Introduction: Current schemes for treatment of immune-mediated hearing loss with sporadic short-c... more Introduction: Current schemes for treatment of immune-mediated hearing loss with sporadic short-course, low-dose corticosteroids, are insufficient. Methods: To determine the role of azathioprine in the control of auditory impairment, a longitudinal, observational, descriptive study was performed with 20 patients treated with azathioprine (1.5-2.5 mg/kg/day into two doses) for 1 year. The loss of 10 dB on two consecutive frequencies or 15 dB on an isolated frequency was considered as relapse. Results: The mean age of the patients was 52.50 years (95% CI: 46.91-58.17), half were women. Bilateral affectation was 65%. 75% had organ specific disease and 25% had systemic autoimmune disease. The difference between baseline PTA (46.49 dB; DS 18.90) and PTA at 12 months (45.47 dB; DS 18.88) did not reach statistical significance (P=.799). There was a moderate positive correlation between female sex and the presence of systemic disease (R=.577). By applying Student's t for paired data, a significant difference (P=.042) was obtained between the PTA in frequencies up to 1000 Hz (PTA 125-1000 Hz). The relative incidence rate of relapse per year was .52 relapses/year (95% CI: .19-1.14]). The median time to audiometric relapse-free was 9.70 months (DS 1.03). Conclusions: Azathioprine maintains the hearing threshold, decreases the risk of relapse, and slows down the rate at which patients relapse, altering the course of immune-mediated inner ear disease.

Introduction: Solitary fibrous tumor/hemangiopericytoma is an uncommon neoplasm that accounts for... more Introduction: Solitary fibrous tumor/hemangiopericytoma is an uncommon neoplasm that accounts for only 1-2% of all soft tissue tumors. To the best of our knowledge only three cases of this tumor have been described in the external auditory canal in the past. Case presentation: We report the case of a 60 years old man with a 6 months history of progressive swelling of the left auditory canal. After resection under general anaesthesia, the immunohistochemical staining showed positivity to CD34, CD99, vimentin and bcl2 with the typical spindle-shaped cells organized in a patternless pattern. The tumor was also positive to specific STAT6 nuclear expression. Therefore, the diagnosis of solitary fibrous tumor/hemangiopericytoma was made. Conclusions: The possibility of a STF/HPC should be considered among the differential diagnosis of tumors in the external auditory canal and, due to its malignant potential, a close and long follow up should be done in all the patients.

An ElectroScan ESEM was used for in situ corrosion studies on cold drawn electropolished 316L sta... more An ElectroScan ESEM was used for in situ corrosion studies on cold drawn electropolished 316L stainless steel tube surfaces in the as-received and passivated conditions. Corrosion product was removed as it formed and the tube surface was viewed before, during, and after corrosive attack. The corrosion process was followed in situ, and the sample features most susceptible to corrosion (draw lines, inclusions, etc.) were identified. In addition, X-ray photoelectron spectroscopy (XPS) was used to study the changes in surface chemistry after corrosive attack. This information provided clear evidence of relevant corrosion mechanisms and relative corrosion susceptibility. o 1993 Wiley-Liss, Inc.

