Increased pulmonary artery pressure results from raised left atrial pressure, pulmonary arteriolar vasoconstriction, and obliterative changes in the pulmonary vascular bed, and usually responds to relief of mitral stenosis. However, severe pulmonary hypertension may persist after mechanical relief of mitral stenosis. Methods. A case cross sectional study was conducted in mitral stenosis patients to study the immediate effect of BMV for severe pulmonary hypertension. Pulmonary hypertension was assessed by echocardiography before procedur and 24-72 hours after procedure. Analysis was done to assess the relation of age, gender, left atrial size, gradient across mitral valve, mitral valve area and systolic PA pressure before procedure on the persistency of severe pulmonary hypertension. Results. There were 183 patients with mitral stenosis were found in January 2007-December 2008. Seventy three were severe pulmonary hypertension (systolic PAP ≥ 80 mmHg) and 61 patients were mildmoderate pulmonary hypertension (systolic PAP < 80 mmHg). Only 54 patients were fulfilling study criteria. Of 54 cases included to analysis, 66.7% were female, 70.4% were in sinus rhytm and had age of 35.44 ± 10.55 yo. The mean mitral valve gradient (MVG) decreased from 18.53 ± 6.40 to 8.50 ± 4.84 mm Hg (p< 0.001). Mitral valve area (MVA) increased from 0.76 ± 0.26 to 1.36 ± 0.36 cm 2 (p < 0.02). LA diameter decreased from 52.87 ± 7.14 to 49.13 ± 7.35 mm (p = 0.01). The pulmonary artery systolic pressure decreased from 100.48 ± 15.77 to 79.83 ± 17.49 mm Hg (p< 0.001). Systolic PAP > 95 mmHg was associated with persistency of PH in mitral stenosis patients with 58% sensitivity and 30% specificity. Conclusions. The degree of Systolic PAP before BMV is associated with persistency of severe pulmonary hypertension. Systolic PAP > 95 mmHg was associated with persistency of PH in mitral stenosis patients with 58% sensitivity and 70% specificity.

Materials and methods A cross-sectional pilot study was done to assess the correlation between Wilkin's score and Wilkin's Calcium score to the result of the Mitral Computed Tomography Calcium Score. Result Mitral computed tomography calcium score has moderate to strong correlation to Wilkin's calcium score (Correlation 0.738 (Strong correlation), p 0.01) and Wilkin's score (Correlation 0.626 (Moderate correlation), p 0.039). Two of the samples showed zero calcium ct score; however, they scored two in Wilkin's calcium score. Whether a single beat method can be used to measure the mitral valve area (MVA) and transmitral gradient for stenosis severity assessment in AF is still uncertain. Methods: Forty-eight MS patients with AF (mean age 46.8+8.6 years) underwent routine echocardiographic study. The MVA by pressure half time (PHT) and transmitral mean gradient were measured by four methods: the standard approach (averaging multiple beats), single short R-R cycle, single long R-R cycle, and single beat RRp/RRpp = 1. 2D and 3D planimetry MVA were measured at the mitral orifice. Concurrent lesions of dynamic left ventricular outflow tract obstruction (DLVOTO) with aortic ste... more Concurrent lesions of dynamic left ventricular outflow tract obstruction (DLVOTO) with aortic stenosis pose a challenge in the measurement of the pressure gradient and severity of each lesion. Determining the true culprit lesion is difficult and challenging. The establishment of true culprit lesion is crucial in deciding the future course of action. We present two cases of concurrent DLVOTO and aortic stenosis. Although the composition of lesions is similar, the severity of each lesion was different and described a variety of technical problems. Finding the culprit through the shape of the stenotic jet from the continuous wave Doppler as well as other different technical approaches is the critical point of this case report. The first patient showed nonsignificant DLVOTO with severe aortic stenosis in which transthoracic echocardiography (TTE) alone was sufficient to find the culprit. Meanwhile, the second patient concluded to have significant DLVOTO with moderate aortic stenosis based on TTE and transesophageal echocardiography examination data. Jet morphology from Doppler examination is a crucial finding to differentiate DLVOTO with aortic stenosis, along with other parameters that might help find the dominant lesion. Multiple modalities with several tailor-made technical considerations might be needed to establish a culprit lesion.

