Knitted sensing fabrics with copper, nickel and tungsten wire elements have been produced with resistances ranging from 3 to 130 Ω. The most successful samples have been created using textile-wrapped, enamelled wire and not only the textile character of the sensing element was enhanced, but also its tensile strength. A mathematical relationship has been derived between the temperature and resistance of the knitted sensors and this can be used to optimise its dimensions to achieve a targeted reference resistance. The temperature-resistance curves demonstrate a linear trend with a coefficient of determination in the range of 0.99–0.999 and can be integrated into garments to monitor skin temperatures.

At Week 16 (primary endpoint) for the per-protocol population, significantly more APR20 (29.4%; P ¼ 0.0235) and APR30 (42.8%; P < 0.0001) patients achieved a modified ACR20 response vs placebo (18.9%). LS mean change from baseline in HAQ-DI at Week 16 was significantly greater in APR30 (-0.19; P < 0.05) patients and numerically greater in APR20 (-0.14) patients vs placebo (-0.07). At Week 24, significant improvements across multiple secondary endpoints were seen with APR vs placebo, including physical function, fatigue, and pain (Table 1). A dose-related effect was observed, with greater efficacy generally seen in patients receiving APR30. A significantly greater proportion of patients receiving APR30 achieved minimal clinically important differences for HAQ-DI and achieved a good or moderate EULAR response or PsARC response vs placebo. APR was generally well tolerated. The most common adverse events were diarrhoea, nausea, headache, and URTI. Most AEs were mild to moderate in severity and did not lead to discontinuation. Conclusion: Over 24 weeks, APR demonstrated statistically significant and clinically meaningful improvements in the signs and symptoms of PsA, physical function, and pain. APR demonstrated an acceptable safety profile and was generally well tolerated.

Improvements in HAQ-DI, PsAQoL, pain and fatigue were maintained to Wk48. In patients with !3% skin involvement at baseline (60.8% CZP-patients), PASI75/90 responses were maintained from Wk24 to Wk48. Radiographic progression remained low (CFB-mTSS: Wk24 ¼ 0.00, Wk48 ¼ 0.13). In the safety set (N ¼ 393), adverse events (AEs) occurred in 304 patients (77.4%) and serious AEs in 39 (9.9%), including 8 serious infections (2.0%; 1 case of tuberculosis [0.3%]), and 3 deaths (0.8%). Conclusion: Clinical efficacy of CZP was maintained over 48 weeks in PsA patients with and without prior anti-TNF exposure. Low radiographic progression and improvements in PASI were maintained to Wk48. Safety profile was in line with that observed for CZP in RA.

Temperature Sensing Fabric is a double layer knitted structure; made of polyester as a basal yarn and embedded with fine metallic wire as sensing element. Fabricated on an industrial scale computerised knitting machine, the TSF samples made of nickel, tungsten and copper wires in the form of bare and insulated form were used in this study. In order to analy ze the performance of TSF under variety of strain and humidity environ ment, tailor-made methods were devised, as relevant standards were not available. In strain testing, the TSF samples made of insulated sensing wire performed better in co mparison to the samples made with bare sensing wires. Results of the humidity testing revealed that all kinds of TSF samples (either made of sensing elements of insulated wire or bare wire) could be used in a high hu mid ity environ ment (up to 90% RH) without any compro mise in their sensing performance.

A cross-sectional study of 472 patients attending GP clinics between March and August 2009 in Karachi. Participants completed questionnaires to assess demographic details, somatic symptoms, anxiety, depression, and QOL. The patients' GP recorded whether the presenting complaint was medically unexplained or medically explained. MUS subjects in our study were more educated, had better social support and fewer financial problems, were less depressed and had a better QOL than subjects who had medically explained symptoms (non-MUS). Both groups (MUS and non-MUS) were comparable in terms of anxiety and number of somatic symptoms, but non-MUS subjects were more depressed than the MUS group. In a regression analysis, the num...

Most gliomas encountered in the CPA in either children or adults involve the CPA as exophytic extensions of primary brain stem and/or cerebellar tumors. We encountered an unusual case of a giant CPA pilocytic astrocytoma arising from the proximal

This paper presents a new textile-based strain sensor design and shows the effect of base fabric parameters on its sensing properties. Sensing fabric could be used to measure articulations of the human body in the real environment. The strain sensing fabric was produced by using electronic flat-bed knitting technology; the base fabric was produced with elastomeric yarns in an interlock arrangement and a conductive yarn was embedded in this substrate to create a series of single loop structures. Experimental results show that there is a strong relationship between base fabric parameters and sensor properties.

