Fatty liver disease (FLD) is the most prevalent form of liver disease worldwide. Overnutrition ca... more Fatty liver disease (FLD) is the most prevalent form of liver disease worldwide. Overnutrition can induce nonalcoholic fatty liver disease (NAFLD), a spectrum of conditions ranging from simple steatosis [or nonalcoholic fatty liver (NAFL)] to nonalcoholic steatohepatitis and cirrhosis. Some of the epidemiological and pathological studies have also suggested an association between the presence of fatty liver and sudden death. A 37-year-old man was found dead when he was asleep in the bed at home. According to his family, he was single and a costermonger. He was not an athlete, and there was no history of any physical and mental disorder. He was not addicted and did not use any drugs or alcohol. The positive points, in this case, were: a large heart with mild coronary stenosis and steatohepatitis in autopsy and sudden death. Since the beginning of the COVID-19 pandemic, efforts have been made to design safe and effective... more Since the beginning of the COVID-19 pandemic, efforts have been made to design safe and effective vaccines against SARS-CoV-2. Numerous vaccines have been designed and tested in limited clinical trials in various countries. Among them, the Sputnik V vaccine has shown a relatively safe profile and, to our knowledge, has no associated major side effects. We describe the case of a 40-year-old female healthcare worker who developed severe persistent eczematous lesions on the second day after she received the first dose of the Sputnik vaccine. The eczematous lesions were refractory to an antihistamine and persisted at the 1 month follow-up. Severe persistent eczematous lesions should be viewed as a potential side effect of vaccination with the Sputnik V vaccine. Moreover, a severe allergic reaction to a COVID-2019 vaccine may indicate the vaccine is ineffective in the recipient.

Beside promising outcomes, some authors are concerned about high rates of complications. In the current clinical trial, complications of EOS surgery using dual growing rods were investigated. METHODS Between June 2006 and February 2017, twenty-two consecutive patients with a coronal curve >45 degrees underwent serial surgical correction using dual growing rods at Shafa Orthopedic hospital, Tehran, I.R. Iran. The rods were secured using hooks or screws or both. The patients were followed for 5.2 ± 1.7 years on average. Wilcoxon test was utilized to compare the pre- and post-operative values. Nonetheless, the aim of this study was to investigate the simultaneous effect of exercise with mental stress on changes in electrical waves of the heart of young men. Methods: 24 healthy inactive men were selected and randomly divided into two groups of exercise and exercise with mental stress. The subjects in the exercise group biked for 37 minutes (60 % of the maximum aerobic capacity), and the subjects in the exercise with mental stress, biked for 37 minutes while they received 20 minutes of mental stress during biking, from minute 10 to 32. Background: The critical care unit is inherently stressful due to its complexity, leading to fati... more Background: The critical care unit is inherently stressful due to its complexity, leading to fatigue and consequences such as restless legs syndrome (RLS). This study aimed to investigate the prevalence of RLS and its relationship with fatigue in critical care nurses. Patients and methods: This cross-sectional study was performed with 200 nurses of critical care units of Shahroud hospitals in 2019 using the census sampling method. Data collection tools included the RLS questionnaire and the Multidimensional Fatigue Inventory. Data analysis was performed using descriptive and inferential statistics (multiple linear regression analysis). Results: The results showed that 72% of the subjects had moderate to high RLS, and their mean fatigue was 55.31 ± 10.53. By increasing one point in the score of RLS and 1 year of critical care nursing experience, the fatigue score increases by 0.3 and 0.71. Conclusions: Considering the relationship between fatigue and RLS in critical care nurses, supportive interventions seem necessary to reduce the effects of these two components.

Immobilized microalgae are a promising approach to incorporate microalgae in recirculating aquacu... more Immobilized microalgae are a promising approach to incorporate microalgae in recirculating aquaculture system (RAS) for water purification. In the present study, two types of biosorbents including sodium alginate–immobilized Scenedesmus spp. and Chlorella spp. beads (algal beads) and sodium alginate beads without microalgae (alginate beads) were prepared. In the first experiment (static test), the potential of two biosorbents to remove different concentrations of total ammonia nitrogen (TAN) and total phosphorus (TP) from water was investigated. In the second experiment, two prepared biosorbents were used as biofilter in a RAS for rearing African cichlid (Labidochromis lividus) for 30 days. The survival rate and growth indices of fingerling fish and removal efficiency of two biosorbents for TAN, NO3—N, and TP were determined. The results of static test showed that the removal efficiency and uptake capacity of the two biosorbents for TAN and TP increased during 30 days of the experiment, and these values for the algal beads were higher than the alginate beads. The TAN removal efficiency of the two biosorbents increased with increasing TAN concentration from 0.5 to 5 mg L−1. The application of algal beads in the RAS improved the survival rate, final weight, final length, weight gain, and daily growth index (DGI%) indices of fish compared to those cultured in the RAS containing the alginate beads and the control (P<0.05). The algal and alginate beads decreased the TAN concentration by 42.85% and 28.57% compared to the control after 30 days of cultivation period, respectively. The uptake of nitrate was not observed by the two biosorbents during cultivation period. The TP removal efficiency of algal beads reached 44.90% after 30 days. The findings of this study indicated that the sodium alginate–immobilized microalgae could be considered as a suitable biofilter to be incorporated into a RAS to improve water quality and consequently enhance the growth and health of fish.

