Rotavirus is the major etiologic agent. Objectives. To describe the epidemiological and clinical characteristics of acute gastroenteritis according to their etiology (rotavirus or other) in children younger than 5 years old in a private institution in the City of Buenos Aires. Secondary objective: to analyze related costs. Material and Methods. Cross sectional, descriptive, observational study conducted during one year in children younger than 5 years old with gastroenteritis. The presence of rotavirus was diagnosed with the VIKIA® Rota-Adeno test. Demographic, clinical and immunization data were collected. A univariate data analysis was performed. Results. A total of 275 patients were included; 18.5% of them were R+. Rotavirus was more common in children younger than 2 years old and in the period between March and June. The cases of rotavirus gastroenteritis were more severe, required more hospitalizations (OR 2.07; 95% CI 1.17-7.13), and resulted in higher costs. In the sample studied, the immunization outcome measure reduced the risk of rotavirus infection. Conclusions. Acute gastroenteritis caused by rotavirus were different from other etiologies in that they had a seasonal peak and in relation to the median age of patients, the severity of the condition, the association with hospitalization and the increase in costs.

Encefalomielitis aguda diseminada; Mycoplasma pneumoniae; Biopsia cerebral Resumen Se presenta el... more Encefalomielitis aguda diseminada; Mycoplasma pneumoniae; Biopsia cerebral Resumen Se presenta el caso de un niño de 5 años sin antecedentes de enfermedad, que se internó en terapia intensiva por convulsiones tónico-clónicas focalizadas en la cara y en el hemicuerpo derecho, con documentación de temperatura axilar de 37,4 • C. Se descartó la presencia de gérmenes comunes y la etiología viral a través de estudios de muestras de líquido cefalorraquídeo (LCR). Se sospechó la presencia de Mycoplasma pneumoniae por comprobarse inmunofluorescencia positiva en suero para anticuerpos de clase IgM. El diagnóstico se confirmó mediante la detección del ADN de dicho patógeno sobre la biopsia cerebral efectuada por el método de la reacción en cadena de la polimerasa (PCR) y una histología compatible con encefalomielitis aguda diseminada. El paciente recibió tratamiento con claritromicina y su evolución fue favorable. Al menos dentro de nuestros conocimientos, este es el primer caso en el que se detectó ADN de M. pneumoniae en una biopsia cerebral por el método de PCR.

Failures in communication are common during patient handoffs between physicians, which predisposes to errors. Few articles have been published on this topic in Argentina. For this reason, our objective was to confirm whether using a structured handoff (I-PASS), which has been successfully used in the USA by Doctor Starmer, may reduce the omission of key data without prolonging its duration at our department. Population and methods. The study was conducted at a private facility in the Autonomous City of Buenos Aires between June 15 th , 2017 and March 31 st , 2018. It had a quasi-experimental, uncontrolled, before-and-after design. Pre-and post-intervention handoffs were assessed. The intervention included training physicians on how to use a structured handoff mnemonic (I-PASS: illness severity, patient summary, action list, situation awareness and contingency planning, synthesis by receiver), training on team work, written computerized handoff document, feedback observations, and simulation. Results. A total of 158 and 124 pre-and postintervention assessments were done respectively. The pre-and post-intervention comparison showed a significant improvement in most of the handoff key points. The time used for the handoff was 199 seconds (174-225) before the intervention and 210 seconds (190-230) after the intervention, p = 0.523; interruptions also decreased significantly. Conclusion. Introducing the I-PASS program reduced key data omission without prolonging handoffs. Interruptions were also reduced.

Generalidades El monitoreo electrocardiográfico de todo paciente críticamente enfermo debería realizarse en forma continua aunque los eventos cardíacos primarios son inusuales en la edad pediátrica. En los niños, las arritmias se deben más frecuentemente a hipoxemia, acidosis e hipotensión, razón por la cual es menester primero dirigir la atención a establecer el A-B-C correctamente. En pediatría una alteración del ritmo debe tratarse como una emergencia sólo si compromete el gasto cardíaco o existe la posibilidad de colapso circulatorio. En ese contexto de emergencias, la gran mayoría de las arritmias asociadas a colapso hemodinámico pueden clasificarse según su efecto sobre los pulsos: • Pulso rápido= taquiarritmias. • Pulso lento= bradiarritmia. • Pulso ausente= paro sin pulso.
