Ministry of Health. Chile Introduction: Non-invasive home mechanical ventilation (NIHMV) is delivered in Chile since 2008 throughout a public program, including equipment and professionals. Objectives: 1) Characterize the socio-demographic and clinical profile of the adult patient under NIHMV and 2) Propose improvements in social health care. Methodology: Descriptive cross-sectional study, through face-toface home interview and review of official databases. Sample of 267 subjects, both gender, over 20 years-old, with Chronic Global Respiratory Failure (CGRF) in NIHMV. In 2016 there were 413 active patients. Results: Women 144 (53.9%), mean age 58.6 ± 18 years-old; 25.5% had COPD and 24% had a hypoventilation obesity syndrome, average PaCO 2 at the time of admission to program was 59 ± 11 mmHg, they were ventilated since 3.2 ± 2.4 years, 7.6 ± 2.4 h a day. Subjects "without instruction" and with "incomplete basic education" represents 40.7% of the sample. 46.4% were retired persons; 3% lived in a precarious hut; 19.8% cohabited with relatives or close friends; 49% did not have a partner; 4.8% lived alone; 68.6% was oxygen dependent; 40% had an altered Golberg test; 17.7% "has a major limitation, unable to perform self-care". Conclusions: Our patients have a severe socio-demographic and clinical deterioration, due to low schooling level, predominance of older adults, a major incapacity for working and patients have a more severe disease with higher baseline PaCO 2 levels, compared to European studies. NIHMV programs must adopt a social health approach and be inserted into the health network in social service, and cardiovascular and mental health programs.

y destacados expositores nacionales, que nos permitieron, revisar y analizar los modelos de atenc... more y destacados expositores nacionales, que nos permitieron, revisar y analizar los modelos de atención de países europeos y también de Chile, acerca de los cuidados domiciliarios, equipamiento actual, supervisión de pacientes que se encuentran recibiendo ventilación mecánica crónica, en forma no invasiva e invasiva y se desarrollaron talleres con los equipos más útiles en este tipo de soporte. Desde el Consenso Chileno de Ventilación no invasiva en el año 2008, varios grupos de trabajo en Alemania, Italia, Francia y otros países, han entregado consensos y guías clínicas recientes 1-4 sobre el manejo de pacientes con ventilación domiciliaria crónica, han publicado nuevos flujogramas para la evaluación precoz de los pacientes, se han establecido puntos de corte para varias pruebas funcionales y así definir el momento del inicio del soporte ventilatorio según el diagnóstico de base, y se han descrito un mínimo de métodos de vigilancia para objetivar resultados y realizar modificaciones en la programación de los equipos. Además se han publicado estudios sobre calidad de vida de los pacientes con este tipo de apoyo. Es necesario destacar cual es el nivel de evidencia para la indicación de soporte ventilatorio domiciliario en las diferentes enfermedades (Tabla 1).

