Study Design: Experimental analytical study Place and Duration of Study: Dow Research Institute of Biotechnology & Biomedical Sciences, Dow International Medical College, Karachi from 1st January 2019 to 31stDecember 2019 Methodology: 40 ml of amniotic fluid was extracted from 10 full term women at the time of elective caesarean. Using cell culturing, inverted phase contrast microscopy and flow cytometry techniques in addition to immune-florescence the nephron progenitor cells were identified. Results: The mean age of women was 30.3±0.4 years. Out of total 10 million WT1 expressed in three samples 1.4 million nephron progenitor cells were identified. Study Design: Prospective, Consecutive sampling. Setting: Children Hospital and Institute of Child Health, Faisalabad. Period: 6 Months June 2019 to November 2019. Material & Methods: All admitted children of age 1-16 years with strong clinical suspicion of typhoid fever and with no comorbidities were included in this study. Blood cultures and other relevant investigations were performed to rule out other causes of fever. After identification of organism as Salmonella typhi, antibiotics susceptibility testing was done by disk diffusion technique and zones of inhibition were interpreted according to clinical and Lab standard (CLSI) guidelines. An Electrocardiography (ECG) based processor for eight Cardiac arrhythmias (CA) detection with sm... more An Electrocardiography (ECG) based processor for eight Cardiac arrhythmias (CA) detection with smart priority logic is presented to minimize the false alarms. The processor utilizes a Multi-Level Linear Support Vector Machine (ML-LSVM) classifiers with one-vs-all approach to distinguish the different CAs. The classification is solely based on 5 features including R-wave, S-wave, T-wave, R-R interval and Q-S interval. The processor employs a priority logic to prioritize the detected conditions if more than one condition are detected. The system is implemented using CMOS 180nm with an area of 0.18mm2 and validated using 83 patient’s recordings from Physionet Arrhythmia Database and Creighton University Database. The proposed processor consumes 0.91uW with an average classification accuracy of 98.5% while reducing the false alarms by 99%, which is 30% superior performance compared to conventional systems.

Hypomagnesaemia accelerates the rate of diabetic complications. Objective: To analyze the association of magnesium levels in DM (Type 2 Diabetes Mellitus) with and without complication of Hypertension. Design: It was a comparative analytical study. Setting: This research was carried out in medicine department, Peoples Medical College Hospital, Nawabshah from March 2016 to February 2017. Samples: 245 patients with Type 2 DM after fulfilling the selection criteria were included, out of them 123 with hypertension and 122 without hypertension. Methods: After a short-lived consultation, all the subjects were categorized for variable analyses like age, gender, Type 2 DM with and without hypertension, duration of diabetes and presence of hypomagnesaemia. Joint National Committee (JNC 8) classification of Hypertension was applied to collect data. Blood samples were drawn for research purpose in fasting state for serum magnesium level analyses on the basis of hypertension and without hypertension. Results: In 245 diagnosed Type 2 diabetic patients, 160 were males and 85 were females. Out of them, 123 were hypertensive while 122 were non hypertensive. Normal magnesium was present in 120 (48.97%) and low magnesium was present in 125 (51.02%) patients overall. A decreased serum level of magnesium was observed in 52.04% hypertensive subjects with diabetes and 50% non-hypertensive subjects with diabetes. Conclusion: Frequency of decreased serum magnesium

To compare the stone free rate at one week after extracorporeal shock wave lithotripsy (ESWL) and... more To compare the stone free rate at one week after extracorporeal shock wave lithotripsy (ESWL) and ureterorenoscopic (URS) manipulation for proximal ureteric stone (10 - 15 mm size). Randomized controlled trial. Sindh Institute of Urology and Transplantation (SIUT), Karachi, from August 2010 to February 2011. One hundred and ninety patients with 10 - 15 mm proximal urteric stone, in each group were treated with ESWL and ureterorenoscopic manipulation by using an 8.0 or 8.5 Fr semi rigid ureteroscope. Intracorporeal lithotripsy was performed by using pneumatic lithoclast. The stone free rate were compared between groups by considering size of stone at one week after procedure. The success rate, retreatment rate, auxiliary procedure and complication rate were compared in each group. Success rate was 49.2% for ESWL and 57.8% for URS (p = 0.008). Serum sickness-like reaction is a rare immunological condition which may develop following exposu... more Serum sickness-like reaction is a rare immunological condition which may develop following exposure to certain drugs such as penicillins, cephalosporins, and trimethoprim-sulfamethoxazole, among many others. It is described classically as a type III hypersensitivity response to heterologous proteins. Its true mechanism is still unclear. We present a case of serum sickness-like reaction to clarithromycin, a commonly prescribed drug for the treatment of respiratory tract infections. The patient had been taking this drug for 3 days when she experienced generalized body aches, rash, arthralgia, and shortness of breath, prompting presentation to the emergency department. Laboratory studies showed decreased C4 and total complement with a slightly elevated sedimentation rate. After exclusion of other possible causes, the diagnosis of serum sickness-like reaction was made. The patient responded well to nonsteroidal antiinflammatory medication, antihistamines, and a short, tapering dose of steroids. To our knowledge, serum sickness-like reaction to clarithromycin has never been reported previously. This case emphasizes the need for increased clinical awareness of such an adverse outcome to clarithromycin use.

