Background: Authorship is a pinnacle activity in academic medicine that often involves collaborat... more Background: Authorship is a pinnacle activity in academic medicine that often involves collaboration and a mentor-mentee relationship. The International Committee of Medical Journal Editors criteria for authorship (ICMJEc) are intended to prevent abuses of authorship and are used by more than 5500 medical journals. However, the binary ICMJEc have not yet been quantified. Aim: To develop a numeric scoring rubric for the ICMJEc to corroborate the authenticity of authorship claims. Methods: The four ICMJEc were separated into the nine authorship components of conception, design, data acquisition, data analysis, interpretation of data, draft, revision, final approval and accountability. In spring 2021, members of an international association of medical editors rated the importance of each authorship component using an 11-point Likert scale ranging from 0 (no importance) to 10 (most important). The median component scores were used to calibrate the pairwise comparisons in an analytic hierarchy process (AHP). The AHP priority weights were multiplied against a four-level perceived effort/ capability grade to calculate an authorship score. Results: Sixty-six decision-making medical editors completed the survey. The components had the median scores/AHP weights: conception 7.5/5.3%; design 8/8.9%; data acquisition 7/3.6%; data analysis 7/3.6%; interpretation of data 8/8.9%; draft 8/8.9%; revision 8/8.9%; final approval 9/20.1%; and accountability 10/31.8%, with Kruskal-Wallis Chi 2 = 65.11, p < 0.001. Conclusion: The editors rated accountability as the most important component of authorship, followed by the final approval of the manuscript; data acquisition had the lowest median importance score for authorship. The scoring rubric (https://tinyurl.com/eyu86y96) transforms the binary tetrad ICMJEc into 9 quantifiable components of authorship, providing a transparent method to objectively assess authorship contributions, determine authorship order and potentially decrease the abuse of authorship. If desired, individual journals can survey their editorial boards and use the AHP method to derive customized weightings for an ICMJEc-based authorship index.

PurposeTo determine sources of variation between countries in per-capita mortality from COVID-19 ... more PurposeTo determine sources of variation between countries in per-capita mortality from COVID-19 (caused by the SARS-CoV-2 virus).MethodsPotential predictors of per-capita coronavirus-related mortality in 200 countries by May 9, 2020 were examined, including age, sex, obesity prevalence, temperature, urbanization, smoking, duration of infection, lockdowns, viral testing, contact tracing policies, and public mask-wearing norms and policies. Multivariable linear regression analysis was performed.ResultsIn univariate analyses, the prevalence of smoking, per-capita gross domestic product, urbanization, and colder average country temperature were positively associated with coronavirus-related mortality. The literature on the performance characteristics of TAB versus imaging is conflicting, and several meta-analyses suggest that TAB is more sensitive than ultrasound. False-positive ultrasound results can occur. Also, diseases that mimic the symptoms, signs or imaging characteristics of GCA may only be revealed on pathology.

However, the summary section implies that temporal artery biopsy (TAB) is no longer the "gold standard" for the diagnosis of giant cell arteritis (GCA). In 2018 the European League Against Rheumatism first suggested that ultrasound and MRI can be used for the first-line diagnosis of GCA at centres with sufficient expertise in performing and interpreting these studies [2]. However, ultrasound machines cost at least £15,000-60,000 [3], and require extensive training. The July 2019 draft British Society of Rheumatology guidelines recommended that the confirmatory diagnostic test for GCA could be either a TAB at least 1 cm in length, or an ultrasound of the temporal and axillary arteries, or both [4]. Most neuroophthalmologists still prefer TAB over ultrasound for the work-up of GCA [5]. On meta-analysis the estimated sensitivity of TAB is 77% [6]. In contrast, systematic review of the literature found that the hypoechoic halo sign on temporal artery ultrasound had 68% sensitivity and 81% specificity compared with a positive TAB [7]. Atherosclerosis and other conditions can cause a false-positive halo on ultrasound [8]. TAB may reveal alternative diagnoses including syphilis, sarcoidosis, metastases, amyloidosis [9], zoster, and granulomatosis with polyangiitis [10], which may not be discovered in an expedient fashion without tissue pathology. The TABUL study [11] is referenced a dozen times by Lyons et al. to support their statement that, "investigations, along with clinician insight, produce the highest sensitivity and specificity" for GCA. However, the TAB in the TABUL study were substandard. Seven percent of the attempted TAB did not retrieve a temporal artery, and 43% of the TAB specimens were <1 cm in length.

