The present study is an examination of effects of an antioxidant, N-acetylcysteine (NAC), on local and systemic histopathological changes and neutrophil functions during AP. METHODS This experimental study was performed on 24 Wistar albino rats equally divided into 3 groups: Group 1 comprised sham laparotomy, Group 2 had AP induced with taurocholate infusion, and Group 3 consisted of AP with NAC treatment. Histopathological features in pancreas, kidney, and lung tissues were examined for local and systemic changes during AP. Neutrophil functions were evaluated using flow cytometry. Gastrointestinal sistem hastalıklarının tanısında kullanılan invaziv ve non-invaziv tekniklerdeki... more Gastrointestinal sistem hastalıklarının tanısında kullanılan invaziv ve non-invaziv tekniklerdeki gelişmeler, kanser tedavisindeki başarılar, yaşlı popülasyonun ve yaşam beklentisinin artması, senkronize özofagus-mide kanserlerinin erken teşhis edilebilmesi bu hastalığın sıklığını arttırmaktadır (1, 2). Son yıllarda gastrointestinal sistemde senkronize multipl primer karsinom görülme sıklığı giderek artmakta olup, farklı çalışmalarda %0.7-11 arasındadır (3-6). Multipl primer karsinom en sık mide, kalın barsak ve karaciğerde görülmektedir (7). Özofagus kanserine eşlik eden senkronize diğer organ tümörlerinin birlikteliğinde prognoz kötü olmasına rağmen, erken evrede tanı konulan hastalarda küratif tedavi iyi netice vermektedir. Biz burada özofagus skuamöz hücreli karsinomu ile senkronize tespit ettiğimiz mide adenokarsinomlu olgumuzu sunduk.

Background/aims: The aim of this study was to evaluate whether administration of single-dose prot... more Background/aims: The aim of this study was to evaluate whether administration of single-dose proton pump inhibitor for 3 days to patients undergoing upper gastrointestinal endoscopy with biopsy decreases the complaints related to the procedure using a severity scale and to investigate whether the procedure has any negative impact on quality of life. Methods: Sixty patients were enrolled in the study. Before the procedure, patients were queried using the complaint severity scale. Following the endoscopy, patients were randomized into two groups: Group A was given pantoprazole 40 mg 1x1, while Group B received placebo 1x1 for 3 days. After 3 days, patients were queried again using the same scale. Results: In Group A, all scores had decreased 3 days after the procedure, but only the decrease in the score for the complaint of bloating was significant. In Group B, the scores for epigastric pain, bloating and nausea had increased, and the increases in the scores for epigastric pain and bloating were statistically significant. Conclusions: Administration of proton pump inhibitor for a short period after the procedure will not only decrease the present complaints but also prevent the negative impacts of the procedure, even if only partially.

We present the case of a 67-year-old man with prostate cancer who had no findings of recurrence, ... more We present the case of a 67-year-old man with prostate cancer who had no findings of recurrence, except diffuse radiotracer uptake in the bone marrow in 68 Ga-PSMA PET/CT. Bone marrow uptake was also represented as multiple focal increased spots without any corresponding lytic or sclerotic lesions in CT. MRI revealed a high and homogeneous T2 signal within the bone marrow, without any contrast-enhanced or diffusionrestricted lesions. Further workup, including a bone marrow biopsy, revealed the diagnosis of myelodysplastic syndrome.

