No current was observed from left sinus of valsalva in CAG. In the aortography conducted it was observed that the left system was leading out from the right sinus of valsalva. In the right coronary angiography conducted, it was observed that the right coronary artery and circumflex lead out from the same ostium while the left anterior descending artery leads out from a different ostium. 80% narrowing was detected after the first diagonal of the left anterior descending artery, 90% narrowing in proximal of the circumflex artery and 70% narrowing in distal of the right coronary posterior descending artery with diffuse plaques. Conclusions: In our case coronary artery anomaly was observed together with serious coronary narrowing that causes angina and these presented cases are considerably rare. The rare coronary artery anomalies generally have good prognosis as long as they do not lead to perfusion anomalies and considering that they made lead to sudden deaths caused by accompanying coronary artery diseases they must be monitored closely.

We present a Turkish family and their 6 children, consecutively affected by Ellis-van Creveld (EV... more We present a Turkish family and their 6 children, consecutively affected by Ellis-van Creveld (EVC) Syndrome. Four of the affected children died in the postnatal period, and 2 of them had been admitted to the pediatric cardiology department for their cardiologic evaluation. Since they had the features of the EVC Syndrome, linkage analysis was performed with the polymorphic markers, D4S3360-D4S2366, selected from 4p 16 locus. There was complete segregation between the disease and marker allels and the two affected siblings were homozygote for the polymorphic markers, as expected in autosomal recessive inheritance. The diagnosis of EVC Syndrome was confirmed by this molecular analysis. Two cases with EVC were presented in this report. Case 1 had partial abnormal pulmonary venous return and pulmonary stenosis additional to ostium primum atrial septal defect and mitral cleft. Partial abnormal pulmonary venous return and pulmonary stenosis were previously not reported with EVC Syndrome. Postaxial polydactyly phenotype of the Case 2 differs from her brother's. There is bifid 5th metacarpal and unilateral (L) bifid middle and distal phalanges resembling syndactyly.
Introduction Abdominal aortic aneurysms can be complicated by some conditions and aortocaval fist... more Introduction Abdominal aortic aneurysms can be complicated by some conditions and aortocaval fistula is one of them. Aortocaval fistula is an unusual entity. Case Report A male patient was admitted to the hospital for abdominal pain and leg oedema of three days duration. In addition, severe anuria was also noted. We determined a fistula from the right lateral wall of aneurysm to the distal of vena cava inferior. The aortocaval fistula was closed with pledged-reinforced 4/0 polypropylene suture in the aneurysmal wall. At the postoperative 10th day, he was discharged with normal renal function. Discussion After a long-term untreated fistula, right-sided heart failure, acute renal and hepatic insufficiency, deep vein thrombosis and pulmonary thrombo-embolism can be seen in these patients. Increased venous pressure should be the reason for decreased arterial flow results in renal insufficiency. Emergency intervention in these patients saves the patient’s life as well as prevents irreversible organ failure.

Methods: Twenty-two patients had cardiac injury among twenty-five patients who had been referred to our hospital with a suspicion of this diagnosis. Data of the patients were retrospectively evaluated. Results: Echocardiography could be performed in 11 patients with stable haemodynamics and pericardial effusion was established in all of them with one false positive result. The patient with false positive result had left internal thoracic artery and venous injury. Cardiac tamponade was seen in 17 (77%) patients. Injury sites were right ventricle 10 (45.5%), left ventricle 7 (31.8%), left ventricle consisting left anterior descending artery 2 (9.1%), right atrium 1 (4.5%) and pericardial injuries and hematoma 2 (9.1%). The mean physiologic index (PI) of the patients was 10.68+/-5.63, penetrating cardiac trauma index was (PCTI) 14.09+/-6.3, penetrating thoracic trauma index (PTTI) was 17+/-8.84 and organ injury scale according to the American Association for the Surgery of Trauma (AAST/OIS) was 3.86+/-1.25. Mortality rate was 27.3% with 6 out of 22 patients. PI, PCTI, PTTI and AAST/OIS scores of the non-survivors were significantly higher than those of the survivors (p<0.05). While there was no difference between survivors and non-survivors regarding the presence of tamponade and the frequency of the associated abdominal injury, left ventricular and left anterior descending artery injuries and associated lung injuries were more common in non-survivors (p<0.05). Conclusion: Non-survivors with penetrating cardiac injury have higher PI, PCTI, PTTI and AAST/OIS scores and they are more severely injured patients comparing to the survivors.

