This section will be directed to highlight the passing of erotic delusion from a concept linked to a literary topos to a recognized disease thanks to the work carried on by Arabic medicians (Razhes, Avicenna) and it's integration into western medicine followed to the works of Arnaldus da Villanova, Jacques Ferrand and Robert Burton, till modern perspectives about erotomania due to Boissier de Sauvage, Esquirol, Ball and then to De Clérambault works, taking into account the psychoanalytic, both Freudian and Lacanian, thought. In the second part of the work a single case study will be presented. A Caucasian 41 years women presenting erotic delusion symptoms toward a psychiatrist who treated her will be presented. The case will be described concerning the dynamics of its recent evolving, the absence of awareness about the disease, the low compliance towards treatments, the unsatisfactory reactions to neuroleptics (haloperidol, zuclopenthixol) and to new generation antipsychotic (risperidone) administred as longacting. Finally, an exhaustive review referring of contemporary Italian, French, Spanish, Portuguese Deutch and English literature. Show that erotomanic syndrome is not culturally dependent being described also in Africans, Arabics and Asian countries. In the conclusion authors wish for a growing of interest toward erotic syndrome or erotic traits in several psychiatric syndromes, which often lead to pharmacologic resistance increasing the poor therapeutic outcome. In appendix a semi-structured interview able to facilitate the spotting of erotic traits will be presented.