Thalamic projections to the anterior suprasylvian and posterior sigmoid coretx: An HRP study of the "vestibular areas" of the cerebral cortex in the cat. BRAIN RES BULL 12(3) 245-252, 1!%4.-We have confirmed electrophysiologically the existence of an oligosynaptic vestibufar projection to the cortex surrounding the rostral end of the anterior suprasylvian sulcus (ASsS). However, we failed to confii a similar projection to area 3a in the posterior sigmoid gyrus. We studied the thrdamic projections to each of these cortical regions by injecting small amounts of HRP in the cortex and looking for neurons retrogradely labeled throughout the thalamus. The exact location of the cortical injections was assessed cytoarchitectonically. The heaviest neuronal labeling after injections in the banks of ASsS was obtained in PO (including in this complex GMmc). A moderate number of projections was found from VPi, VPm and VP1 (the labeling in the latter being particularly prominent in a case injected in the lower bank of ASsS), and also from VL. Occasional labeled neurons were found in the rostro-ventral part of LP. After injections in area 3a in the posterior sigmoid gyrus, which affected to a minor degree either area 3b or 4, many labeled cells appeared in the rostral and dorsal part of VPl, and in the central and lateral parts of VL. Fewer labeled cells were found in VPi, PO and LP. In most cases some occasional labeled cell was observed also in the intralaminar nuclei and in Vm. Many patients with otological symptoms are remitted to the otolaryngology outpatient clinics ever... more Many patients with otological symptoms are remitted to the otolaryngology outpatient clinics every day. These patients commonly undergo imaging studies, generally magnetic resonance imaging (MRI). In some cases, a positive unilateral result is found in the form of a potentially pathological signal that can be observed in the petrous apex region. We present the cases of 6 patients (aged between 26 and 62 years) with asymmetric bone marrow distribution or trapped mucous fluid secretions in the petrous apex, collected over a 6-year period. Diagnosis was made with the use of CT scans and MRI. All of the patients were referred for skull base surgery. In all cases a non-pathologic asymmetry was diagnosed in the petrous apex. Certain non-pathologic conditions of the petrous apex must be treated expectantly without any surgery.

Background: For laryngeal carcinoma, the present TNM clinical staging system does not seem comple... more Background: For laryngeal carcinoma, the present TNM clinical staging system does not seem completely satisfactory as a guide for providing a prognosis for survival. We believe that natural killer cell activity would probably have a role in a more reliable system. Therefore, we analyzed the disease outcome with previously untreated epidermoid carcinoma of the larynx, evaluating classic clinical and pathologic factors, as well as natural killer cell activity in peripheral blood samples of the patients. Objectives: To determine the level of natural killer cell activity in patients with laryngeal carcinoma and to analyze the prognostic value of this finding when associated with other clinical and pathologic variables. Design: Prospective cohort study of surveillance of laryngeal cancer. Mean follow-up of 42.8 months. Settings: Tertiary care referral center and ambulatory and hospitalized care. Participants: We compared 81 men (mean age, 62.4 years; range, 35-89 years) with laryngeal carcinoma with 44 healthy men serving as control subjects (mean age, 57.6 years; range, 37-82 years). Results: Natural killer cell activity was significantly reduced in patients who had died of cancer-related causes in comparison with tumor-free survivors. Overall actuarial survival differed significantly in histologically assessed nodal involvement and low natural killer cell activity. Use of the Cox proportional hazards regression model showed that the factors that seem to have a prognostic effect on survival are histologically determined nodal involvement and low natural killer cell activity. Conclusions: These results support the prognostic significance of the determination of pretreatment natural killer cell activity in peripheral blood samples from patients with laryngeal carcinoma and suggest that assessment of adding such a determination to the current tumor staging system is advisable.

The presence of a serous cyst in the tympanic membrane implies the description of a new or unpublished entity based on our knowledge whose origin may be very unlikely explained on actual embryologic and anatomic background. Clinical Case. We present a case of a 45-year-old woman with progressing right hearing loss. Physical examination revealed a whitish, roundshaped malformation in the posterior-inferior quadrant of the right tympanic membrane. The cyst was removed with a transcanal tympanoplasty. Discussion. A thorough PubMed search that involved the terms tympanic membrane gland, epithelial inclusion cysts, mucous-secreting cyst, and tympanic cyst has shown no positive results. The first description of an unknown entity, such as a tympanic membrane serous cyst, may be the key for clinicians to start paying attention to patients who suffer from similar pathologies and may pass unnoticed because of their rarity or peculiarity.

