At the cross section of the mid-femur, climbing exercise increases outer diameter and area of cortical bone. The mechanical strength of the femur is increased. This change of cortical volume and structure is more marked in anti-gravity exercise, such as climbing and jumping, than aerobic exercise. At the bone marrow area, climbing exercise increases trabecular bone volume and osteoblast number, while it decreases fat volume and adipocyte number. Skeletal loading promotes differentiation from mesenchymal stem cells to osteoblasts and suppresses that to adipocytes by facilitating the signal through PTHĂ·PTHrP receptor.

Anemia is a common complication of patients with chronic kidney disease (CKD), which not only low... more Anemia is a common complication of patients with chronic kidney disease (CKD), which not only lowers their quality of life but also potentially causes cardiovascular diseases such as congestive heart failure and coronary heart disease, and accelerates the progression of renal dysfunction. Pre-dialysis patients were assigned to groups A, B, C or D based on hemoglobin levels of ≤ 8.9 (n = 48), 9.0-9.9 (n = 63), 10-10.9 (n = 53), and ≥ 11.0 g/dL (n = 39), respectively. Cardiac function was estimated using echocardiography to clarify the relationship between anemia and cardiac disorders in patients with CKD immediately before starting hemodialysis. Left ventricular ejection fraction (LVEF) was significantly higher in group D than in groups A and B. The fractions with an LVEF of less than 50% were 16.7, 4.8, 1.9, and 0% in groups A, B, C, and D, respectively. Posterior wall thickness was statistically thicker and the deceleration time of the early diastolic wave was longer in groups A and B, respectively, than in groups C and D. The left ventricular mass index in group D was significantly lower than in any other groups. Anemia in pre-dialysis patients with CKD is a probable cause of impaired left ventricular systolic function and progressive left ventricular hypertrophy. Our results suggest that Hb levels should be maintained at >11 g/dL by EPO administration from the perspective of protecting cardiac function, although the upper limit of the target Hb level was undetermined.

This is the reason why new disease entity "CKD-bone mineral disorden (MBD)" was introduced by the GBMI (Global Bone and Mineral Initiative) (one of the workgroups of KDIGO [Kidney Disease Improving Global Outcome]). GBMI has also produced new pathological classification of renal osteodystrophy (ROD) as TMV (turnover, mineralization, volume) classification and LBC (laboratory abnormalities, bone abnormalities, extra osseous calcification) classification for CKD-MBD.

Pregnancy-associated hypopituitarism (PAH) has been referred to as Sheehan's syndrome and lymphoc... more Pregnancy-associated hypopituitarism (PAH) has been referred to as Sheehan's syndrome and lymphocytic hypophysitis, both of which are occasionally disclosed several decades after delivery. We report the rare case of a 70year-old woman diagnosed with PAH with empty sella 42 years after her second delivery; she had had an episode of hypovolemic shock during her second delivery. She had been referred to our hospital because of the emergence of non-sustained ventricular tachycardia and symptoms of appetite loss and general fatigue accompanied by hyponatremia, hypoglycemia, and hypothyroidism. Hypopi-tuitalism and secondary adrenal insufficiency had been revealed after her hospitalization. She had suffered from Raynaud's syndrome for the past several decades. Our testing revealed that the patient had autoantibodies for thyroid peroxidase, thyroglobulin, and centromere, indicating the diagnosis of chronic thyroiditis and limited scleroderma. Her series of symptoms had been treated successfully by hydrocortisone and thyroid hormone replacement therapy. To the best of our knowledge, latent PAH accompanied by limited scleroderma have not been described in the international literature to date though quite a few latent cases may exist. We assume that some autoimmune mechanism during the perinatal period may possibly be a common step in the pathogenesis of PAH, chronic thyroiditis and limited scleroderma.

Case 1: A 39-year-old woman with schizophrenia, obesity, hypertension and dyslipidemia was admitt... more Case 1: A 39-year-old woman with schizophrenia, obesity, hypertension and dyslipidemia was admitted to our hospital for deteriorating serum Cr level, from 97.2 to 645.3 lmol/l. She had been started on losartan 5 months earlier. After suspension of losartan and infusion of saline, her serum Cr level gradually recovered and she was discharged. Unfortunately, her physician restarted her on losartan, and 5 months after this discharge, her serum Cr level rose again to 194.5 lmol/l; again, serum Cr promptly recovered when losartan was discontinued. Selfinduced vomiting after overeating, suggesting bulimia nervosa, was revealed via the detection of a remarkable reduction of chloride compared to sodium in her spot-urine sample. Case 2: A 39-year-old woman who had been diagnosed with bipolar disorder and diabetes mellitus was admitted to our hospital with suspected diabetic ketoacidosis. After improvement in her blood sugar level, olmesartan administration for hypertension was begun. Four days later, her serum Cr level had risen from 60.1 to 256.4 lmol/l. After suspension of olmesartan and infusion of saline, her serum Cr level gradually fell to normal levels. Urinalysis revealed the dissociation of sodium and chloride. During her hospitalization, habitual self-induced vomiting was discovered. The measurement of urinary electrolytes may be useful for the detection of eating disorders with self-induced vomiting; in these patients, the administration of angiotensin II receptor blockers may induce acute kidney injury.

A 69-year-old woman was admitted to our hospital for rapid increase in serum creatinine level wit... more A 69-year-old woman was admitted to our hospital for rapid increase in serum creatinine level with microscopic hematuria and nephrotic-range proteinuria. Three months prior to admission, she became aware of atypical genital bleeding, leading her gynecologist to suspect endometrial cancer. Light microscopy examination of renal biopsy revealed crescentic glomerulonephritis with peritubular capillaritis. Immunofluorescence microscopic examination did not detect any significant staining, and no electron-dense deposits were detected by electron microscopy. No autoantibodies, including anti-myeloperoxidaseor anti-proteinase3-anti-neutrophil cytoplasmic antibodies were detected. The patient underwent total abdominal hysterectomy and bilateral salpingo-oophorectomy and was found to have endometrial neuroendocrine small cell carcinoma (NSCC), stage 1B. In her clinical course, her serum creatinine level gradually improved without immunosuppression therapy. Endometrial NSCC is a rare endometrial malignancy. This is the first case report of vasculitis associated with NSCC originating from the uterus.

The term CKD-Mineral and Bone Disorder (CKD-MBD) has been established describing a broader clinical syndrome that develops as a systemic disorder of mineral and bone metabolism due to CKD. Vascular calcification and secondary hyperparathyroidism are major diseases in CKD-MBD.