Применение микроводорослей в качестве абсорбентов способствует достижению двойной цели-очистке сточных вод и производству биомассы, и является реальным способом уменьшения экологических проблем. Для иммобилизации клеток использовали несколько вариантов альгинатной матрицы со сшивающими агентами на основе хлорида бария или кальция с добавлением других полимеров (хитозан, метилцеллюлоза, ПВП) и CaCO 3. По данным спектроскопии с индуктивно-связанной плазмой и измерения химической потребности в кислороде, клетки водорослей в обеих формах способны поглощать органические вещества и ионы металлов. Recently, there have been reports of an outbreak of lead toxicity due to opium contamination with lead in Iran. Gastroenterology symptoms were the most presentation of lead toxicity in opium users. The following case report argument about GI presentation of lead toxicity. We here report a case of a 36-year-old woman presenting with an acute, severe abdominal pain. With reviewing of abdominal CT scans, a suspicious diagnosis of lead poisoning was made by detecting of multiple radiopaque lesions in the intestine. She was an oral opium user for several years. Lead poisoning was confirmed by toxic Blood lead level (BLL), and chelation therapy started under the toxicology expert's supervision. Early diagnosis of lead poisoning by assessing the BLL can prevent unnecessary investigations and interventions and permits early commencement of the treatment.

Background: Toxic neuropathy is a global health problem affecting many people over the world, ann... more Background: Toxic neuropathy is a global health problem affecting many people over the world, annually. Lead Poisoning (LP) represents neurological complaints and neurobehavioral disorders. Therefore, its significances, especially its neurological consequences, can be misdiagnosed as other neuropathies like syndromes. Case Presentation: In this case report, we aim at describing a 45-year-old woman with Guillain-Barre-like Syndrome (GBS), who was admitted to the hospital with peripheral neuropathy and complaints of paresthesia in the lower and upper limbs. Suffering from GBSlike syndrome, she incidentally showed opium consumption during the hospitalization and taking her history. Serum lead levels were precisely elevated (88.6µg/dL). To assess the neurologic effects, the brain CT, MRI, EMG, and NCV were performed, indicating severe sensory-motor demyelinating polyradiculopathy. The patient was admitted to the hospital again after 2 years with severe radicular pain in the lower and upper limbs and with positive myoclonus and tremor. While treated with plasma exchange and pregabalin at the first stage of admission, signs were normal and again recurred after 2 years, suggesting the reversibility of the histological findings and misdiagnosis. At the second stage, Intravenous Immunoglobulin (IVIG) was prescribed. The patient recovered and was discharged with chelation therapy of CaNa2EDTA for LP. The most frequent neurological complication induced by LP is severe sensory-motor demyelinating polyradiculopathy and axonal polyneuropathy. But, the clinical examination and the electrophysiological findings may also suggest a GBS-like syndrome. Conclusion: Any discrepancies in this regard should be reconsidered to confirm LP diagnosis.

Background: Physiological stress due to physical activity associated with mental stress can affec... more Background: Physiological stress due to physical activity associated with mental stress can affect the responses of the sympathoadrenal axis and the hypothalamic-pituitary axis. Objectives: The present study aimed to investigate the effect of exercise with mental stress on cortisol and alpha-amylase changes in young men. Methods: In a quasi-experimental study, 20 inactive healthy men were randomly selected and divided into two groups of exercise and exercise with mental stress. The exercise group pedaled on a fixed bike at an intensity of 60% of maximal aerobic power for 37 minutes and the exercise group with mental stress received 20 minutes of mental stress (from 12th minutes to 32nd minutes) during the 37 minutes of pedaling. Salivary samples were collected before and immediately after the end of the exercise. Results: The exercise had a significant effect on increasing salivary cortisol (P = 0.007) and alpha-amylase (P = 0.01). Exercise with mental stress had a significant effect on salivary cortisol (P = 0.004) and alpha-amylase (P = 0.04). The exercise and exercise with mental stress had the same effects on increasing salivary cortisol (P = 0.42) and alpha-amylase (P = 0.83). Conclusions: It seems that exercise and exercise with mental stress have the same effects on increasing salivary cortisol and alphaamylase levels.