This study aimed to compare the efficacy of entecavir and tenofovir in nucleos(t)ide-naive chroni... more This study aimed to compare the efficacy of entecavir and tenofovir in nucleos(t)ide-naive chronic hepatitis B patients after 48 weeks of therapy. We retrospectively reviewed our data of chronic hepatitis B patients. Nucleos(t)ide-naive patients who had received entecavir or tenofovir for at least 48 weeks were included. We compared entecavir and tenofovir after 48 weeks of therapy with respect to virological, biochemical, serological and histological responses. Of the 44 patients, 24 received entecavir and 20 received tenofovir. Pretreatment characteristics of the patients were similar. After 48 weeks, serum HBV DNA levels decreased by 6.93±1.54log copy/ mL in the entecavir group and 6.89±1.22log copy/mL in the tenofovir group (p=0.65). A similar proportion of patients in entecavir and tenofovir groups achieved undetectable serum HBV DNA (87.5% vs. 95%, p=0.39) and serum ALT normalization (79.2% vs. 85%, p=0.62). The mean histological activity index score improved by 3.83±3.51 points in the entecavir group and 2.20±1.91 points in the tenofovir group (p=0.07), and the mean fibrosis scores improved by 0.38±1.61 points in the entecavir group and 0.70±1.17 points in the tenofovir group after 48 weeks (p=0.44). Entecavir and tenofovir are similarly effective in nucleos(t)ide-naive chronic hepatitis B patients with high viral load and/or high fibrosis scores after 48 weeks of therapy.

This disease is often confused with hemorrhoids. The most common site of malignant melanoma following skin and eye involvement is the anorectal region. This is the most commonly involved site in the gastrointestinal tract. We report the case of a 67-year-old patient with lower gastrointestinal hemorrhage for 4 months and hemorrhoid treatment for 2 months. The imaging revealed no distant metastasis but histopathologically, lymph node metastasis and invasion of surrounding tissues. Laparoscopic abdominoperineal resection (APR) was performed.

Hp infeksiyonu kronik gastrit, peptik ülser, gastrik lenfoma ve gastrik kanser gibi gastroduodenal hastal›kla-r›n patojenezinde önemli rol oynar (1). Hp insanlardaki en s›k kronik infeksiyon sebebidir (2). Prevelans› co¤rafik bölge, yafl, ›rk, etnik grup ve sosyoekonomik duruma göre %40 ile %80 aras›nda de¤iflmektedir (3). Hp birçok mekanizma ile konak defans›n›n üzerinden gelerek kronik infeksiyona neden olur. Asidik gastrik mukozada üreaz aktivitesi sayesinde yaflar. Üreaz enzimi üreyi karbondioksit ve amonyaka ay›rarak gastrik asidi lokal olarak tamponlar. Hp, adhesin-like proteinler ile gastrik mukus tabakas›n› geçer ve gastrik mukozaya tutunur (4). Hp fagozom olgunlaflmas›-n› engelleyerek makrofajlar içinde yaflayabilir (5). Gastrik epitel hücreye tutundu¤unda güçlü bir immün yan›ta neden olur. Bu immun yan›t bakterinin eliminasyonunu sa¤layamaz ve kronik inflamasyona neden olur. Hp tedavi edilmedi¤i sürece yok olmaz. Antibiyotik kombinasyon tedavisi uygulanmadan spontan eradikasyon mümkün de¤ildir (6). Topical hemostatic agents are used as auxiliary agents for bleeding post trauma and during operat... more Topical hemostatic agents are used as auxiliary agents for bleeding post trauma and during operation. Ankaferd Blood Stopper (ABS) is an hemostatic agent that is produced in Turkey. We planned to demonstrate the efficacy of this agent in experimental splenectomy model and to compare it with Fibrin Glue (Tisseel®) used in clinics. Forty Wistar Albino female rats were divided in 5 groups. Group 1, Sham; Group 2, Control; Group 3, Suture; Group 4, Fibrin Glue (Tisseel®) and Group 5, Ankaferd Blood Stopper. Partial splenectomy bleeding model was applied to rats after initial hematocrit values were measured. Bleeding in each group was controlled by a different method. Bleeding times of the rats were determined. All the rats were sacrificed on the 5th day after the last hematocrit values were measured. Existence of intraabdominal hematoma and adhesiveness were evaluated and splectonomy was applied for pathological examination. Groups were compared with respect to bleeding time and blood loss and Group 5 and Group 2 were determined to be more effective than Group 3 but Group 4 was determined to be equivalent. Fewer side effects at significant levels were detected in terms of fibrosis and coagulation necrosis during pathological assessment. In conclusion, in spleen bleedings when compared with fibrin glue, Ankaferd Blood Stopper is equally effective and safe and its price is lower.