Retrospective prevalence-based study. Combined linked anonymised electronic datasets gathered on a cohort of women enrolled on the Growing Up in Wales: Environments for Healthy Living (EHL) study. Women were categorised into two groups: normal body mass index (BMI; n=260) and overweight/obese (BMI>25; n=224). 484 singleton pregnancies with available health service records and an antenatal BMI. Total health service utilisation (comprising all general practitioner visits and prescribed medications, inpatient admissions and outpatient visits) and direct healthcare costs for providing these services in the year 2011-2012. Costs are calculated as cost of mother (no infant costs are included) and are related to health service usage throughout pregnancy and 2 months following delivery. There was a strong association between healthcare usage cost and BMI (p<...

The policies for better health, poverty reduction, and less inequality, throughout the world, req... more The policies for better health, poverty reduction, and less inequality, throughout the world, require thorough understanding of both the processes and causal paths that underlie the intricate relationship between health and wealth (income). This is deemed difficult, contingent, and only partially understood. The adage 'health is wealth' is still, primarily, an intuitive proposition. A vast majority of researchers instead present theoretical and empirical arguments of the reverse proposition, i.e. 'wealth is health'. A recent strand of the literature, however, reflects changes in the perceptions: improvements of health and longevity are no longer viewed as a mere end-or byproduct of economic development; but argued as one of the key determinants of, and therefore means to achieve, economic development and poverty reduction. Hence, better health does not have to wait for an improved economy; rather, measures to reduce the burden of disease, to give children healthy childhoods, to increase life expectancy etc. will in themselves contribute to creating richer economies. Drawing on the traditional and emerging perspectives on the health-income relationship, this literature review presents a non-exhaustive survey of existing methodological approaches and their results that are applied to track and measure how health influences economic outcomes.

BACKGROUND Lipomatous medulloblastoma is a rare but apparently distinct variant of medulloblastom... more BACKGROUND Lipomatous medulloblastoma is a rare but apparently distinct variant of medulloblastoma. There have been only 14 prior published cases. We report an additional case of an adult who presented with a multicentric form of this unique lesion. METHODS A 45-year-old woman underwent magnetic resonance imaging for evaluation of progressive headaches, nausea/ vomiting, vertigo, and ataxia. Multiple bilateral mass lesions in the posterior fossa were noted, along with obstructive hydrocephalus and significant mass effect. RESULTS Via a right suboccipital and retrosigmoid craniotomy the largest cerebellar lesion was resected and a second lesion biopsied. The third mass on the contralateral side was not disturbed. Postoperatively the patient received fractionated radiotherapy to a dose of 54 Gy and chemotherapy with cisplatin, PCNU, and vincristine for 1 year. She is alive without deficit, with a Karnofsky Performance Status of 100, and with no evidence of disease on neuroimaging 3 years after diagnosis. CONCLUSION This distinct variant of medulloblastoma appears to occur in adults only and has a uniquely favorable prognosis, even with incomplete resection with institution of appropriate adjuvant therapies. Multicentricity does not imply a less favorable prognosis.

A mixed methods approach was taken to examine fatigue in a cohort of people with AS. Fatigue levels were evaluated from 3 consecutive monthly questionnaires. Open-ended questions on fatigue were analyzed using thematic analysis and logistic regression was used to examine quantitative data. In addition, fatigue levels were examined before and after treatment with anti-tumor necrosis factor (TNF) compared to nontreated controls. Three hundred forty-eight of 385 participants completed a fatigue questionnaire. Fatigue was reported to have significant physical, social, and psychological effects. A third of the participants reported that there was nothing they could do to reduce their fatigue, whereas other participants reported that medication, exercise, and resting helped. The main factor associated with fatigue was pain [β-coefficient: 0.74 (95% CI: 0.66 to 0.81)], whereas depression was much less strongly associated. However, these factors only explained 40% of the variation in fatigue levels. Starting anti-TNF therapy reduced fatigue and pain levels compared to the period of time before taking anti-TNF [difference: 14.4 (95% CI: 5.3 to 23.5) on a scale of 0-100] and this reduction was not seen in controls over the same period. Fatigue is not strongly associated with anxiety, motivation, and depression; instead the factor most associated with fatigue is pain. This suggests that in addition to treatments to reduce disease activity, strategies for alleviating fatigue in AS should focus on pain management techniques and actively treating inflammation.