Sol iç meme venine olan bu beklenmedik kateterizasyon literatürde bildirilmiş olmasına rağmen, bu olgu, koroner arter hastasında intraoperatif belirlenen malpozisyonunun ilk raporu gibi görünmektedir.

The durability of bioprosthetic valve is less with mitral than aortic ones. Herein we present a case who had undergone a bioprosthetic mitral valve replacement 21 years ago when he was 31 years old. Echocardiography showed 3 degree mitral regurgitation with gradient 23/12 mmgh, systolic pulmonary artery pressure (SPAP) 48mmgh, left atrium diameter was 8 cm and 3 degree tricuspid regurgitation. The bioprosthetic valve in mitral position was replaced with No.29 st Jude mechanical valve and Tricuspid valve Devege annuloplasty was performed. The postoperative period was uneventful.

The patient had undergone multiple cardiovascular angiographic interventions in the past. The first diagnosis was thrombosed radial artery pseudoaneurysm. The intraoperative diagnosis was the volar ganglion cyst. In this report, we emphasize the importance of the complications after transradial arterial catheterizations and present our experience.
Background: In this study, we aimed to evaluate the performance of modular and unibody endografts... more Background: In this study, we aimed to evaluate the performance of modular and unibody endografts for the endovascular repair of abdominal aortic aneurysm (AAA). Methods: Between January 2012 and December 2017, 130 elective infrarenal abdominal aortic aneurysms treated in an endovascular manner were retrospectively evaluated. Sixty-six patients with the modular type (Medtronic EndurantTM II and Lifetech AnkuraTM AAA) and 64 patients with the unibody type (Endologix AFX®) were compared with regards to early and postoperative one-year results. Results: There was one in-hospital mortality (0.8%) in the modular group. There was no difference in postoperative first-year mortality rate between the two groups (p = 0.678). Loco-regional anesthesia was used more often in the unibody group [34 patients (53.1%)] and the use of general anesthesia was higher in the modular group [56 patients (84.8%)] (p < 0.001). While the duration of the procedure was shorter in the unibody group (p < 0.001), no statistically significant difference was found in the duration of fluoroscopy (p = 0.813) and the amount of contrast agent used (p = 0.553). The follow-up period in the intensive care unit was shorter in the modular group (p < 0.001). Moreover, the five-year survival rate was similar between the groups (84.8% in the modular group and 78.4% in the unibody group, log-rank p = 0.703). Conclusions: The results obtained in our study show that modular and unibody grafts are effective and reliable, although there are some negligible differences in the early period.

An 86-year-old male patient with dual true aneurysms in the unilateral popliteal artery presented with leg pain without any ischemic physical signs. A proximal 20.1¥17.9-mm saccular aneurysm was treated with a 9¥100-mm endovascular stent graft, and a distal 50.6¥54.2-mm fusiform aneurysm was treated with a 10¥100-mm endovascular stent graft. In conclusion, although surgical treatment is the gold standard, endovascular treatment should be considered as an alternative method with several benefits in high-risk patients.
A rare case of a 79-year old women with dysphagia and dyspnea secondary to compression of the eso... more A rare case of a 79-year old women with dysphagia and dyspnea secondary to compression of the esophagus by an aberrant right subclavian artery (ARSA, A. lusoria) is presented. This abnormality is generally silent and often an incidental x-ray finding. Computed tomography and intraarterial angiography, including direct catheterization of the ARSA, established the diagnosis. The interdependence with a co-existing coronary artery disease is discussed. Thus, ARSA has to be taken into account in patients with abnormal x-ray findings presenting with dysphagia and dyspnea.