EMBRYONAL rhabdomyosarcoma is a malignant tumour of rhabdomyoblasts, the microscopic pattern of w... more EMBRYONAL rhabdomyosarcoma is a malignant tumour of rhabdomyoblasts, the microscopic pattern of which simulates that of embryonic muscle cells. It is a rare condition that tends to occur in the head and neck in children and young adults. In the ear it is extremely rare. The first case of embryonal rhabdomyosarcoma of the ear was reported by Soderberg in 1933. However, most cases of rhabdomyosarcoma were labelled as sarcomas before 1950. Various reports of this entity were made by Gellerstedt (1938), Maconie (1944), Karatay (1949), Stobbe and Dargeon (1950), Holman (1956), Blanchard and House (1957), Horn and Enterline (1958) and Mehra (1960), all in. young children who died within a few months of the first sign of the tumour. Mason and Soule (1965), reporting on 88 cases of embryonal rhabdomyosarcoma of the head and neck treated at the Mayo Clinic from 1910-1964, found only five cases in the external auditory canal, mastoid and the temporal area. Davison(1966) referred to 15 cases of rhabdomyosarcoma of middle ear in the world literature and described four cases in children between the ages of 18 months and 7 years. Jaffe, Fox and Batsakis (1971) reviewed 37 cases treated at Michigan Medical Centre from 1949 to 1969 and described three of their own. The first report from India was by Verma and Marwaha in 1972. Deutsch and Felder (1974) reviewing the world literature could collect only 73 cases arising in the middle ear or mastoid region. This included five cases cited in the Japanese literature by Kawabe et al. in 1971. The authors reported five cases of their own. Three cases were reported by Fajemisiw in 1974 from University College Hospital, Ibadan, Nigeria. In 1976 Pandhi et al. reported four cases and Pahor one case, thus bringing the total of reported cases to 86. The following two cases are reported because of the rarity of the condition.

Different elicitors including the fungus Aspergillus niger (0.2 g/L), methyl jasmonate (MeJA, 100 µM/L), and silver nanoparticles (1 µg/L) were added, individually and in combination, in a hydroponic medium. The application of these elicitors in hydroponic culture significantly increased the concentration of photosynthetic pigments and total phenolic contents. The treatment with MeJA (methyl jasmonate) (100 µM/L) and the co-treatment of MeJA and AgNPs (silver nanoparticles) (100 µM/L + 1 µg/L) exhibited the highest chlorophyll a (29 µg g−1 FW) and chlorophyll b (33.6 µg g−1 FW) contents, respectively. The plants of a drought tolerant sunflower hybrid viz. Hysun-33 were grown to flowering stage under normal conditions and later subjected to drought by withholding irrigation at 50 days after sowing (DAS) for two weeks. Foliar spray of K (1%) and Ct (0.1%) were applied to both normal and water stressed plants, whereas the control plants were treated with water spray (control). The role of exogenous K and Ct application in improving drought tolerance of sunflower was determined by measuring various yield parameters. The obtained data was statistically analyzed following Fisher’s ANOVA technique and difference among the treatments was tested by Tukey test at probability level of 5%. Its a rare condition which is rarely encountered when the meniscus has a bucket handle on both sides of the compartment. Here we present the case of a 28-year-old male cricket player with a total ACL rupture, a double Posterior Cruciate ligament (PCL) sign on MRI (Magnetic Resonance Imaging), an uncommon occurrence of a complex bucket handle rupture of the medial meniscus with a double PCL sign. It was a complete ACL tear and displaced bucket handle medial meniscus that was also visible on the magnetic resonance imaging. In this report, double PCL sign was shown on MRI and that was confirmed during the arthroscopic procedure. Keywords: Complete ACL tear, combined medial & lateral bucket handle, meniscus tears.
