Background: One of the psychological stressors for the patients admitted to the coronary care uni... more Background: One of the psychological stressors for the patients admitted to the coronary care units (CCUs) is being away from family members and lack of visiting time. Also, one of the special needs of families is to visit patients during their hospitalization period. In order to enforce visiting rules in CCUs, it is important to take the needs of all staff, patients, and visitors into consideration, and a visiting policy should be adopted that ensures the most effective visiting system. Objectives: The present study aimed to design a visiting policy based on the challenges of the CCU ward using an interactive approach. Methods: This qualitative research was conducted with a participatory action research (PAR) approach in 2016 at Ganjavian Hospital, Dezful, Iran. Content analysis was performed based on the method proposed by Graneheim and Lundman (2004). Forty stakeholder participants were included in focus groups consisting of 3 to 7 people using a purposeful sampling method. Results: In order to determine the current situation, after analyzing the extracted codes in the focus groups and according to the similarities and conceptual content, 36 subcategories, 11 categories, and 3 subthemes, including improper physical factors, problems related to human factors, and improper policy quality, were obtained. After determining the current status of the visiting policy and problems, the new policy was developed by the planning group consisting of a representative from each of the focus groups. Conclusions: In the new visiting policy, efforts have been made to improve the quality of visiting by providing more opportunities for patient-companion and doctor-companion visiting, focusing on ensuring complete and timely information, assigning rooms for doctor-companion and patient-companion visiting, and paying attention to the required equipment. This program can be used by other planners while adjusting its items according to their conditions.

Aim: To investigate the effect of home-based cardiac rehabilitation on functional capacity, healt... more Aim: To investigate the effect of home-based cardiac rehabilitation on functional capacity, health behavior, and risk factors in patients with acute coronary syndrome in China. Methods: Eighty patients with acute coronary syndrome were enrolled in this prospective randomized controlled study. Patients in the cardiac rehabilitation group (n=52) received home-based cardiac rehabilitation with a heart manual and a home exercise video for 3 months and patients in the control group (n=28) received only routine secondary prevention. The 6-min walk distance, laboratory test results, healthy behavior (questionnaire), quality of life (12-item Short Form Health Survey), anxiety (7-item Generalized Anxiety Disorder Questionnaire), and depression (9-item Patient Health Questionnaire) were evaluated at the beginning and after treatment for 3 months. Results: Compared with baseline data, 52 patients who participated in cardiac rehabilitation had longer 6-min walk distance (515.26±113.74 m vs 0.445.30±97.92 m, P<0.0002), higher proportions of "always exercise" (78.26% vs. 28%, P<0.05), "always limit food with sugar" (65.22% vs 12%, P<0.05), "always eat fruits 200-400 g every day" (82.61% vs. 4%, P<0.05). and "always eat vegetables 300-500 g every day" (21.74% vs. 12%, P<0.06) after treatment for 3 months. The low-density lipoprotein cholesterol control rate (52.17% vs. 28%, P<0.05) and the systolic blood pressure control rate (100% vs. 68%, P<0.05) were also significantly increased after treatment for 3 months in the cardiac rehabilitation group. No significant increase was found in the control group after treatment for 3 months. No cardiac-event related to home exercise was reported in both groups. Conclusion: Home-based cardiac rehabilitation is a feasible and available cardiac rehabilitation mode in China.

Objective: Venous thromboembolism, including deep vein thrombosis (DVT) and pulmonary embolism (P... more Objective: Venous thromboembolism, including deep vein thrombosis (DVT) and pulmonary embolism (PE), is a common complaint in critically ill patients. Therefore, the present study was conducted to determine the effect of nursing interventions, based on the Wells results, on the incidence of DVT in intensive care unit (ICU) patients. Results: The incidence of DVT in both groups showed that 2 patients of the control group who were identified to be at risk using the Wells score were diagnosed with DVT while none of the patients of the intervention group experienced DVT. The present study showed that 22.2% of the patients of the control group suffered from non-pitting edema, which was significantly different from the intervention group (p=0.005). Conclusion: The results of the present study showed that using the Wells score for early identification of the at-risk patients and nursing interventions based on this score's results is helpful in the prevention of DVT. Appropriate nursing interventions were also effective in reducing the incidence of non-pitting edema in the lower extremities.

