Many bariatric procedures are more effective for improving type-2 diabetes mellitus (T2DM) than c... more Many bariatric procedures are more effective for improving type-2 diabetes mellitus (T2DM) than conventional pharmacotherapy. The current research evaluated factors linked to complete and partial remission or improvement of T2DM after laparoscopic sleeve gastrectomy (LSG). The current prospective study included all diabetic patients who were submitted LSG between January 2015 and June 2018 and completed a 2-year follow-up period. Patients were assessed at baseline and 2 years after LSG. This work comprised of 226 diabetic cases. Two years after LSG, 86 patients (38.1%) achieved complete remission of DM, and 24 (10.6%) reached partial remission. Only 14 patients (6.2%) showed no change in their diabetic status. On univariate analysis, age ≤ 45 years, duration of diabetes ≤ 5 years, use of a single oral antidiabetic, HbA1c ≤ 6.5%, HOMA-IR ≤ 4.6, C-peptide > 2.72 ng/mL, and BMI ≤ 40 kg/m2 predicted complete remission. Aim of the work: SLE is an autoimmune disease characterized by a variety of clinical and laborato... more Aim of the work: SLE is an autoimmune disease characterized by a variety of clinical and laboratory abnormalities. It may affect many organs and pulmonary involvement is a common finding in SLE. The purpose of this study was to disclose the pulmonary involvement in early SLE patients not more than 2 years of disease duration using the computed tomography (CT) as well as the pulmonary function tests as ways of pulmonary involvement assessment. Patients and methods: Forty-two patients aged 29 ± 12.5 with early SLE not more than 2 years of disease duration were recruited for the study. All patients were assessed clinically for their SLE with BILAG which was utilized for disease activity determination. Results: Nine and half percent of our patients were found to be clinically involved by ILD, where 28.6% have abnormal HRCT finding, 26.2% with abnormal PFT. Variants that were associated with an abnormal forced vital capacity FVC < 80% in a significant manner were: smoking, long disease duration, self-reported pulmonary symptoms (p 0.001), BILAG global score (p 0.006), Anti dsDNA (p 0.001), Antiphospholipid (IgM or IgG) (p 0.01), anti Sm (p 0.002), anti-RNP (p 0.005), HRCT abnormalities (p 0.001), current medication of steroid (any dose) (p 0.005), immunomodulator therapy (p 0.002), and Rituximab therapy (p 0.001). Conclusions: ILD occurs as early as in the first 2 years in the course of SLE patients. There was a clear predilection of ILD with certain variables in our cohort of patients.

Background and aim: Transurethral resection of the prostate (TURP) is the standard procedure for ... more Background and aim: Transurethral resection of the prostate (TURP) is the standard procedure for treatment of benign prostatic hyperplasia which is the most common non-malignant disorder of the prostate, affecting over 50% of the elderly male population. This randomized study was done to compare the effect of bipolar cautery using saline with monopolar cautery using glycine regarding hemodynamic and biochemical changes in TURP. Methods: After approval from the ethical committee in Kasr Al Ainy university hospital, fifty consenting ASA II and III male patients scheduled for TURP were randomly allocated into two equal groups. Monopolar group had TURP performed with monopolar cautery using 1.5% glycine and bipolar group had it done using bipolar cautery with 0.9% saline. Spinal anesthesia was used. Hemodynamic data (mean arterial blood pressure (MAP), heart rate (HR), central venous pressure (CVP), and oxygen saturation (SPO 2)) were recorded preoperatively and postoperatively. Hemoglobin (Hb) and biochemical laboratory findings (Na + , ABG) were recorded pre-and postoperatively. Volume of irrigant was recorded by the end of surgery. Results: No significant difference in patients' age or size of the prostate was present. Duration of surgery in bipolar group was longer (p-value 0.003). Preoperatively there was no significant difference in the hemodynamic variables, SPO 2 , CVP, biochemical markers and hemoglobin. Securing e-commerce sites has become a necessity as they process critical and sensitive data to c... more Securing e-commerce sites has become a necessity as they process critical and sensitive data to customers and organizations. When a customer navigates through an e-commerce site his/her clicks are recorded in web log file. Analyzing these log files using data mining reveal many interesting patterns. These results are used in many different applications and recently in detecting attacks on web. In order to improve quality of data and consequently the mining results data in log files need first to be preprocessed. In this paper, we will discuss how different web log files with different formats will be combined together in one unified format using XML in order to track and extract more attacks. And because log files usually contain noisy and ambiguous data this paper will show how data will be preprocessed before applying mining process in order to detect attacks. We will also discuss the difference between log preprocessing for web intrusion and for web usage mining.

