In der zweiten Woche der Behandlung erwiesen sich zwei Terbinafin-Patienten (10 yo), in der dritten Woche 14 Terbinafin-Patienten (70 yo) und finf Bifonazol-Patienten (25 %) als mykologisch saniert. Die vorgelegte kontrollierte klinische Studie zeigt daher, daB Terbinafin ein schnell wirksames und gut vertragliches Medikament fur die Behandlung der Pityriasis versicolor darstellt.

It codes a DNA-helicase, whose defects cause genomic instability. The highest incidences are reported in Japan and Sardinia (Italy). On this major island of the Mediterranean Basin, the WS cases have been observed in the northern areas. The authors describe the apparently first case reported in southern Sardinia, a 51-year-old woman, who was born in and resides in the province of Cagliari. She presented with a 9-year history of an intractable leg ulcer and other characteristic symptoms, including "bird-like" face, high-pitched voice, premature greying, short stature, abdominal obesity in contrast with thin body type, scleroderma-like legs, decreased muscle mass, diabetes, atherosclerosis, and premature menopause. Darier's disease is a disorder of keratinisation which usually shows autosomal dominant inher... more Darier's disease is a disorder of keratinisation which usually shows autosomal dominant inherance, although frequent sporadic cases also occur. Onset is usually between the ages of 5 and 10 years, with a peak at puberty. The disease is characterized by papular hyperkeratotic lesions distributed prevalently in seborrhoeic areas of the scalp, face, chest and back. Other features my be seen, including nail involvement, palmar pits, hypertrophic intertriginous plaques, verrucous papules on the dorsal hands. Involvement of the mucous membranes is less frequent. Histology is characterised by dyskeratosis with round bodies and granules and by suprabasal acantholysis with lacunae or clefts in which the cells of basal layer, disposed in a single row, grow in an irregular manner. ABSTRACT Background Allopurinol is extensively prescribed for conditions associated with urate e... more ABSTRACT Background Allopurinol is extensively prescribed for conditions associated with urate excess, despite being responsible for severe cutaneous adverse drug reactions (ADR). Objective A cross-sectional survey of allopurinol cases observed at the main Dermatology Department with inpatients facilities in southern Sardinia. (approx 560 836 inhabitants). Material and methods Data collection of all consecutive patients referred for ADR between 2001 and 2010. Causality assessment followed the WHO Collaborating Centre for Drug Monitoring criteria; illness severity score was adopted for toxic epidermal necrolysis (SCORTEN). Results Allopurinol was the culprit drug in 84 of 780 cutaneous ADR cases (10.7%; 8.4 cases/year). Mean age was 74 years, 58% of the patients were female, 95% of patients required hospitalization. Clinical forms were maculo-papular eruptions (34 cases), Stevens-Johnson Syndrome/Toxic Epidermal Necrolysis (31 cases), vasculitis (six cases), Drug Rash Eosinophilia and Systemic Symptoms (DRESS) (three cases), Acute Generalized Exanthematous Pustolosis (AGEP) (three cases), Pityriasis rosea-like eruption (three cases), lichenoid dermatitis (two cases), fixed drug eruption (one case), erythroderma (one case). The indication for allopurinol prescription was asymptomatic hyper-uricemia in 95% of the patients. Twelve patients were under allopurinol dosage adjustment according to creatinine clearance. Final causality assessment was definite for 12% of the cases and probable for the remaining 88%. Full recovery was achieved in 88% of subjects; ten SJS/TEN patients died (12% overall mortality; 32% mortality of the SJS/TEN cases). Conclusion Considering the populations size of Southern Sardinia, is plausible that 1.5/100 000 Sardinian will be affected by allopurinol related ADR per year. Advanced age, and inappropriate allopurinol prescription were the main conditions affecting morbidity and mortality.

Among associated comorbidities, metabolic syndrome plays an important role. The effects of biological therapies on metabolic syndrome is controversial. Thirty-one psoriatic patients with metabolic syndrome, eligible to treatment with anti-TNFα agents, were enrolled. Metabolic parameters were measured during 4 subsequent visits, one every 40 to 60 days. PASI, BSA and DLQI assessed the severity of psoriasis and the impact on quality of life. We include 31 patients, 18 treated with etanercept and 13 with adalimumab. Metabolic parameters evaluated at V4 in both groups showed different trends in the blood glucose values: a slight decrease in adalimumab group, an increase in etanercept group, with an almost significant comparison test (p=0.073). Delayed recognition and prolonged illness are major concerns. We report the case of a 65-year-old man who underwent treatment with corticosteroids for rheumatoid arthritis and then presented with multiple abscesses on the right leg. A complete assessment performed by the dermatologist led to the diagnosis of disseminated nocardiosis with involvement of the lungs and central nervous system. The patient&amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;#39;s history revealed development of pulmonary disease approximately 1 year prior to the appearance of the skin lesions, which was not recognized by several specialists. Long-term antibiotic therapy was effective in treating the skin and central nervous system lesions, while the chest radiologic profile improved more slowly.

Primary cutaneous nocardiosis is more common, with incidence rates ranging from 5% to 24% in various countries like India, UK, and Spain [10-12]. A major problem is correct identification, as clinical presentation is not peculiar as well as histological findings, suggestive of an infective process, but not able to detect the microorganisms, unless a tissue culture has been performed and Gram stained. The laboratory should be advised of the clinical suspicion, as Nocardia is a slow-growing bacterium, and cultures are often discarded when no growth is seen within 48 hours, or obscured by rapidly growing bacteria [1]. Once the Nocardiosis is presumed, speciation might take 1-3 weeks, and each strain has individual antibiotic susceptibility pattern, conditioning empiric treatment and possible fatal course. Newer molecular technologies promise to provide rapid and accurate identification [13], but first of all have revolutionized taxonomy, and we have to face with currently more than 30 species. Clinical relevance has been determined for about seventeen species [14,15] in respect to the 4 pathogens identified by traditional hydrolysis patterns: N. asteroides, N. brasiliensis, N. otitidiscaviarum, and N. transvalensis. Distinct serotypes and biotypes might share the same antibiotic susceptibility profile [8,13-18]. In western Europe and USA, the term "N.asteroides complex" is frequently used, failing to differentiate the strain cluster (N.asteroides sensu strictu) from N. cyriacigeorgica, N. farcinica, N. nova, N. abscessus, Nocardia brasiliensis, which is the second most common isolates in the majority of countries [14,16], differs from N.pseudobrasiliensis in mycolic acid pattern, adenine decomposition, nitrate reduction, and antimicrobial agent susceptibilities [18] For all these issues, clinical management and treatment of patients with nocardial disease remain a challenge. This review characterizes the cutaneous manifestations and related risk factors, to suggest clues in clinical diagnosis, raise physicians alert to avoid delayed recognition and address the correct assessments. Skin manifestations From a general health perspective, cutaneous nocardiosis (CN) traditionally occurs with 3 main patterns: (1) actinomycetoma, (2) lymphocutaneous (sporotrichoid) infection, (3) superficial skin infection (Table 1).
