The differential diagnosis for a wide complex tachycardia includes all causes of supraventricular... more The differential diagnosis for a wide complex tachycardia includes all causes of supraventricular tachycardia (SVT) with bundle branch block or all causes of SVT with antegrade pre-excitation by bystander involvement of any accessory pathways, myocardial or bundle brunch ventricular tachycardia, and antidromic (atriofascicular or nodofascicular/nodoventricular) and other pre-excited reciprocating tachycardias. We present a case of wide complex QRS tachycardia with a left bundle branch block QRS morphology.
The predictive value of leukocyte counts has been reported in patients with acute myocardial infa... more The predictive value of leukocyte counts has been reported in patients with acute myocardial infarction (AMI). We aimed to evaluate the predictive value of the total leukocyte count and neutrophil–lymphocyte (N/L) ratio for mortality due to AMI. A total of 522 patients with acute ST-elevated MI were included in the study. The study population was divided into tertiles based on admission N/L ratio values. High (n = 174) and low N/L (n = 348) ratio groups were defined as patients having values in the third tertile (>5.77) and lower 2 tertiles (≤5.77), respectively. The high N/L ratio group had a significantly higher incidence of in-hospital cardiovascular mortality (13.8% vs 4.6%, P < .001). An N/L ratio >5.77 was found to be an independent predictor of in-hospital cardiovascular mortality (hazard ratio: 3.78, 95% confidence interval: 1.71-8.30, P = .001). A high N/L ratio is a strong and independent predictor of in-hospital cardiovascular mortality of AMI with ST elevation.

Coronary angiography showed a significant proximal right coronary artery stenosis and percutaneous transluminal coronary angioplasty with stenting was successfully performed. Ten minutes after the procedure the patient experienced sudden-onset of severe back and left flank pain accompanied by nausea and vomiting. The abdominal examination revealed moderate to severe tenderness, localized mainly over the left upper quadrant. Her lower extremities were well perfused with no ischemic or gangrenous lesions. At first, we believed that the patient’s initial symptoms were probably related to retroperitoneal hemorrhage due to severe back pain, and emergency abdominal computer tomography angiogram was planned, which revealed no retroperitoneal

The most significant medical problem associated with this syndrome is cardiovascular disease caused by narrowed arteries. Because of the variability in the way that WS affects different people, it often goes undiagnosed for many years. Clinical suspicion is essential because the diagnostic genetic finding is not detectable on routine chromosomal analysis and is not cost effective to screen all patients. We present the nicely transthoracic echocardiographic and angiographic images of the SVS caused by discrete fibrous membrane in a young patient with WS. It is well-known that autonomic nerve modulation has an important role in the occurrence of ventr... more It is well-known that autonomic nerve modulation has an important role in the occurrence of ventricular tachyarrhythmias in Brugada syndrome. A 59-year-old man underwent cardiac evaluation before surgery for urinary bladder stone. He had no cardiac complaints and the only coronary risk factor was heavy smoking. The electrocardiogram showed a saddleback type ST-segment elevation in leads V1-V2, and left axis deviation. During exercise stress test, ventricular tachycardia with a left bundle branch block pattern appeared, and the saddleback type ST-segment elevation in V2 changed into a coved-type ECG at the recovery phase. The ventricular tachycardia was hemodynamically stable and normalized without medication. An echocardiogram showed normal left and right ventricular functions, and subsequent coronary angiography revealed normal coronary arteries.
