Background The Department of Health of the Government of New Brunswick and Regional Health Author... more Background The Department of Health of the Government of New Brunswick and Regional Health Authorities elected to implement Stepped Care 2.0 (SC2.0) in 2021, and began with One-at-a-Time (OAAT) therapy in Community Addiction and Mental Health Centres (CAMHCs) to facilitate rapid access to addiction and mental healthcare. This study: 1) explicated the process of implementing OAAT therapy as it aligned to evidence-based implementation frameworks and strategies; 2) assessed readiness for change among providers during the implementation; and 3) evaluated initial client and system outcomes. Methods The process of implementing OAAT therapy within CAMHCs was documented and retrospectively aligned with the Active Implementation Frameworks-Stages of Implementation, Consolidated Framework for Implementation Research, and incorporated strategies endorsed by the Expert Recommendations for Implementing Change. Stepped care models have the potential to provide a much needed framework for the development of ... more Stepped care models have the potential to provide a much needed framework for the development of comprehensive mental health systems. However, definitions of stepped care lack consistency, resulting in varying interpretations reflected in its implementation. To help foster greater alignment in research and practice, we propose a set of principles for stepped care which can provide guidance on how to bridge multiple mental health services together, reduce fragmentation, and respond to the full breadth of mental health needs along a continuum of care in diverse settings. We hope that articulating these principles will spur mental health stakeholders to translate them into actionable standards.

Single-session, or one-at-a-time, open-access care is central to the mental health care system tr... more Single-session, or one-at-a-time, open-access care is central to the mental health care system transformation process now underway in several parts of Canada. Single-session principles align well with a mental health recovery strategy and the new Stepped Care 2.0 (SC2.0) model under consideration across Canada. SC2.0 provides a framework for integrating single-session open-access care within the broader mental health ecosystem. Through continuous co-design the model connects values and addresses inevitable tensions that arise when attempting system integration. Model development and scaling is still in the early stages. More work needs to be done, including both program evaluation and independent research. Continued critical attention is the only way to maximize impact at a population level. With open access to an array of resources, organized to meet people where they are in terms of readiness, functioning and capacity for engagement, population mental health is possible. This pape...

First on the list is Dr. Christopher Davis. Chris's generosity with his time and ideas, along with his sharp pen, helped push this dissertation forward and to completion. During the process I would often say, ". ..if I get it done", while Chris would keep repeating "...when you get it done". It really made a difference to have a supervisor who believed in the project. Dr. Brian R. Little must be acknowledged for introducing me to George Kelly and Personal Construct Theory in one o f our graduate personality courses. In my mind's eye I can still see Kelly peering down at us from the imaginary perch that Brian (passionately) created for him at the front of the classroom. Personal Construct Theory provided a framework for my own understanding of human thought and behaviour and later became pivotal to my dissertation research. This dissertation would not have been completed without persistence and a solid work ethic, the origins o f which I attribute to my parents-William R. Callahan and Daphne M. Ryan. They personified the mantra "When the going gets tough... keep on going". My own family provided the environment and inspiration to undertake and complete the arduous task o f doctoral research. My three sons, Nathan, Ryan, and Dylan, through their own unique approaches to the world, must be credited with broadening my own constructs related to people and life in general. And last but not least on the list is Garfield James Churchill, my soul mate, who filled in the blanks at home and, from the very beginning on "Fantasy Island" (NFLD), was an inspiration and encouraged me to push the limits.