Detection of the A1555G mutation was seen by mean of techniques for molecular diagnosis. Results: the A1555G mutation was identified in all of the individuals from 4 families with aminoglycoside-induced cochlear damage and in 6 of 8 individuals with familial hearing loss. None of the sporadic cases carried the mutation. Conclusions: a high proportion of patients with cochlear damage due to aminoglycoside ototoxicity and having a familial history of hearing loss, related or not to aminoglycoside exposure, harbor the A1555G mutation.

A number of intra and postoperative accidents in close cavity surgery when using nitrous oxide ha... more A number of intra and postoperative accidents in close cavity surgery when using nitrous oxide have been reported. In order to check the influence of nitrous oxide anesthesia in tympanic surgery, 38 patients were operated on by the usual techniques in middle ear surgery. Half the patients received continuous nitrous oxide inhalation throughout the procedure while, in the remaining cases, the anesthetic gas was suspended 30 minutes before applying the tympanic graft. Audiological differences in both groups, as controlled at 6 and 12 months, were negligible.

However, cure rates are directly related with the early diagnosis. In this study of 200 patients affected by this tumor we analyse the different clinical manifestations, their chronology and their value as early indicators of nasopharyngeal tumour.

Labyrinthitis ossificans is a recently recognized entity with the extending cochlear implant surg... more Labyrinthitis ossificans is a recently recognized entity with the extending cochlear implant surgery. Up to date there are not so many studies of the cellular response leading the organization of the inflammatory reaction within the cochlea. Immunemediated labyrinthitis is a valid experimental model that allow the knowledge of the cellular infiltration mechanisms within the cochlea. The inner ear communicates with systemic immunity via the circulation by the passage of cells through the spiral modiolar vein (SMV) and its collecting venules in the scala tympani. Inflammatory reaction within the cochlea lead to the formation of fibrotic tissue and bone (osteoneogenesis) inside it and injuring the neurosensory organs of hearing and balance. To test which cells are proliferating early in an inflammatory response, an animal model of Keyhole limpet haemocyanin (KLH) induced labyrinthitis was utilized, showing a granulomatous lesions not previously reported in this experimental model.

Objective:To evaluate patients with systemic lupus erythematosus and normal hearing over 10 years... more Objective:To evaluate patients with systemic lupus erythematosus and normal hearing over 10 years, compared with healthy controls.Methods:Thirty patients diagnosed with systemic lupus erythematosus were evaluated in a prospective, descriptive study. Eight patients fulfilled the inclusion criteria, i.e. normal otoscopy, normal hearing, normal imaging and disease duration of less than one year. Eleven healthy companions of ENT patients were recruited as controls.Results:At study commencement, the mean patient age was 32.75 years (range, 15–49 years) and there were no statistically significant audiometric differences between patients and controls. No statistically significant audiometric changes were found either within or between the patient and control groups at 10-year follow up.Conclusion:These results supply no evidence for progressive hearing loss in systemic lupus erythematosus patients with no hearing involvement at study commencement. Several systematic experiments have been carried out with Wistar rats treated with cisplatin, carboplatin, and oxaliplatin to study Pt-drugs accumulation and elimination, and Pt-biomolecule distribution in the cells and cytosols of ear, kidney, and liver. Inductively coupled plasma-mass spectrometry (ICP-MS) analysis shows a cisplatin accumulation capability between oxaliplatin (the highest) and carboplatin (the lowest). The maximum concentration of Pt in all the organs studied was achieved around the first week after cisplatin treatment. During the first 30 days, the elimination was very fast, decreasing in the subsequent 60 days in all the organs. Analysis of cytosols by liquid chromatography (LC)-ICP-MS showed an analogous behavior. In most samples, the distribution of the three drugs in the cellular and cytosolic fractions was similar for all the tissues. For kidney and ear, approximately 60% and 30%, respectively, of the metal accumulated was present in the cytosol, the cytosolic fractions smaller than 50 KDa being especially important. Cisplatin-biomolecule interaction strength under denaturing conditions was evaluated by LC-ICP-MS and showed a quite strong bond.
