ABSTRACT Obstructive Sleep Apnea Syndrome (OSAS) is defined as recurrent episodes of complete or ... more ABSTRACT Obstructive Sleep Apnea Syndrome (OSAS) is defined as recurrent episodes of complete or partial obstruction of the upper airway during sleep. The airflow can be reduced or completely stopped despite of inspiratory effort, resulting in intermittent episodes of hypoxemia and hypercapnia. OSAS may be a factor in the worsening of nocturnal hypoxemia, of the underlying disease, leading to acute chest syndrome. The aim of this work was to review data on the pathophysiology of OSAS in children and adolescents with sickle cell anemia. We revisited articles published over the last ten years linked to the Medline and Lilacs databases, as well as cross-referencing using these articles. The following keywords were used: sleep apnea, obstructive sleep apnea, sickle cell anemia, sickle cell disease. Studies suggest that OSAS may increase clinical complications, such as painful crises, delayed growth, cognitive and intelligence disorders, nocturnal arterial desaturation and strokes in children with sickle cell anemia. Introdução: Este trabalho visa discutir uma das complicações da Artroplastia total do joelho, apr... more Introdução: Este trabalho visa discutir uma das complicações da Artroplastia total do joelho, apresentando um caso clínico e o tratamento indicado. Vários fatores podem levar à instabilidade patelar após Artroplastia Total do joelho (ATJ). Problemas de desenho da prótese, posição do componente femoral, mau posicionamento do componente tibial, valgo excessivo, tensão no retináculo lateral. Objetivos: Discutir estas possibilidades e os tratamentos recomendados para cada situação. Apresenta-se também um caso clínico de luxação patelar pós-ATJ e o tratamento proposto. Materiais e Métodos: Apresenta-se um caso clínico de luxação da patela após dois meses de ATJ, no qual optou-se pelo realinhamento proximal e distal com osteotomia e medialização do Tubérculo anterior da Tíbia (TAT), bem como fixação com parafusos corticais. A opção baseou-se no fato de constatar-se, nas radiografias e no exame intraoperatório, que os componentes da prótese encontravam-se firmes e sem nenhum erro importante de posicionamento. Resultados: Observou-se o retorno da função do mecanismo extensor, com a patela recolocada em sua posição natural, e a presença da restauração da marcha. No pós-operatório, não ocorreu nenhuma complicação. Conclusão: Procura-se defender a osteotomia da TAT como uma possibilidade de tratamento para casos selecionados de luxação da patela após ATJ. Chama-se a atenção para as complicações e, segundo critérios de Wolf e Whitside, defende-se uma boa técnica como forma de evitá-las. Palavras-chave: Joelho. Luxação. Artroplastia.

Esta enfermidade associada à lesão de manguito rotador constitui a principal indicação de artroplastia de ombro sendo a prótese reversa a principal opção nos casos de degeneração avançada da articulação gleno-umeral. Objetivo: esse estudo tem por objetivo apresentar um caso de paciente submetida à artroplastia reversa do ombro direito e seus resultados após aplicação de programa de tratamento fisioterapêutico. Metodologia: As informações foram colhidas através do relato da paciente durante a avaliação e nos prontuários do médico e da fisioterapeuta. Aplicou-se a University of California at Los Angeles (UCLA) no período pós operatório e após 6 meses de programa de fisioterapia. To evaluate the influence of hip dysfunction on quality of life of pediatric patients with sickle cell disease (SCD). Methods. Clinical demographic, Charnley hip score, Barthel scale and Pediatric Quality of Life Inventory (PedsQL 4.0) questionnaires were used. Results. Seventeen patients were placed into the group “with hip dysfunction,” and 54 into the group “without hip dysfunction.” The group “with hip dysfunction” had a higher average age ( P = .026), weight ( P = .029), and height ( P = .019) than the group “without hip dysfunction.” There was also a higher prevalence of hip trauma ( P = .05) and pain ( P = .00). The study demonstrated that hip dysfunction negatively influenced quality of life in the physical activity ( P = .11) and psychosocial ( P = .003) domains. Conclusion. Hip dysfunction and previous hip trauma negatively influenced quality of life, especially in the physical activity and psychosocial domains.