In one, this led to misdiagnosis of the lesion as an abscess. On histological evaluation, a large number of white blood cells or macrophages was seen at the neoplastic sites. Reasons for leukocyte accumulation around metastatic brain neoplasms are discussed. In contrast to the current reports that indium-labeled leukocyte scans can differentiate intracranial infection from tumor, these cases demonstrate their lack of specificity in the detection of brain abscess.

Object The preponderance of progesterone receptors (PRs) and the scarcity of estrogen receptors (... more Object The preponderance of progesterone receptors (PRs) and the scarcity of estrogen receptors (ERs) in meningiomas are well known. The expression of PRs may relate to tumor grade and recurrence. Cytogenetic abnormalities are associated with aggressive behavior, recurrence, and progression. In this study, the authors focus on the prognostic implications of hormone receptors in meningiomas to help determine the clinical and biological aggressiveness of tumors and their correlations with cytogenetic abnormalities. Methods Two hundred thirty-nine patients with meningiomas were separated into three groups. Group 1 (PR-positive group) comprised patients whose meningiomas displayed expression of PRs alone. Group 2 (receptor-negative group) included patients whose lesions did not have receptors for either progesterone or estrogen. Group 3 (ER-positive group) included patients whose tumors displayed expression of ERs. Clinical and histological findings, proliferative indices, tumor recurre...

METHODS: Primary care records were linked with all hospital admissions and deaths caused by MI or stroke in Wales for the years 1999-2010. The linked data were then stratified by AS diagnosis and survival analysis was used to obtain the incidence rate of MI and separately cerebrovascular disease (CVD)/stroke. Cox regression was used to adjust for gender and age. Logistic regression was used to examine prevalence of diabetes, hypertension, or hyperlipidemia for those with AS compared to those without. RESULTS: There were 1686 AS patients (75.9% male, average age 46.1 years) compared to 1,206,621 controls (48.9% male, average age 35.9 years). Age- and gender-adjusted hazard ratios for MI were 1.28 (95% CI: 0.93 to 1.74) P = 0.12, and for CVD/stroke 1.0 (95% CI: 0.73 to 1.39) P = 0.9, in AS compared to controls. The prevalence ...

This region can be the site of vascular, neoplastic, degenerative, congenital, or traumatological diseases, 2,8,17, 21,22,25 the operative management of which demands an indepth understanding of the complex anatomy. We studied the microsurgical anatomy of this region, concentrating on the third segment (V 3) of the VA, which extends from the transverse foramen of the axis to the dural penetration by the VA. We also studied the loops, branches, supporting fibrous rings, and surrounding venous structures of the V 3 , as well as the relationships of these anatomical elements to their surrounding structures. Our study revealed an astonishing anatomical resemblance between this complex and the cavernous sinus. A review of the literature showed their related embryological development and functional and pathological features, as well as similar transitional patterns of the structures of the V 3 and petrous-cavernous internal carotid artery (ICA) walls. Hence, because of its analogy to the cavernous sinus, we call this suboccipital anatomical complex the "suboccipital cavernous sinus." We also discuss its various functional roles and its relationship to the adjacent structures and the clinical and surgical implications of these factors. Materials and Methods The craniocervical regions of ten cadaver heads (20 sides) were dissected with the aid of an operating microscope (magnification 4-40). The cadavers previously had been embalmed in a formalin solution. The ICAs, the VAs, and the internal jugular veins (IJVs) were dissected, cannulated, and irrigated with saline to remove any residual blood clots in the lumens. Two colored silicone rubber mixes were prepared by first adding red and blue powder paint (Tempera Powder Paint; Sargen Art, Inc., Hazleton, PA) to the liquid solvent poly-di-methylsiloxane (Dow Corning, Inc., Midland, MI). Silicone rubber was then added immediately before administering the injections. The mixture was prepared so that it was sufficiently fluid to allow perfusion of the smaller blood vessels; it solidified after injection. To further ensure adequate perfusion of the microcirculation, we perfused the arteries individually under pressure (by injecting one ICA while clamping the contralateral ICA and both VAs). Forty hours after the injections were given, the cadavers were ready for dissection. In two additional formalin-fixed specimens, the V 3 and its surrounding structures were dissected under the microscope, from the transverse foramen of the axis to the dural ring. Serial cross sections were made and embedded in paraffin, and then histological cross sections were cut and stained with hematoxylin and eosin and Masson trichrome. All specimens were examined under the microscope.