To determine whether temporal artery biopsy (TABx) or Doppler ultrasound (US) of the temporal art... more To determine whether temporal artery biopsy (TABx) or Doppler ultrasound (US) of the temporal artery is the preferred confirmatory test for giant cell arteritis, an online survey of ophthalmologists and neurologists in North America, Europe and Israel was conducted in 2019; Canadian rheumatologists were also included. There were 406 survey participants with an estimated survey response rate of 18%. Ninety-four per cent of North American practitioners preferred TABx compared with 74% of their European counterparts. Two per cent of North American practitioners preferred Doppler US versus 24% of European physicians. Regional differences were statistically significant (p < .001).

On first meeting this kindhearted and unassuming clin ician, you might easily forget he is the op... more On first meeting this kindhearted and unassuming clin ician, you might easily forget he is the ophthalmologist in-chief of the world-renowned Hospital for Sick Children, Toronto, and arguably one of the most expe rienced neuro-ophthalmologists in North America. Ray was born in Windsor, Ont., and did his under graduate training at the University of Windsor and the University of Toronto. In 1965 he received his MD degree from McGill University, Montreal, then did an internship and a year of internal medicine at the Royal Victoria Hospital, in Montreal. He completed his oph thalmology residency at the University of Toronto in 1970 and trained in neuro-otology the following year. In 1972 he did a fellowship in San Francisco with the incomparable William F. Hoyt, followed by supple mental training in neuro-ophthalmology at the famed Queens Square, in London. Since that time he has been on staff at the Hospital for Sick Children and in 1991 became chief of the service. Ray's curriculum vitae is voluminous, with many peer-reviewed articles and book chapters. He has self lessly encouraged many residents and fellows to be first author on his impressive series of neuro-ophthalmic patients, which have taken the bulk of his career to col lect. He has been invited to lecture at almost all the Canadian eye centres as well as Guatemala City, Miami, Chicago, San Francisco and Seattle. As chief of ophthalmology at Sick Kids he oversees an army of ophthalmologists, fellows and numerous residents, orthoptists and clinical scientists. He has helped mould the Toronto pediatric ophthalmology program into one of the best, if not the best, fellow ship offering of its kind. The inferior portion of the plastic dressing can be crimped to make a channel for blood to seep through. Cold compresses can be still applied through this thin plastic dressing.

A 50-year-old man was assessed by an oncologist in 1998 for suspected "orbital lymphoma" and refe... more A 50-year-old man was assessed by an oncologist in 1998 for suspected "orbital lymphoma" and referred to neurosurgery for orbital biopsy. A right pterional craniotomy using an orbital-zygomatic approach was performed with removal of portions of the orbital roof. The surgical report indicated that "the intraorbital content appeared to be infiltrated and very, very firm" and that "biopsy times two from the extraocular muscles" was obtained. Although there was a chronic inflammatory and lymphocytic infiltrate on pathology, no lymphoma was found.