To estimate the prevalence of obstructive sleep apnea syndrome (OSAS) in children and adolescents... more To estimate the prevalence of obstructive sleep apnea syndrome (OSAS) in children and adolescents with sickle cell anemia (SCA); to investigate the possible correlation between mean annual hemoglobin level and total sleep time with SpO2 < 90%, as well as between mean annual hemoglobin level and total sleep time with SpO2 < 80%; and to investigate the possible correlation between apnea-hypopnea index (AHI) and painful crisis. The study involved 85 patients with SCA. The patients completed a questionnaire, were submitted to polysomnography and underwent clinical evaluation (by a pediatrician and an otolaryngologist). An AHI > 1 was considered indicative of a diagnosis of OSAS. The prevalence of OSAS was 10.6%. Ultrasonography of the hip performed by the Graf method is widely accepted as an important diagno... more Ultrasonography of the hip performed by the Graf method is widely accepted as an important diagnostic and treatment tool for hip dysplasia. This study aims to examine the reliability and reproducibility of the interpretations of ultrasound scans by general orthopedists. Thirteen examiners evaluated five hip ultrasounds of the same normal child. The examiners measured alpha and beta angles according to Graf. Statistical analysis was performed by variance analysis (ANOVA) and agreement was calculated and expressed as the intraclass correlation coefficient (ICC). The alpha angle mean was 37.32 degrees (11.12) and the beta angle mean was 45.05 degrees (10.49). Error of the intra-observer was 17.65 for the alpha angle and 17.63 for the beta. Error of the inter-observer was 7.68 for the alpha angle and 17.21 for the beta. ICC intra-observer was 0.18 for the alpha angle and 0.29 for the beta. The aim of this study is to describe the clinical and functional profile of these patients. 136 patients undergoing clinical treatment for PSS were evaluated by a questionnaire regarding the following variables: sex, age, occupation, affected shoulder and its dominance, image diagnosis, pain location, intensity and characteristics, physical activity involving the shoulder, preferential position of upper limbs during occupational activity, limitation of movements during activities o daily living, and job absenteeism. 74.3% of the patients were women (p<0.001), and 54.4% were at or below the mean age of 50+/-12.5 years. The right shoulder was affected most frequently (58.8%) and dominant (91.9%). Rotator cuff tear was the most frequent shoulder pathology (75.4%). Introduction: Pregnancy in adolescence is a frequent public health problem worldwide, because it ... more Introduction: Pregnancy in adolescence is a frequent public health problem worldwide, because it is associated with increased clinical and social complications affecting both mothers and newborn/infants. Materials and Method: An analytical prospective study was conducted, comparing 200 pregnant women followedup during pre-natal care in the city of Cruz das Almas-Bahia-Brazil, in the period between November, 2009 and October, 2010. Women were divided into two groups: adolescents (between 10 and 19 years old) and adults (between 20 and 36 years old). The following socio-demographic variables were analyzed: self-declared color, educational level, marital status, family income. The maternal clinical variables evaluated were: parity, gestational age, gynecological age, weight, height, number of pre-natal consultations, weight gain during pregnancy, mode of birth and complications at birth. The clinical variables described for the newborn were: prematurity, birth weight, Apgar Score in first and fifth minute post-birth. Results: The socioeconomic characteristics differed between the groups. Among adolescent group, there were more single women; family income was lower; first sexual intercourse took place earlier than adult group. There was no difference in maternal and newborn clinical outcomes between the groups, with exception of birth weight, which was lower in adolescent group than adult group. Conclusion: Pregnancy in adolescence is associated with greater social vulnerability, and not with biologic risk when compared with pregnancy at adult age. General Context: The results founded herein demonstrated that pregnancy in adolescence was not associated with unfavorable perinatal or obstetric results, but was associated with greater social vulnerability when compared with those of adult pregnant women.