In this cross-sectional study, the file of suicide cases referred to the Central Legal Medicine Organization was investigated retrospectively. Of 1681 suicide cases, the highest suicide mode was hanging by 993 cases (57.4%) and intoxication by 674 cases (39%). The mean age of hanging was 36.22±15.76 and for other suicide methods it was 32.61±13.70 (P=0.000). Hanging was higher in males. Female victims used other methods like intoxication (P=0.000). The most common seasons for hanging were spring and winter. Toxicological analysis in cases of hanging was positive with opioid alkaloids in 95 cases (9/9%) and methamphetamine in 68 cases (7%). In addition to abdominal fat thickness, the question arises whether other visceral fat deposits, ... more In addition to abdominal fat thickness, the question arises whether other visceral fat deposits, including epicardial and mesenteric fat, even in men with a normal body mass index (BMI < 30 and acceptable waist/hip ratio [WHR]) can lead to atherosclerosis and cardiac hypertrophy. The present study was conducted to compare the distribution of visceral fat between men under 40 who experienced sudden death due to coronary artery obstruction and a control group who have passed from traffic accidents in Tehran. Autopsy samples were collected from a total of 94 men and divided into 2 groups of 47 men. The case group (sudden death from coronary obstruction) and the control group (death due to traffic accidents) were matched in terms of age, height and weight. BMI and waist/hip ratio were within the acceptable range in both groups. Neither of the groups had cardiovascular or other diseases. The findings of the study showed a greater thickness of subcutaneous abdominal fat around the navel and a higher epicardial fat, mesenteric weight, heart mass and heart mass to height ratio in the group of men who died due to coronary artery obstruction. Accordingly, even in seemingly healthy men under 40 with no obese men (with normal BMI and acceptable WHR), the accumulation of internal visceral fat such as subcutaneous abdominal fat around the navel and epicardial and mesenteric fat can contribute to coronary artery obstruction and cardiac hypertrophy.

In the present study, two types of biosorbents including sodium alginate–immobilized Scenedesmus spp. and Chlorella spp. beads (algal beads) and sodium alginate beads without microalgae (alginate beads) were prepared. In the first experiment (static test), the potential of two biosorbents to remove different concentrations of total ammonia nitrogen (TAN) and total phosphorus (TP) from water was investigated. In the second experiment, two prepared biosorbents were used as biofilter in a RAS for rearing African cichlid (Labidochromis lividus) for 30 days. The survival rate and growth indices of fingerling fish and removal efficiency of two biosorbents for TAN, NO3—N, and TP were determined. The results of static test showed that the removal efficiency and uptake capacity of the two biosorbents for TAN and TP increased during 30 days of the experiment, and these values for the algal beads were higher than the alginate beads. The TAN removal efficiency of the two biosorbents increased with increasing TAN concentration from 0.5 to 5 mg L−1. The application of algal beads in the RAS improved the survival rate, final weight, final length, weight gain, and daily growth index (DGI%) indices of fish compared to those cultured in the RAS containing the alginate beads and the control (P<0.05). The algal and alginate beads decreased the TAN concentration by 42.85% and 28.57% compared to the control after 30 days of cultivation period, respectively. The uptake of nitrate was not observed by the two biosorbents during cultivation period. The TP removal efficiency of algal beads reached 44.90% after 30 days. The findings of this study indicated that the sodium alginate–immobilized microalgae could be considered as a suitable biofilter to be incorporated into a RAS to improve water quality and consequently enhance the growth and health of fish.

Применение микроводорослей в качестве абсорбентов способствует достижению двойной цели-очистке сточных вод и производству биомассы, и является реальным способом уменьшения экологических проблем. Для иммобилизации клеток использовали несколько вариантов альгинатной матрицы со сшивающими агентами на основе хлорида бария или кальция с добавлением других полимеров (хитозан, метилцеллюлоза, ПВП) и CaCO 3. По данным спектроскопии с индуктивно-связанной плазмой и измерения химической потребности в кислороде, клетки водорослей в обеих формах способны поглощать органические вещества и ионы металлов. Recently, there have been reports of an outbreak of lead toxicity due to opium contamination with lead in Iran. Gastroenterology symptoms were the most presentation of lead toxicity in opium users. The following case report argument about GI presentation of lead toxicity. We here report a case of a 36-year-old woman presenting with an acute, severe abdominal pain. With reviewing of abdominal CT scans, a suspicious diagnosis of lead poisoning was made by detecting of multiple radiopaque lesions in the intestine. She was an oral opium user for several years. Lead poisoning was confirmed by toxic Blood lead level (BLL), and chelation therapy started under the toxicology expert's supervision. Early diagnosis of lead poisoning by assessing the BLL can prevent unnecessary investigations and interventions and permits early commencement of the treatment.