Histopathological features in pancreas, kidney, and lung tissues were examined for local and systemic changes during AP. Neutrophil functions were evaluated using flow cytometry. RESULTS: Serum levels of pancreatic enzymes were elevated, and histopathological parameters showed acinar cell damage and pancreatic tissue necrosis in the 2 groups with AP. Severe histopathological changes were found in pulmonary and renal tissues, and flow cytometry results indicated defective neutrophil functions in the group with AP alone. NAC treatment significantly ameliorated phagocytosis, chemotaxis, and opsonization of neutrophils (p<0.05). NAC treatment also ameliorated systemic changes in pulmonary and renal tissue damage in all microscopic parameters (p<0.05). CONCLUSION: Uncontrolled and defective neutrophil functions could provoke severe systemic inflammatory responses. In addition to local inflammation and necrosis, severe systemic responses and histopathological changes in extrapancreatic vital organs occur during AP. Treatment with antioxidant NAC significantly reverses detrimental systemic responses in extrapancreatic vital organs by significantly ameliorating neutrophil functions despite ongoing AP.

Objectives: Endoscopic procedures are usually carried out in order to diagnose the cause of iron ... more Objectives: Endoscopic procedures are usually carried out in order to diagnose the cause of iron deficiency anemia (IDA). We aimed to evaluate endoscopic findings of our patients with iron deficiency anemia retrospectively. Materials and methods: We retrospectively reviewed our computerized data of patients who attended to Haydarpasa Numune Education and Research Hospital Gastroentology Endoscopy Unit between 2008 and 2010 years. Patients in whom upper gastrointestinal endoscopy and/ or colonoscopy were performed in order to diagnose the cause of IDA were included in this study. Results: This study included 490 patients with iron deficiency anemia in whom upper gastrointestinal endoscopy and/or colonoscopy were performed. The mean age of patients was 58.94 ± 14.8 years. Of these patients, 288 (172 female, 116 male) underwent only upper gastrointestinal endoscopy, 116 (61 female, 55 male) underwent only colonoscopy and 84 (39 male, 45 female) underwent both upper gastrointestinal endoscopy and colonoscopy. The rates of failure to reveal the cause of IDA in patients in whom either upper gastrointestinal endoscopy or colonoscopy had been performed were 18.75% and 46.55%, respectively. On other hand, the ratio of failure to reveal the cause of IDA was 3.48% in patients in whom both upper gastrointestinal endoscopy and colonoscopy had been performed. Conclusion: When referring any patient with iron deficiency anemia to endoscopy unit, ordering both upper gastrointestinal endoscopy and colonoscopy may allow us to find the reason more accurately.

Methods Pediatric patients with HL, who underwent both bilateral iliac BMB and PET/CT imaging at disease staging were retrospectively analyzed. In determining bone marrow involvement (BMinv), BMB and/or first/follow-up PET/CT imaging were eligible for review. Results Fifty-six patients were included. BMinv was detected by PET/CT imaging in 6/56 (10.7%), whereas the proportion was 3/56 (5.3%) in BMB specimens. Background/aim: The rationale behind using immunonutrition in cancer patients is to prevent malnu... more Background/aim: The rationale behind using immunonutrition in cancer patients is to prevent malnutrition, manage the host's immune response, and keep cancer under control by utilizing the potential immune system available in the host against the tumor. This prospective-study aims to assess the impact of immunonutrition on tumor-infiltrating lymphocytes (TILs) and regulatory T cells (Tregs) in rectal cancer patients receiving neoadjuvant chemoradiotherapy. Materials and methods: This is a single-center, prospective study. Forty patients diagnosed with middle and lower rectal tumors were enrolled in the study between March 2018 and December 2019. Nutrition protocols were given to all study subjects prior to surgery. Tissue CD4, CD8, and Fox P3 expression prior to enrollment (endoscopic biopsy specimens) and following surgery (resected tissue) were compared. Results: Longitudinal data was available for 30 patients. In the present study, 15 patients were given immuno-nutrition, and 15 patients received standard nutrition.