However, most of them deal with isolated aspects of the DW and do not provide designers with an integrated and standard method for designing the whole DW life cycle (ETL processes, data sources, DW repository and so on).Some approaches are depending on specific platform or neglecting important issues in DW design life cycle. Extraction-transformation-loading (ETL) processes play an important role in data warehouse architecture because they are responsible of integrating data from heterogeneous data sources into the DW repository. It is vitally important to consider security requirements from the earliest stages of the development process. This paper proposes a conceptual model to refresh data warehouse by (insert, update, delete) data using ETL processes and considering DW security requirements. Firstly, the proposed ETL model is based on Unified Modeling Language (UML), which allows us to accomplish the conceptual modeling of ETL processes .secondly; this part focuses on how to integrate the proposed ETL model with the DW model. The proposed DW model depends on the Model Driven Architecture (MDA). MDA is a standard framework for software development that addresses the complete life cycle of designing, deploying, integrating, and managing applications by using models in software development. This paper proposes an integrated conceptual model for addressing temporal data warehouse security requirements (CMTDWS). One key advantage of our model is that we accomplish the conceptual modeling of secure temporal DWs independently of the target platform where the DW has to be implemented, allowing the implementation of the corresponding TDWs on any secure commercial database management system.

Background: Lupus nephritis is one of the most serious manifestations of systemic lupus erythemat... more Background: Lupus nephritis is one of the most serious manifestations of systemic lupus erythematosus (SLE). Novel biomarkers are necessary to enhance the diagnostic accuracy, prognostic stratification, monitoring of treatment response, and detection of early renal flares. Methods: Our study was conducted on 90 participants. They were divided into three groups, group I (controls) encompassed 30 ages and sexmatched healthy personnel. Group II included 30 non-nephritic SLE patients and finally group III included 30 SLE nephritic patients. Urinary monocyte chemoattractant protein-1 (UMCP-1) and hepcidin were evaluated by ELISA technique, compared and correlated in different groups, with each other and with other routine variables and with renal biopsy done to study group (III). Results: Both UMCP-1 and hepcidin in group III showed significant increase compared to other two groups (controls and group II) (468 ± 128, 111 ± 12, 252 ± 56 pg/ml, respectively, for UMCP-1 and 40 ± 12, 11 ± 2, 20 ± 5 ng/ml, respectively, for hepcidin, P < 0.01). Also both UMCP-1 and hepcidin in group III showed significant increase in diffuse proliferative subgroup compared to focal proliferative and mesangioproliferative subgroups (580 ± 43, 502 ± 46, and 352.6 ± 100 pg/ml, respectively, for UMCP-1 and 47.8 ± 9.5, 41.4 ± 6, and 32.9 ± 10.8 ng/ml, respectively, for urinary hepcidin, P < 0.05). Conclusion: UMCP-1 and hepcidin could be associated with the susceptibility of lupus nephritis.

Interferon alpha is considered the backbone for any effective treatment. These therapies are not without adverse effects nor are they always cost effective. Accordingly, it is important to identify patients with a considerable chance of response before initiation of therapy. Rapid virological response (RVR) is important for identification of non-responders permitting therapy discontinuation and avoiding adverse effects and costs. Evaluate the reliability of homeostasis model assessment (HOMA) method in prediction of HCV treatment response among Egyptian patients with chronic HCV genotype 4 infections. Our study included 80 participants; 50 non-cirrhotic non-diabetic patients with chronic HCV genotype 4 (group I), and 30 age-and sexmatched patients negative for HCV RNA and consequently they did not receive hepatitis treatment at the time of sampling (group II) were included. According to HOMA score, group I was further divided into group Ia with insulin resistance (IR) >2.5 (36 %) and group Ib with IR <2.5 (64 %). HOMA-IR was significantly associated with high RVR, progressive fibrosis stages, high pretreatment ALT levels, and body mass index. Our data suggests that IR strongly affects VR. HOMA-IR would appear to be useful in predicting RVR and should be evaluated at baseline in all chronic HCV patients before initiating interferon therapy to reduce unnecessary and ineffective treatments and enhance cost effectiveness of treatment.

The purpose of this study was to disclose the pulmonary involvement in early RA patients not more than 2 years disease duration using the computed tomography (CT) as well as the pulmonary function tests as ways of pulmonary involvement assessment. Forty patients aged 37.6 ± 10.3 with early rheumatoid arthritis not more than 2 years of disease duration were recruited for the study. All patients were assessed clinically for their RA with DAS28, which was utilized for disease activity determination. Ten percent of our patients were found to be clinically involved by interstitial lung disease (ILD), where 27% have abnormal HRCT finding and 32.5% with abnormal PFT. Predilection for clinically manifest ILD was evident in active RA patients with high DAS28 score, seropositive RA patients, and in patients receiving steroids and anti-TNFα therapy. ILD occurs early in the course of RA, with more predilection for clinically active RA disease.
