Various tissues were used as a second layer after urethroplasty to improve outcomes. The aim of the current study was to demonstrate the efficacy of the corpus spongiosum and its capability to be the one and only 2nd layer after urethroplasty. Patients and methods: A total number of 30 distal cases were included with an age ranging from 6-120 months most of them was coronal/ sub coronal, non-circumcised, with developed proximal urethra and spongiosum and absent ventral curvature. Urethroplasty technique was either tubularized incised plate (TIP) or Thiersch-Duplay and the second layer was always the mobilized then approximated spongiosal pillars. The mean operative time was 87.80 (± 8.56) min and patients were discharged at the same day of surgery with a three month follow up period and monitoring of signs of any post-operative complications. Results: Results at one month showed overall complication rate of 16.7% including spraying of urinary stream, meatal stenosis, Urethro-cutaneous fistula (1 mm in size at the coronal sulcus), skin sloughing and slipped urinary catheter in one case (3.3%) for each of them. No glans dehiscence, urethral diverticulum, residual chordee, symptoms of lower urinary tract infection or major aesthetic issues were encountered. Conclusion: Spongioplasty alone can be used as a second layer if a well-developed, strong spongiosum is available.

The transoral approach targeted mainly the concept of scarless surgery, in addition, to keeping the marginal mandibular nerve safe. The transoral approach faces many limitations as pathology type, lump size, lump mobility, and enough workspace. Patients and methods: This prospective study was carried out on 20 consecutive patients. All of them were presented with submandibular salivary gland swelling. All swellings included should be mobile, 2 cm or less, and the patient showed a positive 3fingers test. Results: In this study, 13 patients showed evidence of calcular submandibular sialadenitis, 4 patients with non calcular submandibular sialadenitis, and 3 patients with pleomorphic adenoma. All cases were presented with small mobile gland. The mean lump size was 16.4 mm (12-20 mm). all patients passed the 3finger test to evaluate maximal mouth opening. Two patients developed wound infection and showed delayed wound healing. Five cases developed abnormal tongue tip sensation. No patients developed limitations in tongue movement. Conclusion: In selected cases, the transoral approach can be a good alternative, especially for those patients who are strongly appreciating scarless operations as young age females.

Recurrence and treatment resistance occurs in one-third of cases, triggering them to the progressive stage of DLBCL after treatment. Detection of novel predictive and prognostic biomarkers leads to improvement of its treatment and prognosis. Aim of the study: To assess the prognostic roles of protein expression of myeloid differentiation factor 88 (MYD88) and transducin (β)-like receptor 1 (TBLR1) in tissues of DLBCL patients. Material and methods: In the current study we included tissues from 100 cases of DLBCL. For immunohistochemistry, tissues were stained with MYD88 and TBLR1. We followed patients for about 3 years, and then we correlated their expression with clinicopathological and prognostic parameters. Results: Higher MYD88 and TBLR1 expressions were associated with presence of B symptoms, fever, night sweat, advanced stage, bone marrow involvement and bulky nodal size, presence of extra-nodal extension, unfavourable relapse-free survival, and unfavourable overall survival rates (p < 0.001). Conclusions: overexpression of MYD88 and TBLR1 expression was present in DLBCL patients and was associated with unfavourable clinicopathological and prognostic parameters.

This study was performed at Benha university hospitals and Kasr El Aini University Hospitals on 30 early complicated cases post sleeve gastrectomy at the period from 1/1/2018 to 30/6/2018. Patients and Methods: This study was performed at Benha university hospitals at the period from 1/1/2018 to 30/6/2018 on 30 early complicated cases post sleeve gastrectomy. Consent of the patients was taken and data were collected included: age, sex, BMI, and comorbid conditions at the time of admission. Results: According to the co morbidities, eight of the studied cases (26.7%) suffered from hypertension, and 22 others (73.3%) didn"t suffer from hypertension, also half of the studied cases (50.0%) suffered from diabetes. As regard the complications that the patients suffered after the operation in our study, 7 patients (23.3%) suffered of Tachycardia, 5 patients (16.7%) suffered of Vomiting, 4 patients (13.3%) suffered of Hemorrhage, Leakage and Hematemsis, 3 patients (10.0%) suffered of Wound infection, 2 patients (6.7%) suffered of Gastric stenosis, Conversion to open and Melena. On the other side, none of the patients suffered from DVT, Gastric volvulus, pulmonary embolism and Mortality. Conclusion: LSG is one of the most effective surgical procedures in weight loss and is considered safe if its complications are diagnosed and managed early and we recommended to do this study on largenumber of patients to support our results and to know more about the sleeve gastrectomy complications to manage them as early as possible to get benefit from sleeve gastrectomy and to be used on a wide range.

