A growing body of research suggests an association between attention deficit hyperactivity disord... more A growing body of research suggests an association between attention deficit hyperactivity disorder (ADHD) and allergic disorders, but little work has been done to explore the role of external factors such as parental smoking at home in the development of comorbid ADHD and allergic disorders. This study aimed to examine the association between allergic diseases and ADHD adjusted for exposure to parental smoking at home in early adolescents. We recruited 250 male (41.7%) and 350 female (58.3%) adolescents (mean [SD] age, 13.29 [0.52] years) via chain-referral sampling. Their ADHD symptoms were assessed by the parent proxy-report version of the Chinese Strengths and Weaknesses of Attention-Deficit/Hyperactivitysymptoms and Normal-behaviours (SWAN) rating scale. Data on the participants' history of clinician-diagnosed allergic diseases, family socio-demographics, and parental smoking habit were collected using a parent-completed questionnaire. Regression analyses were performed to examine the associations of interest. The levels of ADHD symptoms were comparable between allergic and non-allergic participants after controlling for child and family demographics and parental smoking at home. Notably, the risk of probable ADHD was particularly high in participants with food allergies (odd ratio = 4.51, p = 0.011) but not in those with allergic rhinitis after adjusting for parental smoking at home. Our findings suggest that secondhand smoke exposure at home is a potential risk factor underlying the link between ADHD and allergic diseases. Current management guidelines should emphasize the importance of early identification and cessation of tobacco smoke exposure for prevention of comorbidity of ADHD and allergic disorders. Stimulant medications such as methylphenidate are the most commonly prescribed treatment for ADHD, but the association between their therapeutic use and the risk of seizures is unclear. This study aims to investigate the association between methylphenidate treatment and the risk of seizure in patients with ADHD. Methods: We conducted an observational study using population-based, electronic medical record database from the Hong Kong Clinical Data Analysis & Reporting System to identify individuals aged 6 to 25 years who were treated with methylphenidate between January 1, 2001, and December 31, 2017. Patients treated with methylphenidate who had seizures were included in the subsequent analyses and a self-controlled case series design was used to control for time-invariant patient characteristics. Additional analysis was conducted using skin infection as a negative control outcome. Relative incidence of seizure during periods when patients were exposed to methylphenidate was compared with non-exposed periods. Findings: Among 29,604 patients prescribed methylphenidate, 269 had incident seizures during the study period. The mean (SD) age at baseline was 6•66 (2•01) years and 199 (74•0%) were male. The overall incidence of seizure during methylphenidate treatment was 4•4 per 10 000 patient-years. An increased risk of seizure was detected during the 30-day period following initiation of methylphenidate compared to nonexposed periods, with an incidence rate ratio (IRR) of 4•01 (95% CI, 2•09-7•68). No increase in risk was identified during the 31 to 180 days of the treatment (IRR, 1•13; 95% CI, 0•56-2•25) or during subsequent treatment (IRR, 1•38; 95% CI, 0•92-2•07). No increased risk was identified in all risk windows for the negative control outcome analysis. No patient died due to seizure. Interpretation: The incidence of seizures was higher in the period immediately after the start of the methylphenidate treatment compared to the non-exposed period. The risk returned to baseline levels during continuation of methylphenidate treatment. The association between methylphenidate treatment and seizures immediately following initiation of medication can be seen as a potential safety signal. Monitoring of 5 neurological outcomes in methylphenidate users is essential when they first start on medication is recommended.

METHODS: A population-representative sample of 514 Chinese preschool children was recruited when in kindergarten (K3). Parents reported on their socioeconomic status and children's sleep duration. The cohort was reassessed 3 years later when the children were in Grade 3 (P3). Parents reported on children's sleep patterns and ADHD symptoms. Information on OSA diagnosis and methylphenidate use was retrieved from health records. RESULTS: Among the 514 parent-child dyads (mean [SD] age, 5.52 [0.33] years), 411 were reassessed (80.0% retention; 9.35 [0.33] years) at follow-up. There were no significant baseline differences between follow-up and drop-out groups. A gradient relationship was observed between probable ADHD in P3 and sleep duration in K3. The risk of probable ADHD was 15.5 per 100 for children with <8 h of sleep in K3, whereas it was 1.1 per 100 for children with 11-12 h of sleep in K3. The adjusted risk ratio was 14.19 (p = 0.02). CONCLUSIONS: Sleep deprivation in early childhood is associated with higher risk of ADHD in middle childhood.