Objectives: To evaluate the efficacy of an intervention based on the theory of planned behavior for improving foot care in patients with type II diabetes in 2013 in Ahvaz, Iran. Patients and Methods: In this clinical trial, 69 patients (30-60 years old) who were referred to Golestan Hospital Diabetes Clinic between June and May 2013 were selected randomly and divided into 2 groups (intervention and control). The control group received the clinic's routine training. In contrast, a 4-week educational program based on the theory of planned behavior was held for the intervention group. Data were collected before and after the intervention using a questionnaire developed for this study that assessed components of the theory of planned behavior, and foot care. Data were analyzed with SPSS version 19 using paired and independent t-tests and chi-squared. Results: The two groups showed no significant difference in terms of demographics or in the mean scores of other variables before the intervention (P > 0.05). After the intervention, a significant increase was observed in the mean scores of attitudes, and foot care performance in the intervention group, and the intervention group had higher scores than did the control group (P < 0.05). Conclusions: An intervention based on the theory of planned behavior was effective in improving the foot care in diabetic patients.

Objectives: The current study aimed to evaluate the effect of an educational intervention on knowledge, attitude and practice of foot care among the patients with type 2 diabetes. Patients and Methods: In this randomized controlled clinical trial, 69 patients aged 30-60 years who were referred to an Iranian diabetes clinic were randomly selected and divided into two groups, intervention and control. The control group just received the routine training of the center. A three week educational program was held for the intervention group. Data were collected before and after the intervention by a researcher-made questionnaire and analyzed with SPSS version 19 to apply dependent and independent t-tests and chi-square test. Results: There was insignificant difference between the two groups in terms of demographic variables and the mean scores of other assessed variables before the intervention (p > 0.05). After the intervention a significant increase was observed in the mean scores of knowledge, attitudes, and foot care practice of the intervention group compared to those of the control group (p < 0.05). Conclusions: The educational program was effective on the foot care performance of the patients with diabetes.
Background and aim: Among the complications of diabetes, foot problems such as ulcers, infections... more Background and aim: Among the complications of diabetes, foot problems such as ulcers, infections of the feet and lower limb amputation is increasing due to nonconformity of safety and preventive issues. Therefore, this study has been conducted with the aim to evaluate the foot care status based on the theory of Planned Behavior in diabetic patients. Methods: In this cross-sectional study, 69 type II diabetic patients referred to the Golestan Hospital Diabetes Clinic of Ahvaz during a three months period from June to September 2013 were selected randomly. Data were collected by means of a three-parts researcher made questionnaire(59 items) including medical and demographic data, knowledge, planned theory's structures (attitude, subjective norm, perceived behavioral control and intention) and foot care performance were evaluated. Results: The findings showed that 53.6% of participants had poor knowledge in foot care practices, whereas 79.71% had high attitudes toward these practices. 52.17% reported pressures and social support in high level and 52.1% stated an average level of the control on foot care practices. 79.7% had a high intention toward these practices, while 43.4% had poor performance. Also, the results showed that there is a direct and significant correlation between the mean of knowledge and performance and all theory's structures (P<0.05). Conclusion: The results of this study emphasis on necessity attention to a series of effective factors in diabetic patients' behavior change and could be the basis for developing theory-based educational interventions with the aim of improving the foot care.

This study aims to determine and compare employees’ anxiety levels and perceived stress before and after the accreditation program in private hospitals in Ahvaz City, Iran. Methods: This research was a quantitative, observational, and longitudinal study. A total of 456 employees of Ahvaz private hospitals were recruited by convenience sampling. The state-trait anxiety inventory (STAI) and the perceived stress scale-14 (PSS-14) were used to measure the level of anxiety and stress among hospital employees one month before and one month after implementing the hospital accreditation program.
