Ipsilateral fracture of the femur and tibia, denominated as floating knee, are results of high-en... more Ipsilateral fracture of the femur and tibia, denominated as floating knee, are results of high-energy trauma related to high rates of associated injuries and complications. This case reports a type IIIA exposed floating knee, with loss of joint bone fragments of the tibia and femur during an accident. Guided by the MESS score, limb saving was performed, fragments were reinserted and joint reconstruction was performed. The patient had bone consolidation in four months, infection with fistula in the knee at six months with some improvement when treated and osteonecrosis of the reinserted fragments was observed at sixteen months (lateral femoral condyle and lateral tibial plateau). The patient maintains the arc of motion in the knee of 10-90 degrees of flexionextension, walks without crutches, but with a limp, valgus and pain. The use of scores like MESS helps in the decision to rescue or amputate severe cases, but it does not predict the functional outcome, and the floating knee is commonly associated with complications and worse case functional results, especially when it involves intraarticular fractures, comminuted fractures and infection, factors that are present in this reported case.

Introduction The aim of this study is to present the results of the surgical treatment of the hum... more Introduction The aim of this study is to present the results of the surgical treatment of the humeral neck fracture applying two different plates based on the presence or not of bone compression. Material and Methods Thirty two patients with displced proximal humerus fracture were operated on between January 2002 and August 2007. After radiographic analysis, the fractures were classified into two types: non-compressive (without loss of bone tissue – not impacted fracture) and compressive (with permanent bone loss due to compression between the fragments – impacted valgus fracture) fractures. Depending on the presence or not of bone compression, two different plates were applied for osteosynthesis: a locking angled blade plate, for “non-compressive fractures” and a locking angled “spacer” plate, for “compressive fractures”. Results Twenty (59%) fractures were considered “non-compressive” and operated on applying the locking angled blade plate, and 12 (41%) were classified as “compressive fractures” and were operated on applying a locking angled “spacer” plate. Conclusion the displaced humeral neck fractures may or may not undergo compression and permanent deformity among the fragments. The displaced fractures should be treated in two different manners, depending on whether or not there is bone compression between the fragments.

A analise dos resultados em 11 pacientes levou em consideracao a satisfacao destes e a presenca de dor na avaliacao subjetiva. Sob o ponto de vista objetivo, foram avaliados aspectos finais radiograficos e de exames fisicos. Obtiveram, utilizando esses criterios, 81,81 por cento de bons resultados. (AU)

OBJECTIVES To identify the value of the evoked potential as an adjuvant predictive tool in pelvic... more OBJECTIVES To identify the value of the evoked potential as an adjuvant predictive tool in pelvic ring and acetabulum fractures focusing on the neurological deficit. MATERIAL AND METHODS Adhering to PRISMA guidelines, we performed a systematic review with a priori design to identify randomized controlled trials, retrospective and prospective cohort studies, and case-control studies of interventions in acetabular and pelvic ring injuries. The search strategy included the use of Title/Abstract related to: ("somatosensory" or "evoked potential" or "electrophysiology") and ("randomized controlled trials" or "clinical studies" or "controlled clinical trial" or "systematic literature review" or "meta-analysis" or "review" or "overview") and ("complications"). From 01/1985 to 12/2019, we searched MEDLINE, PubMed, EMBASE, and Cochrane, Centre for Reviews and Dissemination, reference lists and journals. The PRISMA checklist was adopted to evaluate reporting quality. Two reviewers independently selected studies and extracted data. Statistical analysis was conducted to assess the probability of evoked potential tests performed on random samples, with the assumption of normal distribution of the populations from which they were derived and knowledge of the parametric variance. RESULTS Five articles fulfilled eligibility criteria. CONCLUSION Evoked potential tests can quickly and accurately detect imminent neurological impairment during surgery of acetabular and pelvic ring injuries. This results in significantly better surgical outcomes, although in some cases the irreversible nature of the injury cannot be prevented.

