This study aimed to identify and categorise the stressors experienced and reconstitution strategies adopted by older adults, their informal caregivers, and healthcare professionals as they manage older adults’ medications after hospital discharge. A primary study collected the perspectives of 28 older adults, 17 informal caregivers, and 13 healthcare professionals using a qualitative descriptive design. The Neuman Systems Model was used as the basis for a secondary deductive content analysis. Findings revealed that post-discharge medication management at home involved numerous stressors, often including dysfunctions in communication, collaboration, and coordination between the multiple stakeholders involved. Reconstitution strategies for safe medication management were not always successful or satisfactory and were sometimes identified as stressors themselv...

When combined with frailty, cognitive impairment and changing pharmacological prescriptions, older adults’ polymedication regimes increase the risk of medication-related problems (MRPs) and hospitalisation. Effective, well-organised medication management could avoid MRPs and their clinical outcomes. OBJECTIVE Identify medication management models and analyse their impact on managing and preventing MRPs for polymedicated home-dwelling older adults. METHODS We will conduct a systematic review of published articles in relevant professional scientific journals, and in the following electronic databases, from inception until 31 March 2019: Embase.com, Medline Ovid SP, PubMed (NOT Medline[sb]), CINAHL EBSCO, PsycInfo Ovid SP, Cochrane Library Wiley and Web of Science.. We will also hand search the bibliographies of all the relevant articles found and search for unpublished studies. We will ...

Method: This is a phenomenological study, according to Giorgi’s method, which uses semi-structured interviews. Eighteen specialist nurses participated, recruited using convenience sampling, in two hospitals in the northern region of Portugal, between March and December 2018. Results: Five themes emerged: use of instruments; use of theoretical framework; nursing diagnoses; prescription of interventions; nursing records. Conclusion: Specialist nurses use instruments that essentially promote the elderly’s physical capacity, within the scope of autonomy. The information systems used to record the diagnoses and prescriptions for interventions do not reflect the intervention for the person’s autonomy. Autonomy in a multidimensional way is not very well developed, despite the recognition of its importance for the elderly’s quality of life and dignity.

The aim of this study is to assess the impact of a game in teaching families nursing practices. It is a quasiexperimental study with a time setting, integrating nurses in different contexts within the hospital, which it was possible to collect information before and after the intervention. The intervention was the "Family Nursing Game"®, it is a board game. To assess the impact of the training strategy, information was analyzed and documented by nurses before and after the intervention, based on the evaluation and intervention model of Calgary. The results show an increased focus on the family in the various fields of the evaluation and intervention model of Calgary, mainly due to intervention in the affective domain. We can conclude that the game is a useful.

Most studies define that it is important to involve the family (caregivers) after the diagnosis of heart disease to guarantee the treatment's success, and CR is no different. Not being an implicit objective of secondary prevention of CR, some of the interventions included in CR can positively impact caregivers. Therefore, it is essential to understand which interventions can be used to promote the role of caregiver and their health. Objective: Map which interventions have been implemented that support caregivers of patients with the cardiac disease taking part in a cardiac rehabilitation program, in their role and health. Methods: Scoping Review guided by the methodology proposed by the Joanna Briggs Institute for conducting Scoping Reviews...

However, significant variability currently exists in scoping review conduct and reporting, thus limiting the potential of the methodology to advance research and practice about skin self-care of people with spinal cord injury (SCI). Our objective was to perform a scoping review protocol within the health rehabilitation context of people with SCI, focusing on skin self-care.Methods and analysisThe protocol was developed by using the scoping review methodological framework proposed by Arksey and O’Malley and further refined by the Joanna Briggs Institute, incorporating insights from more recent innovations in scoping review methodology. Sensitive searches of 13 electronic databases from 2007 to 2017 will be supplemented by grey literature searches. Tw...

The family is asked to look after these people, without being prepared for that. The family doesn't understand their ill relatives neither the revelation of the illness, dealing as well with guilt, physical and emotional overload, social isolation and frequently economical difficulties. This study allows us to identify the health needs in the families with relatives suffering from mental illness, and develop healthy strategies to overcome the difficulties caused by the illness. Our aim is to promote health in these families. For this investigation we conducted semi-structured interviews within a sample composed by relatives of mental patients. The families feel insecure and don't know what to do, saying it would be important to know how to deal with the ill person and where to turn to in case of family instability. The information allows us to help the family to readjust their new life projects.