Amaç: Femoro-asetabular sıkışma (FAS) sendromu ağrı ve hareket kısıtlılığı ile beraber dejenerati... more Amaç: Femoro-asetabular sıkışma (FAS) sendromu ağrı ve hareket kısıtlılığı ile beraber dejeneratif eklem hastalığının bir nedenidir. Artroskopik yöntemler erken dönemde FAS tedavisinde etkili olarak kullanılmaktadır. Ancak eklemde dejeneratif bulguların ortaya çıkmış olması artroskopik tedavinin başarısını olumsuz olarak etkilemektedir. Literatürde, artroz gelişmiş kalçalarda artoskopik yöntemlerin başarısını araştıran çalışma sayısı kısıtlı olmakla beraber artroskopik tedavinin başarısı konusunda bir fikir birliğine varılamamıştır. Biz bu çalışmamızda kliniğimizde FAS sendromu ile beraber Tönnis evre 1-2 artrozu (OA) olan hastalarda artroskopik tedavinin kısa dönem klinik sonuçlarını araştırmayı amaçladık. Yöntemler: Kliniğimizde FAS sendromu ile beraber Tönnis evre 1-2 OA'sı olan ve artroskopik olarak tedavi edilmiş 17 hasta retrospektif olarak incelendi. Artroskopik tedavide artroskopik bulgulara göre femoral osteoplasti, asetabular tıraşlama, labral tamir, labral debridman, mikro kırık ve lig teres debridmanı yapıldı. Radyolojik değerlendirmede Tönnis evrelemesi ve OA'nın değrelendirilmesinde kullanıldı. Hastaların fonksiyonel durumları operasyon öncesi ve sonrası Harris Kalça skoru (HKS), Hip Outcome skoru (HOS) ve Short Form 12 ile değerlendirildi. Results: At the last follow up, the mean HHS and HOS increased from 53.2 and 65.3 to 82.4 and 88.1, respectively. In total, 16 of 17 (94%) patients reported that they were satisfied with the outcome of the surgery. One patient reported that the symptoms relieved for 6 months, but returned. He was offered arthroplasty but he refused. Conclusion: In the current study, it was observed that arthroscopic treatment could help to relieve patient symptoms in early stage OA with FAI.

The surgical report indicated that "the intraorbital content appeared to be infiltrated and very, very firm" and that "biopsy times two from the extraocular muscles" was obtained. Although there was a chronic inflammatory and lymphocytic infiltrate on pathology, no lymphoma was found.

Artroskopik yöntemler erken dönemde FAS tedavisinde etkili olarak kullanılmaktadır. Ancak eklemde dejeneratif bulguların ortaya çıkmış olması artroskopik tedavinin başarısını olumsuz olarak etkilemektedir. Literatürde, artroz gelişmiş kalçalarda artoskopik yöntemlerin başarısını araştıran çalışma sayısı kısıtlı olmakla beraber artroskopik tedavinin başarısı konusunda bir fikir birliğine varılamamıştır. Biz bu çalışmamızda kliniğimizde FAS sendromu ile beraber Tönnis evre 1-2 artrozu (OA) olan hastalarda artroskopik tedavinin kısa dönem klinik sonuçlarını araştırmayı amaçladık. Yöntemler: Kliniğimizde FAS sendromu ile beraber Tönnis evre 1-2 OA'sı olan ve artroskopik olarak tedavi edilmiş 17 hasta retrospektif olarak incelendi. Artroskopik tedavide artroskopik bulgulara göre femoral osteoplasti, asetabular tıraşlama, labral tamir, labral debridman, mikro kırık ve lig teres debridmanı yapıldı. Radyolojik değerlendirmede Tönnis evrelemesi ve OA'nın değrelendirilmesinde kullanıldı. Hastaların fonksiyonel durumları operasyon öncesi ve sonrası Harris Kalça skoru (HKS), Hip Outcome skoru (HOS) ve Short Form 12 ile değerlendirildi. Results: At the last follow up, the mean HHS and HOS increased from 53.2 and 65.3 to 82.4 and 88.1, respectively. In total, 16 of 17 (94%) patients reported that they were satisfied with the outcome of the surgery. One patient reported that the symptoms relieved for 6 months, but returned. He was offered arthroplasty but he refused. Conclusion: In the current study, it was observed that arthroscopic treatment could help to relieve patient symptoms in early stage OA with FAI.
