The first edition of the guidelines for the use of ultrasound contrast agents was published in 20... more The first edition of the guidelines for the use of ultrasound contrast agents was published in 2004, dealing with liver applications. The second edition of the guidelines in 2008 reflected changes in the available contrast agents and updated the guidelines for the liver, as well as implementing some nonliver applications. The third edition of the contrast-enhanced ultrasound (CEUS) guidelines was the joint World Federation for Ultrasound in Medicine and Biology-European Federation of Societies for Ultrasound in Medicine and Biology (WFUMB-EFSUMB) venture in conjunction with other regional US societies such as Asian Federation of Societies for Ultrasound in Medicine and Biology, resulting in a simultaneous duplicate on liver CEUS in the official journals of both WFUMB and EFSUMB in 2013. However, no guidelines were described mainly for Sonazoid due to limited clinical experience only in Japan and Korea. The new proposed consensus statements and recommendations provide general advice on the use of Sonazoid and are intended to create standard protocols for the use and administration of Sonazoid in hepatic and pancreatobiliary applications in Asian patients and to improve patient management.

A 66-year-old man with postsigmoidectomy status for colon cancer received laparoscopic partial he... more A 66-year-old man with postsigmoidectomy status for colon cancer received laparoscopic partial hepatectomy due to a hepatic mass with employing titanium clips were for a vascular clamp. Histological examination showed liver metastasis from sigmoid colon cancer. Twenty-nine months after the partial hepatectomy, a mass developed on the stump at the hepatic resection. Laparoscopic left lateral segmentectomy was conducted under suspicion of cancer recurrence and an automatic titanium stapling device was used. The macroscopically cut surface of the liver showed a grey-white solid nodule measuring 23 x 20 mm and involving metal clips. The nodule was consistent with granuloma microscopically. Twenty-three months after the segmentectomy, a mass reappeared on the hepatic radial margin and an open left lateral hepatic lobectomy was performed because of its growth tendency. Histopathological examination revealed granuloma similar to the previous instance. Since these nodules formed a granulomatous lesion surrounding metal staples/clips and evidence of caseous necrosis was lacking, granuloma due to surgical staples/clips was suspected. Sporadic case reports of postoperative pulmonary granuloma at the staple line have been published previously, but there are no articles detailing a case involving hepatic granuloma. We present our case as the first report of postoperative staple-line hepatic granuloma.