RESUMO Os autores revisaram 84 artroplastias do quadril realizadas em 79 pacientes com o objetivo... more RESUMO Os autores revisaram 84 artroplastias do quadril realizadas em 79 pacientes com o objetivo de determinar os possíveis fatores predisponentes à ocorrência da ossificação heterotópica (OH). As variáveis consideradas foram sexo, idade, massa corpórea, diagnóstico pré-operatório, tipo de anestesia, via de acesso, tipo de fixação das próteses. A incidência de OH foi de 45,2%, predominantemente em pacientes do sexo masculino, na faixa etária entre os 50 e 60 anos, portadores de osteonecrose de cabeça femoral ou osteoartrose do tipo hipertrófico. Acreditam ser conveniente a adoção de medidas profiláticas para a OH, especialmente nos pacientes que apresentem essas condições. Unitermos-Quadril; artroplastias; ossificação heterotópica SUMMARY Risk factors of heterotopic bone formation in hip arthroplasty Eighty-four hip arthroplasties in 79 patients were reviewed to investigate predisposing factors for the development of heterotopic bone formation. Factors such as gender, age, weight, preoperative diagnoses, anesthesia, surgical approach, and the use of cement were considered. The frequency of HBF was 45.2%. The risks are higher men aged between 50-60 years with osteonecrosis of the femoral head or hypertrophic osteoarthrosis. This study recommends that patients under these conditions should receive postoperative prophylaxis.

To report our experience with a novel alternative method of freehand interlocking of intramedulla... more To report our experience with a novel alternative method of freehand interlocking of intramedullary nails. This method requires the use of only anterior-posterior image intensification and an intramedullary guide wire to verify screw placement. Our results are compared with historical results in the literature. Methods: A total of 815 patients were treated using this technique from January 2008 to December 2012; 603 patients had fractures of the tibia and 212 had fractures of the femur. Results: The mean duration of surgery for tibial shaft fractures was 55.6 minutes (range 42-60 minutes) and that for fractures of the femur was 78 minutes (range 50-90 minutes). The mean time for each distal locking was 3.8 minutes (2.5-5.1 minutes), with 7.65 seconds of exposure to radiation during each block. Conclusions: The surgical technique is simple, easy and reproducible. Mean time of surgery and radiation exposure was less than that in the literature. A comparative study should be performed.

The standardization of this method is being more and more debated and evaluated, thus contributing to improved quality of published trials. The purpose of this article is to present the guidelines suggested by international groups to authors and publishers who critically evaluate controlled and randomized clinical trials.

T he Madelung deformity can result in pain and decreased function of the wrist and hand. None of ... more T he Madelung deformity can result in pain and decreased function of the wrist and hand. None of the surgical techniques available has been shown consistently to improve grip strength, range of movement or relieve pain. In this prospective study we have treated 18 patients with the Madelung deformity (25 wrists) by wedge subtraction osteotomy of the radius and shortening of the ulna. Our results show statistically significant improvement in grip strength and range of movement of the wrist and forearm. Pain improved in 80% of the patients and 88% were satisfied with the appearance. One patient had a wound infection and another developed reflex sympathetic dystrophy. Two had some recurrence due to continued growth of the ulna and it is recommended that the procedure be delayed until skeletal maturity, or else combined with epiphysiodesis of the ulna.

We undertook a retrospective study of 50 consecutive patients (41 male, 9 female) with an infecte... more We undertook a retrospective study of 50 consecutive patients (41 male, 9 female) with an infected nonunion and bone defect of the femoral shaft who had been treated by radical debridement and distraction osteogenesis. Their mean age was 29.9 years (9 to 58) and they had a mean of 3.8 (2 to 19) previous operations. They were followed for a mean of 5.9 years (2.0 to 19.0). The mean duration of the distraction osteogenesis was 24.5 months (2 to 39). Pin-track infection was observed in all patients. The range of knee movement was reduced and there was a mean residual leg-length discrepancy of 1.9 cm (0 to 8) after treatment. One patient required hip disarticulation to manage intractable sepsis. In all, 13 patients had persistant pain. Bony union was achieved in 49 patients at a mean of 20.7 months (12 to 35). Although distraction osteogenesis is commonly used for the treatment of infected femoral nonunion with bone defects, it is associated with a high rate of complications.
