We recorded single-neuron activity in primary somatosensory (S1) and dorsal premotor (DPC) cortex while trained monkeys performed a challenging pattern discrimination task. We found a faithful representation of the stimuli in S1 and a heavily transformed, more abstract, and highly varied set of responses in DPC. Most notably, in addition to memory-related activity and responses encoding the monkeys’ choices, the DPC data included a large set of categorical neurons that code specific combinations of past and present stimuli and, at the same time, are strongly predictive of the monkeys’ behavior.

The basic idea is to use the Lagrangian relaxation to get a decomposition of the problem that emulates the necessary decentralization process. The feasibility of this procedure is analysed using a simplified model of the problem. In particular, the solution obtained in this decentralized way is contrasted with the numerically exact solution of the problem by evaluating the duality gap. The examples presented suggest that in this way reasonably approximate solutions can be obtained, to improve those obtained through empirical procedures used by the market operators.

The aim of the present study was to estimate the residual risk of blood-borne viral infections in Spain. STUDY DESIGN AND METHODS: Incidence rates of seroconversion for HIV, HBV, and HCV were calculated among 673,018 persons who donated blood more than once (repeat donors), from 1997 through 1999 at 22 blood donation centers (for a total of 2,464,964 allogeneic blood donations and 1,052,752 person-years). RESULTS: Incidence rates per 100,000 person-years and their 95-percent CIs were as follows: for HBV, 8.36 (5.24-12.62); for HIV, 3.23 (2.24-4.52); and for HCV, 3.70 (2.63-5.07). After adjusting incidence rates for the estimated duration of the infectious window period for each virus, the residual risk per unit transfused was estimated at 1 in 513,000 for HIV, 1 in 74,000 for HBV, and 1 in 149,000 for HCV. The introduction of new screening test based on NAT would have reduced these risks by 27 to 50 percent for HIV, by 42 percent for HBV, and by 62 to 65 percent for HCV. CONCLUSION: The residual risks of transmission of HIV, HBV, and HCV in Spain are similar to those reported in other countries and should be further reduced in the future.

The usual treatment is systemic chemotherapy followed by optional resection of the mass. We introduce the case of a 73-year-old male patient with multiple comorbidities in whom retroperitoneal carcinoma recurrence of 31 mm was diagnosed via positron emission tomography-computed tomography scan with 18-fluorodeoxyglucose about 5 years after he had undergone RNU owing to urothelial carcinoma of the upper urinary tract. The patient was treated with computed tomography-guided percutaneous radiofrequency ablation. Later scans with contrast controls showed lack of contrast uptake and a decrease of the lesion's size. Twenty-four months after the procedure, the patient is free of the disease. To date, this is the first case of recurrence of urothelial carcinoma that was treated with percutaneous radiofrequency ablation, thus establishing an alternative to chemotherap...

Studies suggest that the quality of VIA is highly variable. Over 36 000 women were screened with Pap and VIA in the TATI (Tamizaje y Tratamiento Inmediato de Lesiones Cervico-uterinas) project conducted in Amazonian Peru. Within a nested study to compare several screening techniques (C-TATI), a total of 5435 women were additionally screened with liquid-based cytology (LBC) and high-risk human papillomavirus testing (HR-HPV). We investigate the variation of positivity rates of VIA, Pap, LBC and HR-HPV in C-TATI and of VIA in the full TATI intervention. At the screening visit, midwives collected three cervical samples for Pap, LBC and HC2 before performing VIA. The dispersion factor…

This cost will become disproportionately high when a patent cost is introduced by the manufacturers for each unit of blood tested [1]. Since July 1999, all allogeneic blood donations in the Valencian Community Blood Transfusion Centre (Spain) have been tested by reverse transcription–polymerase chain reaction (RT–PCR) (Ampliscreen HCV 2·0; Roche Diagnostics, Branchburg, NJ) analysis of minipools of 44 or 22 donations.

The multidrug resistant HL60 cell line, HL60/ADR, was less sensitive than parental cells to cytostatic activity of low (0.4-2 ng/ml) doses of PMA. However, during treatment with standard differentiating doses of PMA (10 ng/ml), no difference between the two cell lines in cytostasis and differentiation was found. Downregulation of c-myc and upregulation of p21/Waf1 proteins showed the same time-course in both cell lines. The bcl-2 mRNA was rapidly downregulated while bax and bclxL gene expression was not altered in both differentiating HL60 and HL60/ADR cells. Significant downregulation of bcl-2 protein occurred only in parental HL60 cells. In HL60/ADR, despite rapid cessation of bcl-2 protein synthesis, almost no change in steady-state bcl-2 protein level was found. The lack of bcl-2 protein downregulation was a result of the prolonged half-life of this protein in HL60/ADR cells. Thus, although downregulation of bcl-2 mRNA is coupled to differentiation, actual loss of bcl-2 protein is not required for accomplishment of the differentiation program.

STUDY DESIGN: Women with preterm premature rupture of membranes and a singleton gestation at 24 to 34 completed weeks were randomized to receive either piperacillin 3 gm or placebo intravenously every 6 hours for 72 hours and were managed conservatively until spontaneous delivery, chorioamnionitis, or fetal distress. RESULTS: Between January 1987 and January 1992, a total of 75 patients were randomized to receive piperacillin (n = 38) or placebo (n = 37). There were no differences between the piperacillin group and the placebo group in mean gestational age at randomization (30.2 ± 3 vs 30.3 ± 2.9 weeks). However, a greater number of patients had pregnancy prolonged beyond 7 days (42.1 % vs 10.8%, P = 0.005) and the mean latency period was significantly prolonged (11.4 ± 18.8 vs 6.1 ± 13.6 days, p = 0.001) in the piperacillin group compared with the control groups. CONCLUSIONS: Use of intravenous piperacillin for 72 hours in preterm premature rupture of membranes significantly prolongs the latency period between membrane rupture and delivery.