Arrival time parametric imaging (At-PI) in contrast-enhanced ultrasonography is useful for assess... more Arrival time parametric imaging (At-PI) in contrast-enhanced ultrasonography is useful for assessing liver fibrosis in chronic hepatitis C (CHC) infection. The study aimed to elucidate the effect of hepatic inflammation on At-PI efficiency. Subjects were 159 CHC patients who underwent contrast-enhanced ultrasonography immediately before liver biopsy. Ultrasound contrast agent was injected, and contrast dynamics of the S5 to S6 region of the liver and right kidney were recorded for 40 seconds. The At-PI of liver parenchyma blood flow was generated using saved video clips. Hepatic blood flow during the first 5 seconds after starting contrast injection was displayed in red and that after another 5 seconds was displayed in yellow. Purpose Arrival time parametric imaging (At-PI) using contrast-enhanced ultrasonography (CEUS) is... more Purpose Arrival time parametric imaging (At-PI) using contrast-enhanced ultrasonography (CEUS) is a procedure for evaluating liver disease progression in chronic hepatitis C infection (CHC). We investigated At-PI diagnostic efficacy in predicting development of collateral veins. Methods In total, 171 CHC patients underwent CEUS and upper gastrointestinal (UGI) endoscopy before liver biopsy. Conventional US was performed before CEUS to identify paraumbilical veins (PV) or splenorenal shunts (SRS). After intravenous perflubutane, contrast dynamics of liver segments 5-6 and the right kidney were saved as raw data. At-PI image ratio of red (ROR) pixels to the entire liver was analyzed. Receiver operating characteristic (ROC) curves were generated to investigate the utility of At-PI for collateral vein identification. Results Conventional US revealed PV in two patients and SRS in five patients; UGI endoscopy detected esophageal varices (EV) in eight patients. Diagnostic capability of At-PI for detecting PV, SRS, and EV was satisfactory, and high for PV and SRS [PV; area under the ROC curve (AUROC) 0.929, cutoff value 77.9%, SRS; AUROC 0.970, cutoff value 82.0%, EV; AUROC 0.883, cutoff value 66.9%]. Conclusions Evaluation of hepatic arterialization by At-PI was useful for predicting collateral vein development in CHC patients. Gli Autori hanno indagato l'efficacia diagnostica di At-PI nel predire lo sviluppo di circoli collaterali. Metodi In totale 171 pazienti sono stati sottoposti a CEUS e ad endoscopia del tratto digestivo superiore (UGI) prima della biopsia epatica. Un'ecografia convenzionale è stata eseguita prima della CEUS per identificare pervietà della v. ombelicale (PV) o di shunt spleno-renale spontaneo (SRS). Dopo iniezione e.v. di perflubutano sono stati salvati i dati grezzi della dinamica del m.d.c. raccolti dai segmenti 5 e 6. E' stato inoltre analizzato il rapporto dell'immagine At-pi con i pixel rossi (ROR) dell'intero fegato. Sono state ottenute delle curve ROC per valutare l'utilità della At-PI nell'identificare circoli collaterali. Risultati L'ecografia di base ha identificato PV in due pazienti e SRS in cinque pazienti; UGI ha diagnosticato varici esofagee in otto pazienti. La capacità diagnostica di At-PI per identificare PV, SRS e varici esofagee è stata soddisfacente ed elevata per PV ed SRS (PV : area sotto la curva ROC (AUROC) = 0,929 con valore di cutoff = 77,9%; SRS : AUROC = 0,970 con cutoff = 82;0% ; varici esofagee : AUROC = 0,883 con cutoff = 66,9). Conclusioni La valutazione dell'arterializzazione epatica mediante At-PI è stata utile nel predire circoli collaterali spontanei nei pazienti con malattia cronica di fegato HCV relata.

Methodology: Forty-seven patients treated with sorafenib (sorafenib group) and 77 patients treated with HAIC (HAIC group) were investigated retrospectively using propensity score matching (PSM) to minimize selection bias. The cumulative survival rate was investigated before and after PSM in each of the sorafenib and HAIC groups. The cumulative survival rate was compared between the sorafenib and HAIC groups, and among the TNM stage by the Liver Cancer Study Group of Japan (LCSGJ TNM stage). Results: No significant difference was noted in overall survival (OS) between the sorafenib and HAIC groups regardless of before or after PSM. An increase of serum fasting total bile acid (FTBA) and maximum total bile acid (MTBA) was closely reflected the degrees of hepatic fibrosis and they were seemed to be useful for differential diagnosis and clinical observation of the alcoholic liver disease. We have been reported the criteria of the grading for bile acid tolerance test at Inuyama Symposium in 1977. With the use of this criteria, we applied it to alcoholic liver disease. The results of bile acid tolerance test were normal in patients with ALF mild, fatty liver and no remarkable change, these were slightly abnormal in patients with ALF moderate and severe, these were severe abnormal in patients with ALC. In order to elucidate the mode of progression of alcoholic liver disease, relationships among the... more In order to elucidate the mode of progression of alcoholic liver disease, relationships among the drinking style, laboratory data, anti-HCV antibody and histological changes were investigated on 36 patients in whom the liver biopsy was repeatedly done. Following results were obtained (1) In the group of continuous drinking over 100g ethanol per day, histological progression was found in 11 of 13 patients (85%) regardless of positive anti-HCV. On the other hand, in the group of abstinence or temperance less than 60g daily alcohol intake, histological improvement was found in 6 of 11 patients (55%). (2) Histological improvement was predominantly seen by abstinence or temperance in the cases with lower levels of serum IgA and adenosine deaminase (ADA) on hospitalization and those with rapid decrease in serum gamma-GTP after hospitalization.
