most important risk factors for the development of catastrophic health consequences such as coronary artery disease, myocardial infarction, diabetes mellitus Type II, fatty liver, and malignant diseases. While most of these patients are treated by pharmacotherapy, there is a growing interest in studying the effectiveness of alternative therapies, including Yoga. Yoga has been practiced in India for thousands of years. Yoga, particularly Asana and Pranayama, is effective not only in improving the overall health and well-being of an individual but also reported to be effective in managing chronic medical conditions such as hypertension, diabetes, and autoimmune disorders. Various studies have found Yoga to be not only preventive but also of therapeutic value. Yoga not only has physical elements but also consists of breathing techniques, mindfulness, meditation, and relaxation techniques; hence it not only strengthens the body (secondary to physical exercise) but also positively impacts the psychosocial well-being of individuals practicing Yoga regularly. We undertook this review to determine the effectiveness of Yoga in patients with metabolic syndrome and to know whether Yoga has therapeutic benefits in patients with metabolic syndrome. Moreover, an attempt was also made to determine whether there was a significant reduction in the risk of secondary cardiovascular complications in these patients.

There is much discussion and confusion on micro-credentials within the world of work, particularly in the space of learning management and experience platforms, and in various educational, quality assurance and other institutional and policy contexts¹. Within the world of work, micro-credentials are linked with ideas of just-in-time skills, a future-fit and agile workforce ecosystem, and the future skills-based organisations. Another example is the link with the suggestions for an internal talent marketplace, where technology platforms are seen as the enabler for matching (1) the supply of skills and time available in or to the organisation with (2) the organisational demand in terms of work and projects that need to be achieved. Micro-credentials are seen as part of the solution for the skilling-whether up, multi, or re-skilling-that is required for the future-fit and agile organisation. However, we find that there are many approaches to micro-credentials, which are informed by different implicit and/or explicit assumptions. This could be assumptions around learning, the different forms thereof, and how these relate to each other and how these can be credentialed, organised, and managed. We also find that online platforms, such as the Massive Open Online Courses (MOOCs) and other commercial platforms (whether Coursera, EdX, Degreed or LinkedIn Learning for example), provide stand-alone courses as well as long form learning programmes. These platforms offer a hybrid of short courses, learning programmes, and qualifications linked to higher education institutions. In this Fact Sheet we explore how we may define a micro-credential, and how it relates to different forms of learning and learning spaces. We note the discussion on different quality assurance frameworks in relation to the different forms of learning and learning spaces. And we consider how as HR practitioners we can leverage these forms of learning and learning spaces as well as ensure good practice. To orient ourselves the below figure identifies aspects of micro-credentials. The figure that follows later lays out the relations to forms of learning, learning spaces, and quality assurance frameworks. The key question for HR practitioners is how we locate and position micro-credentials in the education, training, and the world of work ecosystems to meet the learning and development strategy and needs of our organisations. We need to explore micro-credentials as an ecosystem itself and in relation to the other ecosystems. This means understanding micro-credentials in all its forms and creating a differentiated as well as integrated approach to the different forms of learning.

Self-reflexive videogames are videogames designed to materialize critical and/or satirical perspectives on the ways in which videogames themselves are designed, played, sold, manipulated, experienced, and understood as social objects. This essay focuses on the use of virtual worlds as mediators, and in particular on the use of videogames to guide and encourage reflections on technical, interactive, and thematic conventions in videogame design and development.