Aim of the current study was to compare between classic inguinal approach and scrotal single incision approach to perform orchiopexy for management of un-descended testis in infants and to clarify which is better; inguinal orchidopexy or scrotal orchidopexy. Patients and methods: we included forty male patients with undescended testis who were divided into two groups; group A included twenty patients and they were surgically managed with single transscrotal orchiopexy and group B included twenty patients and they were surgically managed with classic inguinal approach by two incisions (inguinal and scrotal). We assessed operative time and complications as wound infection, recurrence and testicular atrophy. Results: Operative time in group A was shorter than the operative time of group B which was managed by the inguinal approach (p=0.004). Immediate post-operative complications as; skin site infection, hematoma and edema, in group A were slightly more than the complications in group B (p=0.002). group A showed no recurrence or testicular atrophy after six months while group B showed 2 cases of recurrence and one case of testicular atrophy (p=0.005). At three months after surgery, cosmetic results and patients satisfaction of the incision scar was higher in group A than group B (p<0.001). Conclusions: performing orchiopexy through single scrotal incision approach is better than the approach of inguinal incision regarding operative time, rabid healing time, parents' satisfaction, fewer long-term complications, less incidence of testicular atrophy and recurrence.

Laparoscopic sleeve gastrectomy is superior in comparison to other restrictive procedures due to the decrease of large parts of ghrelin producing stomach mass. Aim of the Work: to outline recent modalities in management of early complications of sleeve gastrectomy. This study was performed at Benha university hospitals and Kasr El Aini University Hospitals on 30 early complicated cases post sleeve gastrectomy at the period from 1/1/2018 to 30/6/2018. Patients and Methods: This study was performed at Benha university hospitals at the period from 1/1/2018 to 30/6/2018 on 30 early complicated cases post sleeve gastrectomy. Consent of the patients was taken and data were collected included: age, sex, BMI, and comorbid conditions at the time of admission. Results: According to the co morbidities, eight of the studied cases (26.7%) suffered from hypertension, and 22 others (73.3%) didn"t suffer from hypertension, also half of the studied cases (50.0%) suffered from diabetes. As regard the complications that the patients suffered after the operation in our study, 7 patients (23.3%) suffered of Tachycardia, 5 patients (16.7%) suffered of Vomiting, 4 patients (13.3%) suffered of Hemorrhage, Leakage and Hematemsis, 3 patients (10.0%) suffered of Wound infection, 2 patients (6.7%) suffered of Gastric stenosis, Conversion to open and Melena. On the other side, none of the patients suffered from DVT, Gastric volvulus, pulmonary embolism and Mortality. Conclusion: LSG is one of the most effective surgical procedures in weight loss and is considered safe if its complications are diagnosed and managed early and we recommended to do this study on largenumber of patients to support our results and to know more about the sleeve gastrectomy complications to manage them as early as possible to get benefit from sleeve gastrectomy and to be used on a wide range.
Background: Surgical intervention is mostly needed in many cases of undescended testis to improve... more Background: Surgical intervention is mostly needed in many cases of undescended testis to improve fertility; it is still unsure which approach is better than the other in management of undescended testicle in infants; classic inguinal approach or scrotal single incision approach. Aim of the current study was to compare between classic inguinal approach and scrotal single incision approach to perform orchiopexy for management of un-descended testis in infants and to clarify which is better; inguinal orchidopexy or scrotal orchidopexy. Patients and methods: we included forty male patients with undescended testis who were divided into two groups; group A included twenty patients and they were surgically managed with single transscrotal orchiopexy and group B included twenty patients and they were surgically managed with classic inguinal approach by two incisions (inguinal and scrotal). We assessed operative time and complications as wound infection, recurrence and testicular atrophy. Results: Operative time in group A was shorter than the operative time of group B which was managed by the inguinal approach (p=0.004). Immediate post-operative complications as; skin site infection, hematoma and edema, in group A were slightly more than the complications in group B (p=0.002). group A showed no recurrence or testicular atrophy after six months while group B showed 2 cases of recurrence and one case of testicular atrophy (p=0.005). At three months after surgery, cosmetic results and patients satisfaction of the incision scar was higher in group A than group B (p<0.001). Conclusions: performing orchiopexy through single scrotal incision approach is better than the approach of inguinal incision regarding operative time, rabid healing time, parents' satisfaction, fewer long-term complications, less incidence of testicular atrophy and recurrence.