Lead Poisoning (LP) represents neurological complaints and neurobehavioral disorders. Therefore, its significances, especially its neurological consequences, can be misdiagnosed as other neuropathies like syndromes. Case Presentation: In this case report, we aim at describing a 45-year-old woman with Guillain-Barre-like Syndrome (GBS), who was admitted to the hospital with peripheral neuropathy and complaints of paresthesia in the lower and upper limbs. Suffering from GBSlike syndrome, she incidentally showed opium consumption during the hospitalization and taking her history. Serum lead levels were precisely elevated (88.6µg/dL). To assess the neurologic effects, the brain CT, MRI, EMG, and NCV were performed, indicating severe sensory-motor demyelinating polyradiculopathy. The patient was admitted to the hospital again after 2 years with severe radicular pain in the lower and upper limbs and with positive myoclonus and tremor. While treated with plasma exchange and pregabalin at the first stage of admission, signs were normal and again recurred after 2 years, suggesting the reversibility of the histological findings and misdiagnosis. At the second stage, Intravenous Immunoglobulin (IVIG) was prescribed. The patient recovered and was discharged with chelation therapy of CaNa2EDTA for LP. The most frequent neurological complication induced by LP is severe sensory-motor demyelinating polyradiculopathy and axonal polyneuropathy. But, the clinical examination and the electrophysiological findings may also suggest a GBS-like syndrome. Conclusion: Any discrepancies in this regard should be reconsidered to confirm LP diagnosis.

Background: Physiological stress due to physical activity associated with mental stress can affec... more Background: Physiological stress due to physical activity associated with mental stress can affect the responses of the sympathoadrenal axis and the hypothalamic-pituitary axis. Objectives: The present study aimed to investigate the effect of exercise with mental stress on cortisol and alpha-amylase changes in young men. Methods: In a quasi-experimental study, 20 inactive healthy men were randomly selected and divided into two groups of exercise and exercise with mental stress. The exercise group pedaled on a fixed bike at an intensity of 60% of maximal aerobic power for 37 minutes and the exercise group with mental stress received 20 minutes of mental stress (from 12th minutes to 32nd minutes) during the 37 minutes of pedaling. Salivary samples were collected before and immediately after the end of the exercise. Results: The exercise had a significant effect on increasing salivary cortisol (P = 0.007) and alpha-amylase (P = 0.01). Exercise with mental stress had a significant effect on salivary cortisol (P = 0.004) and alpha-amylase (P = 0.04). The exercise and exercise with mental stress had the same effects on increasing salivary cortisol (P = 0.42) and alpha-amylase (P = 0.83). Conclusions: It seems that exercise and exercise with mental stress have the same effects on increasing salivary cortisol and alphaamylase levels.

In this cross-sectional study, the file of suicide cases referred to the Central Legal Medicine Organization was investigated retrospectively. Of 1681 suicide cases, the highest suicide mode was hanging by 993 cases (57.4%) and intoxication by 674 cases (39%). The mean age of hanging was 36.22±15.76 and for other suicide methods it was 32.61±13.70 (P=0.000). Hanging was higher in males. Female victims used other methods like intoxication (P=0.000). The most common seasons for hanging were spring and winter. Toxicological analysis in cases of hanging was positive with opioid alkaloids in 95 cases (9/9%) and methamphetamine in 68 cases (7%). In addition to abdominal fat thickness, the question arises whether other visceral fat deposits, ... more In addition to abdominal fat thickness, the question arises whether other visceral fat deposits, including epicardial and mesenteric fat, even in men with a normal body mass index (BMI < 30 and acceptable waist/hip ratio [WHR]) can lead to atherosclerosis and cardiac hypertrophy. The present study was conducted to compare the distribution of visceral fat between men under 40 who experienced sudden death due to coronary artery obstruction and a control group who have passed from traffic accidents in Tehran. Autopsy samples were collected from a total of 94 men and divided into 2 groups of 47 men. The case group (sudden death from coronary obstruction) and the control group (death due to traffic accidents) were matched in terms of age, height and weight. BMI and waist/hip ratio were within the acceptable range in both groups. Neither of the groups had cardiovascular or other diseases. The findings of the study showed a greater thickness of subcutaneous abdominal fat around the navel and a higher epicardial fat, mesenteric weight, heart mass and heart mass to height ratio in the group of men who died due to coronary artery obstruction. Accordingly, even in seemingly healthy men under 40 with no obese men (with normal BMI and acceptable WHR), the accumulation of internal visceral fat such as subcutaneous abdominal fat around the navel and epicardial and mesenteric fat can contribute to coronary artery obstruction and cardiac hypertrophy.
