Although pelvic lymphadenectomy is the gold standard to assess the status of pelvic lymph nodes, a diagnostic imaging tool to non-invasively explore patients and to detect metastases, both in staging and in re-staging phase, would be of particular help in clinical management. In staging phase, while choline PET/CT specificity has been reported to be fairly high in lymph nodal detection, its sensitivity is not adequate due to its spatial resolution. Its role in the evaluation of patients with biochemical relapse or with suspected relapse has been successfully documented. Medical and surgical management of benign prostatic hyperplasia (BPH) is a high economic burden, ... more Medical and surgical management of benign prostatic hyperplasia (BPH) is a high economic burden, but no objective reliable information is available about epidemiology, prescription modalities of BPH-related medications, and surgical practice. We conducted a nationwide , comprehensive survey to precisely describe actual clinical practice in this field. Materials & Methods: From 2004 to 2006, we yearly extracted information about all patients treated in France for BPH from two national databases registering all medical prescriptions and all surgical acts for reimbursement purpose. Content, date, prescriptor (urologist or general practitioner) of medical prescriptions and surgical data (date, location, type of surgery and hospital stay duration) were obtained. Descriptive analysis was conducted about medical treatment (types, associations, evolution, prescriptors) and surgical activity (number and types of interventions, hospital stay duration, and interregional variations). results: This survey concerned 1,975,494 patients, with a median follow-up of 1.29 years. Prevalence of BPH requiring specific management was 13% of patients over 50. Initial management was medical in 94.8% of cases, and 5.2% of patients had surgery as firstline treatment. Concerning medical management (table 1), the ratio between urologists and general practitioners was 1/10. Most widely used drugs were α1-blockers, then plants and 5α-reductase inhibitors. The most frequent combination therapy was α1-blockers and plants. Over the study period, treatment switch rate was 11%, and 4.8% of patients initially medically treated underwent surgery (depending on medical treatment modality). In 25% of cases, more than 8 drugs were associated to BPH medications. Over the study period, 48996 surgical acts were registered, of which 89% were endoscopic procedures. Number of surgical acts adjusted to the number of men over 50 was highly variable according to regions considered. Conclusions: This study provides a previously unreleased, dynamic evaluation of real clinical practice by using registries initially not dedicated to this purpose. These data raise the unique opportunity to estimate and monitor the gaps between guidelines and clinical practice in the future, and thus identify the critical targets for health policies in the field of BPH.

The aim of the study was to design the HACCP system audit method which allows precise assessment ... more The aim of the study was to design the HACCP system audit method which allows precise assessment of the system functioning in practice. The method was based on specially elaborated audit questionnaire, covering all HACCP steps and principles, associated with analysis of audit findings by FMEA. External third party audits were carried out in two medium-size bakeries located in Poland. The method allowed precise identification of high and critical risks in HACCP areas of verification and recordkeeping. In view of the obligatory of HACCP system in food industry and periodically emerging food safety scandals in the EU and other countries, it appears advisable to strength control and use methods enabling precise identification of the risk. Therefore, the incorporation of FMEA analysis within the verification procedure of HACCP system may be a convenient tool for better food safety assurance. The designed method is ready to use in all types food enterprises.

[11C]choline positron emission tomograhy can be useful to detect metastatic disease and to locali... more [11C]choline positron emission tomograhy can be useful to detect metastatic disease and to localize isolated lymph node relapse after primary treatment in case of prostate-specific antigen failure. In case of lymph node failure in prostate cancer patients, surgery or radiotherapy can be proposed with a curative intent. Some reports have suggested that radiotherapy could have a role in local control of oligometastatic lymph node disease. This is the first reported case of [11C]choline positron emission tomography-guided helical tomotherapy concomitant with estramustine for the treatment of pelvic-recurrent prostate cancer. At 24 months after the end of helical tomotherapy, prostate-specific antigen was undetectable and no late toxicities were recorded. A disease-free survival of 24 months, in the absence of any type of systemic therapy, is uncommon in metastatic prostate cancer. The therapeutic approach of the case report is discussed and a literature review on the issue is presented.

The radiographic appearance of inflammatory pseudotumor is quite non-specific and the definitive diagnosis is based on the histological evaluation of tissue specimens. Resection of the lesion is the treatment of choice. However, nonsurgical treatments such as radiotherapy and steroids have been employed in the setting of incomplete surgical resection, tumor recurrence, and patients being unfit for surgery. The case described here is being reported because of the rare mediastinal location and atypical treatment approach including salvage irradiation and monitoring with FDG-PET/CT.