Background: Long-term home non-invasive ventilation (LTH-NIV) has an impact on the health-related... more Background: Long-term home non-invasive ventilation (LTH-NIV) has an impact on the health-related quality of life of patients with chronic hypercapnic respiratory failure (CRF) of different causes. There are generic and specific questionnaires for respiratory diseases. In 2003 a specific questionnaire was developed for patients with CRF in LTH-NIV, called the Severe Respiratory Insufficiency (SRI) questionnaire, which has been shown to be reproducible and reliable and has been validated in several languages. The aim of the study was to translate and culturally adapt the SRI questionnaire for adult Chilean patients under LTH-NIV, and to assess its psychometric properties. Methods: The Chilean version of the SRI was obtained using the translation-back translation method, which was then applied by cross-sectional study to a non-probabilistic convenience sample of stable patients from five regions of Chile. The validated Chilean version of the SRI questionnaire and SF-36 (gold standard) questionnaire were applied, demographic and ventilatory data were collected. Reliability was analysed using Cronbach's alpha and intraclass correlation (test-retest). Construct validity was tested using exploratory factor analysis (principal component extraction and equimax orthogonal rotation) and hypothesis testing (Mann-Whitney test). Convergent criterion validity was tested using Spearman's rho. Results: The sample comprised 248 patients, 132 women (53.2%), median age (IQR) was 62 years (51-75), 146 patients (58.9%) were 60 years or older, 40% had a low education level. The mean ± SD completion time of the questionnaire was 18.8 ± 9.1 min, and 100% of the items were answered. The questionnaire was self-applied by 46.8% of the sample. The validated Chilean version of the SRI questionnaire showed very good overall reliability (0.95) and by scales (> 0.7). It showed a good correlation with the SF-36, with equivalent scales, a rotated matrix with 8 factors and hypotheses that explain the underlying constructs. Conclusions: The validated Chilean version of the SRI questionnaire has good psychometric properties. It is feasible, valid, and reliable for application to evaluate patients with CRF in LTH-NIV. It was found to be sensitive to assess the characteristics of the local population.

The Chilean Ministry of Health started in 2006 a program for HMV in Children and in 2008 it began a program for HMV in adults. All belonged to the state health insurance.Methods: Prospective cohort of adult patients with CRF in 10 regions of Chile admitted to the national HMV program, their demographic, clinical and functional characteristics, mode of admission, time in the program and survival.Results: A total of 1,105 patients were included. Median age was 59 years (44-58, IQR1-IQR3). Women were 58.1%. The body mass index was 34.9 (26-46) kg/m2, and 942 (85.4%) belonged to low-income socioeconomic groups. The Chilean Ministry of Health started a program for adults in 2008. Methods This study examined the following data from a prospective cohort of patients with CRF admitted to the national HMV program: characteristics, mode of admission, quality of life, time in the program and survival. Results A total of 1105 patients were included. The median age was 59 years (44–58). Women accounted for 58.1% of the sample. The average body mass index (BMI) was 34.9 (26–46) kg/m2. A total of 76.2% of patients started HMV in the stable chronic mode, while 23.8% initiated HMV in the acute mode. A total of 99 patients were transferred from the children's program. Tuberculosis is a pathology that has accompanied humanity since ancient times and regardless the ... more Tuberculosis is a pathology that has accompanied humanity since ancient times and regardless the existence of antibiotics for more than a century and that tuberculosis is a preventable and treatable disease, eradication is a goal not yet achievable. In Chile 40 to 70 cases are diagnosed every week which means physicians are constantly faced to this disease. The diagnosis is suspected based on typical respiratory symptoms and chest radiographs is the first imagenological approach. Several radiographical patterns, with interstitial and parenquimatous involvement are main features of this disorder. These manifestations are related to the patient's age, immunological status, previous exposure and disease stage. Different radiological patterns are shown from patients admitted in our hospital at the first semester of this year.

A frequency of malformations of 7.2 per born alive and of 22.1 per stillborn was detected in this population. To report the frequency of digestive system congenital malformations in this population. Analysis of data from the births that occurred at the University of Chile Hospital, that was gathered using codified form for the Latin American Collaborative Study for Congenital Malformations. Ninety seven digestive congenital malformations were detected, with a rate of 26.4 per 10,000 born alive and 12.2 per 10,000 stillborn. Diaphragmatic hernia was the most frequent malformation found, followed by imperforated anus, onphalocele and esophageal atresia. There was a secular increase in the frequency of these malformations. In the Clinical Hospital of the University of Chile all malformations in newborns are registered, as part of the Collaborative Latin American Study of Congenital Malformations (ECLAMC). The frequency of chromosomal aberrations, determined by cytogenetical studies, was determined in newborns with malformations. In the study period, there were 32,214 births. Of these, 2,268 live newborns and 43 stillbirths had malformations. Ninety nine children with malformations had chromosomal aberrations (4.3%).