The aim of this study is to analyze diastolic function in preeclampsia patients during pre- and post-partum period using E/e’ parameter measurement.Methods: This is a prospective cohort study of pregnant women with preeclampsia who were hospitalized and planned for pregnancy termination. Basic clinical characteristics were obtained from all samples. Echocardiography was done prepartum, 48-72 hours after termination, and 40-60 days postpartum. Background: Pulmonary artery catheter (PAC) or Swan-Ganz until now is still considered as gold st... more Background: Pulmonary artery catheter (PAC) or Swan-Ganz until now is still considered as gold standard for hemodynamic measurement. It is, however, high cost, has some complications risks, and has shown little benefit/risk ratio. It, therefore, makes many studies assessing correlation or agreement between non-invasive or semi-invasive modalities with PAC for hemodynamic measurement. There were many studies for agreement between TEE and PAC, while only few studies that assessed agreement between TTE and PAC for hemodynamic profiles, especially SVR. Method: This is an analytic cross-sectional study in post CABG patients in ICU whose PAC were performed. Hemodynamic measurement by TTE measuring LVOT diameter, VTI, IVC diameter that can be calculated into SV, CO, and SVR. There were three SVR categories: SVR1 using top limit of estRAp, SVR2 using bottom limit of estRAp, and SVR3 using mean of the estRAp. Statistic analysis used paired t-test for mean comparison. Agreement analysis used Bland-Altman method. Intraobserver and interobserver variability were assessed by intraclass correlation (ICC) and repeatibility coefficient (RC). Percentage error was calculated as follows: twice the SD for the difference between the two methods being compared was divided by the mean value obtained from both methods {2xSD mean difference / [(mean from PAC + mean from TTE) /2] x 100}. Data was analyzed by SPSS 15.0 Results:There were 50 data from 28 samples. As many as 30 data was also measured by second observer for interobserver variability. Bias/mean difference between TEE and PAC were: 0,11 for SV; 0,03 for CO; -15,76 for SVR1; 67,75 for SVR2; 25,99 for SVR3. There were also good agreement between methods with quite narrow limits of agreement (SV -15,22; 15,44, CO -1,30; 1,36, SVR1 -293,18; 261,66, SVR2 -220,10; 355,60, SVR3 -256,23; 308,21. Conclusion: There is good agreement between TTE and PAC for measuring SV, CO, and SVR. Therefore, TTE can be used to evaluate hemodynamic variables. Introduction: Atrial fibrosis and ischemia were related to the genesis of atrial fibrillation (AF... more Introduction: Atrial fibrosis and ischemia were related to the genesis of atrial fibrillation (AF). It remains to be answered whether the structural abnormalities lead to the functional changes of LA in paroxysmal AF? Methods: Sixty-five AF patients and 29 controls were included. The ECG gated and 64 slices multi-detector computed tomography was performed during sinus rhythm before ablation. The atrial images at end-diastolic and endsystolic phases were obtained. Nine hundred points at left atrium (LA) contour were sampled and the spatial differences of each point were compared between the two phases. They were further integrated into 18 segments in the bull-eye plots. Therefore, the ejection fraction (EF) the regional wall motion of LA could be assessed. Results: The EF of LA body and LA appendage were significantly reduced in AF compared with controls (LA: 26.9±13.4 versus 38.1±11.6%, p < 0.001; appendage: 36.5±20.2 versus 53.0±13.5%, p < 0.001). The magnitudes of wall motion of the 18 segments were all significantly reduced in AF compared to control. In addition, an unique pattern of LA contraction was illustrated. The magnitudes of wall motion in AF was smallest in the upper third LA, medium in the middle third LA and greatest in the lower third LA (5.15±0.51 versus 7.42±1.16 versus 9.52±1.48mm, p < 0.01). The pattern of LAmovement was similar in controls. Conclusion: The contractility of LA body and appendage were reduced in paroxysmal AF. This was attributed to the global hypokinesia of LA. No apparent regional wall motion abnormalities could be demonstrated in AF
