We aimed to evaluate the safety and efficacy of azithromycin in patients admitted to hospital with COVID-19. Methods In this randomised, controlled, open-label, adaptive platform trial (Randomised Evaluation of COVID-19 Therapy [RECOVERY]), several possible treatments were compared with usual care in patients admitted to hospital with COVID-19 in the UK. The trial is underway at 176 hospitals in the UK. Eligible and consenting patients were randomly allocated to either usual standard of care alone or usual standard of care plus azithromycin 500 mg once per day by mouth or intravenously for 10 days or until discharge (or allocation to one of the other RECOVERY treatment groups). Patients were assigned via web-based simple (unstratified) randomisation with allocation concealment and were twice as likely to be randomly assigned to usual care than to any of the active treatment groups. Participants and local study staff were not masked to the allocated treatment, but all others involved in the trial were masked to the outcome data during the trial. The primary outcome was 28-day all-cause mortality, assessed in the intention-to-treat population. The trial is registered with ISRCTN, 50189673, and ClinicalTrials.gov, NCT04381936. Findings Between April 7 and Nov 27, 2020, of 16 442 patients enrolled in the RECOVERY trial, 9433 (57%) were eligible and 7763 were included in the assessment of azithromycin. The mean age of these study participants was 65•3 years (SD 15•7) and approximately a third were women (2944 [38%] of 7763). 2582 patients were randomly allocated to receive azithromycin and 5181 patients were randomly allocated to usual care alone. Overall, 561 (22%) patients allocated to azithromycin and 1162 (22%) patients allocated to usual care died within 28 days (rate ratio 0•97, 95% CI 0•87-1•07; p=0•50). No significant difference was seen in duration of hospital stay (median 10 days [IQR 5 to >28] vs 11 days [5 to >28]) or the proportion of patients discharged from hospital alive within 28 days (rate ratio 1•04, 95% CI 0•98-1•10; p=0•19). Among those not on invasive mechanical ventilation at baseline, no significant difference was seen in the proportion meeting the composite endpoint of invasive mechanical ventilation or death (risk ratio 0•95, 95% CI 0•87-1•03; p=0•24). Interpretation In patients admitted to hospital with COVID-19, azithromycin did not improve survival or other prespecified clinical outcomes. Azithromycin use in patients admitted to hospital with COVID-19 should be restricted to patients in whom there is a clear antimicrobial indication.

Primary data has been collected through fieldwork in four communities in the Southern Region, consisting of individual interviews, focus group discussions, and key informant interviews. The study reveals that coastal fishing communities in Bangladesh face recurrent hazards that, coupled with other natural and anthropogenic drivers and pressures, have disastrous consequences on human well-being and the environment. Disaster impacts on coastal communities vary and differ by the capacities, situations, experiences, and capitals of households. In response to disasters’ adverse impacts, affected communities adopt various coping strategies, which produce both beneficial and harmful impacts. To include those impacts into long-term development goals of coastal communities, the study calls for the implementation of the Small-Scale Fisheries Guidelines (SSF Guidelines), which could help in the immediate relief phase, as well as the rehabilitation, reconstruction, and recovery after disasters and reduce vulnerabilities of small-scale fishers.

There exist an inverse relationship between cerebral blood flow and the viscosity of blood. The concept of autoregulatory mechanism for blood viscosity was suggested in the recent past. This autoregulatory mechanism which suffer a loss in ischemic stroke cases is quantitatively presented in the present investigation in terms of power law coefficients namely, nand k, through an empirical relation. Further it is shm-m that the blood viscosity autoregulation factor (A) in the follow-up stroke cases having no risk factor except age, is restored by seventh day to the range shovm for age matched healthy controls. The quantification of already existing concept of blood viscosity autoregulation especially with reference to follow-up stroke cases may help the clinicians understand the relationship between the severity of disease and the extent of alteration in the resistance to the flow of blood. This method of analysis may reveal the action of drugs destroying or restoring the autoregulation in different diseases.

The effect of adding a nonionic polymer -polyvinylpyrrolidone (PVP) with an anionic surfactant has been studied at two different temperatures; the rate of reduction increased as the reaction temperature was increased from 60 to 100°C. These nano-aggregated nickel particles were characterized by using SEM with EDX facilities and TEM. TEM characterization showed the presence of spherical Ni particles as fine as 10nm in diameter. However, the SEM images showed a very spiky morphology, very small spherical shaped objects were clearly observed within these spiky structures. The combination of SDS/PVP reaction produced nano-sized nickel particles which were much finer than the reactions where SDS was used on its own. PVP has shown some dispersion power, and was found to be capable of preventing nickel par...

Information on cancer patterns are essential for effective planning of cancer control interventions. There is limited published information available on pattern of cancer for the state of Odisha, India. The present study was an attempt to explore the pattern and trend of cancer in Odisha. To fulfill the objectives retrospective data available from 2001-2011 at Acharya Harihar Regional Cancer Center (AHRCC), Cuttack, Odisha, were analyzed. Medical records of cancer patients were reviewed and relevant information on diagnosis, primary site and demographic data were retrieved. Data were entered and analyzed using SPSS 16.0 (SPSS Inc.). A total of 74,861 cancer inpatients were registered at AHRCC for the years 2001-2011. The proportion of females outnumbered males with female:male ratio 1.1:1. The number of female cases increased four folds and that of males three fold over the period studied. Malignancies such as oral cancer (16.93%), acute lymphocytic leukemia/non Hodgkins lymphoma (14.09%) and cancer of gastrointestinal tract (21.07%) are leading cancers among males and carcinomas of breast (28.94%), cervix (23.66%) and ovary (16.11%) were leading among females. Findings from this study indicate an overall increase in cancer reporting which could be regarded as proxy measure for overall cancer situation in Odisha. There is scope and need for integrating other government hospitals, existing private health service providers and research institutions across the state for better planning of cancer control program.

In recent years the use of these inhibitors in producing high performance concrete has increased significantly as a result of aggressive environment. This report is based on the corrosion inhibitors used in concrete and also on the published literatures. Most authors agreed that reinforcement corrosion is the most important causes of premature failure of reinforced concrete structure worldwide and generate a great research concern due to its effects on global economy. The report presents information related to basic mechanism of corrosion protection methods to extend the service life of reinforced concrete structures which include inhibitors, sealers and barriers. Environmental sustainability issues concerning the present inhibitors being used were examined, viability and versatility of green inhibitors to concrete was also x-rayed.