This study aims at providing a profile of the association between different types of child victim... more This study aims at providing a profile of the association between different types of child victimization and polyvictimization and health-related quality of life (HRQoL) among school-aged children, and examining the impact of family structure and social support on the negative health consequences associated with violent victimization. We conducted a cross-sectional school survey in Hong Kong using a two-stage stratified sampling procedure. The final sample comprised 4139 children's self-reports and proxy-reports (boys = 51.5%; mean age = 6.3). The main outcome was HRQoL measured with the Pediatric Quality of Life Inventory (PedsQL). Family structure was represented by parents' marital status, major caregivers, number of siblings and the living arrangement of children. Child victimization, social support, and demographic characteristics were also measured. All types of child victimization were associated with compromised HRQoL, and the strength of association varied across different types of child victimization. Family structure (in particular the number of siblings and whether additional childcare was received from grandparents) and social support were associated with better HRQoL. The negative associations between child victimization and polyvictimization and HRQoL were reduced when there was an adjustment made for family structure and social support. Findings show that family structure and social support are related to a reduction in negative health consequences for child victimization. The varying strengths of negative associations between victimization and HRQoL highlight the possibility that the effects of child victimization on health might not be homogeneous.

Technology-based anticipatory guidance has been suggested to reinforce the effectiveness of injury prevention and improve parents' knowledge of child safety. Objective: This study aims to examine the effectiveness of a technology-based injury prevention program with parental anticipatory guidance for enhancing mothers' knowledge of child safety. Methods: In this randomized controlled trial, 308 mothers will be recruited from the antenatal clinics and postnatal wards of two major public hospitals in Hong Kong. Participating mothers will be randomly assigned into intervention and control groups. Mothers in the intervention group will be given free access to a technology-based injury prevention program with anticipatory guidance, whereas mothers in the control group will be given a relevant booklet on parenting. The injury prevention program, available as a website or on a mobile app, includes behavioral components based on the Theory of Planned Behavior. The primary outcome measure will be the change in the mother's knowledge of child safety. The secondary outcome measures will be age-appropriate domestic safety knowledge, attitudes, intentions, perceived behavioral control, and self-reported behavior related to home safety practice. We will also determine dose-response relationships between the outcome measures and the website and mobile app usage. Results: Enrolment of participants will begin in October 2016. Results are expected by June 2018. Conclusions: Parents will be able to easily access the domestic injury prevention website to find information regarding child injury prevention. It is anticipated that the technology-based intervention will help parents improve their knowledge of child safety and raise their awareness about the consequences of domestic injuries and the importance of prevention.

A retrospective study of subjects aged under 19 years with a primary diagnosis of child maltreatment admitted to hospitals in Hong Kong from 2001 to 2010. Data were retrieved from the centralised database of all 42 public hospitals in the Hospital Authority. Child maltreatment incidence rate. A consistent seasonal pattern was found for non-sexual maltreatment in children aged 6-18 years (p<0.001). Hospitalisations peaked in May and October but dipped in August and January. Stimulant medications such as methylphenidate are the most commonly prescribed treatment for ADHD, but the association between their therapeutic use and the risk of seizures is unclear. This study aims to investigate the association between methylphenidate treatment and the risk of seizure in patients with ADHD. Methods: We conducted an observational study using population-based, electronic medical record database from the Hong Kong Clinical Data Analysis & Reporting System to identify individuals aged 6 to 25 years who were treated with methylphenidate between January 1, 2001, and December 31, 2017. Patients treated with methylphenidate who had seizures were included in the subsequent analyses and a self-controlled case series design was used to control for time-invariant patient characteristics. Additional analysis was conducted using skin infection as a negative control outcome. Relative incidence of seizure during periods when patients were exposed to methylphenidate was compared with non-exposed periods. Findings: Among 29,604 patients prescribed methylphenidate, 269 had incident seizures during the study period. The mean (SD) age at baseline was 6•66 (2•01) years and 199 (74•0%) were male. The overall incidence of seizure during methylphenidate treatment was 4•4 per 10 000 patient-years. An increased risk of seizure was detected during the 30-day period following initiation of methylphenidate compared to nonexposed periods, with an incidence rate ratio (IRR) of 4•01 (95% CI, 2•09-7•68). No increase in risk was identified during the 31 to 180 days of the treatment (IRR, 1•13; 95% CI, 0•56-2•25) or during subsequent treatment (IRR, 1•38; 95% CI, 0•92-2•07). No increased risk was identified in all risk windows for the negative control outcome analysis. No patient died due to seizure. Interpretation: The incidence of seizures was higher in the period immediately after the start of the methylphenidate treatment compared to the non-exposed period. The risk returned to baseline levels during continuation of methylphenidate treatment. The association between methylphenidate treatment and seizures immediately following initiation of medication can be seen as a potential safety signal. Monitoring of 5 neurological outcomes in methylphenidate users is essential when they first start on medication is recommended.

We investigated the association between parenting style and child maltreatment in Hong Kong, and examined whether family socio-economic status and child gender moderate this relationship. METHODS: A population-representative sample of 514 Chinese preschool children was recruited when in kindergarten (K3). Parents reported on their socioeconomic status and children's sleep duration. The cohort was reassessed 3 years later when the children were in Grade 3 (P3). Parents reported on children's sleep patterns and ADHD symptoms. Information on OSA diagnosis and methylphenidate use was retrieved from health records. RESULTS: Among the 514 parent-child dyads (mean [SD] age, 5.52 [0.33] years), 411 were reassessed (80.0% retention; 9.35 [0.33] years) at follow-up. There were no significant baseline differences between follow-up and drop-out groups. A gradient relationship was observed between probable ADHD in P3 and sleep duration in K3. The risk of probable ADHD was 15.5 per 100 for children with <8 h of sleep in K3, whereas it was 1.1 per 100 for children with 11-12 h of sleep in K3. The adjusted risk ratio was 14.19 (p = 0.02). CONCLUSIONS: Sleep deprivation in early childhood is associated with higher risk of ADHD in middle childhood.

METHODS: This is a randomized controlled trial in which we recruited students from 12 secondary schools in Hong Kong, China. Participants were randomly assigned in a 1:1 ratio to an intervention or a control arm after stratification for school from October 2013 to June 2014. Participants were not blinded to allocation because of the nature of the intervention. Students in the intervention arm received an after-school, PYD-based sports mentorship for 18 weeks. This study aims at providing a profile of the association between different types of child victim... more This study aims at providing a profile of the association between different types of child victimization and polyvictimization and health-related quality of life (HRQoL) among school-aged children, and examining the impact of family structure and social support on the negative health consequences associated with violent victimization. We conducted a cross-sectional school survey in Hong Kong using a two-stage stratified sampling procedure. The final sample comprised 4139 children's self-reports and proxy-reports (boys = 51.5%; mean age = 6.3). The main outcome was HRQoL measured with the Pediatric Quality of Life Inventory (PedsQL). Family structure was represented by parents' marital status, major caregivers, number of siblings and the living arrangement of children. Child victimization, social support, and demographic characteristics were also measured. All types of child victimization were associated with compromised HRQoL, and the strength of association varied across different types of child victimization. Family structure (in particular the number of siblings and whether additional childcare was received from grandparents) and social support were associated with better HRQoL. The negative associations between child victimization and polyvictimization and HRQoL were reduced when there was an adjustment made for family structure and social support. Findings show that family structure and social support are related to a reduction in negative health consequences for child victimization. The varying strengths of negative associations between victimization and HRQoL highlight the possibility that the effects of child victimization on health might not be homogeneous.

Background: Provision of anticipatory guidance for parents is recommended as an effective strateg... more Background: Provision of anticipatory guidance for parents is recommended as an effective strategy to prevent injuries among young children. Technology-based anticipatory guidance has been suggested to reinforce the effectiveness of injury prevention and improve parents' knowledge of child safety. Objective: This study aims to examine the effectiveness of a technology-based injury prevention program with parental anticipatory guidance for enhancing mothers' knowledge of child safety. Methods: In this randomized controlled trial, 308 mothers will be recruited from the antenatal clinics and postnatal wards of two major public hospitals in Hong Kong. Participating mothers will be randomly assigned into intervention and control groups. Mothers in the intervention group will be given free access to a technology-based injury prevention program with anticipatory guidance, whereas mothers in the control group will be given a relevant booklet on parenting. The injury prevention program, available as a website or on a mobile app, includes behavioral components based on the Theory of Planned Behavior. The primary outcome measure will be the change in the mother's knowledge of child safety. The secondary outcome measures will be age-appropriate domestic safety knowledge, attitudes, intentions, perceived behavioral control, and self-reported behavior related to home safety practice. We will also determine dose-response relationships between the outcome measures and the website and mobile app usage. Results: Enrolment of participants will begin in October 2016. Results are expected by June 2018. Conclusions: Parents will be able to easily access the domestic injury prevention website to find information regarding child injury prevention. It is anticipated that the technology-based intervention will help parents improve their knowledge of child safety and raise their awareness about the consequences of domestic injuries and the importance of prevention.

A retrospective study of subjects aged under 19 years with a primary diagnosis of child maltreatment admitted to hospitals in Hong Kong from 2001 to 2010. Data were retrieved from the centralised database of all 42 public hospitals in the Hospital Authority. Child maltreatment incidence rate. A consistent seasonal pattern was found for non-sexual maltreatment in children aged 6-18 years (p<0.001). Hospitalisations peaked in May and October but dipped in August and January. No significant seasonal patterns were found for sexual maltreatment or among children under 6 years. The seasonal pattern of child maltreatment coincided with the two school examination periods. The annual child maltreatment hospitalisation rate in Hong Kong in 2010 was 73.4 per 100 000 children under 19 years, more than double that in 2001.
