Methods This is a prospective, randomized comparative study of consecutive patients undergoing anterior orbitotomy and mass excision enrolled into two groups: group A (with drain) and group B (without drain). Clinical data included visual acuity assessment, proptosis measured by exophthalmometry, pain score assessment, eyelid swelling, and ocular motility. Postoperative data were compared for one to five days and at 14 and 30-day follow-ups in the two groups to evaluate the efficacy of closed-suction drain in orbital mass excision. Results Twenty-five patients planned for anterior orbitotomy were divided into two groups: group A (drain, n = 12) and group B (without drain, n = 13). The subsidence of proptosis (p = 0.041), eyelid swelling (p = 0.04), and restoration of ocular motility (p = 0.04) were faster in the drain group as compared to the non-drain group, which was observed as statistically significant. The outcomes at 30 days were comparable in both groups and none of the patients developed any long-term complications. Conclusion The use of orbital drains aids early postoperative recovery with faster subsidence of proptosis and eyelid edema, and rapid recovery of ocular movements but does not affect the final outcome. Orbital surgeons can individualize the use of closed-suction drains after anterior orbitotomy in cases with expected postoperative edema.
Cutaneous horns are uncommon skin tumors consisting of keratotic material infrequently found on e... more Cutaneous horns are uncommon skin tumors consisting of keratotic material infrequently found on eyelids. We report the case of a 65-year-old male with a two-month history of cutaneous horn arising from the right lower eyelid. Histopathological examination following the excision biopsy disclosed a keratotic basal cell carcinoma (BCC). Basal cell carcinomas are slow-growing lesions with a history ranging from months to years. The keratotic form of BCC is a less common presentation of a cutaneous horn. A cutaneous horn is usually derived from an underlying lesion that may be benign, premalignant, or malignant. The diagnostic dexterity of keratotic BCC emphasizes the importance of histopathological confirmation in establishing the diagnosis and modifying management. To the best of our knowledge, this is the first case report of a keratotic BCC masquerading as a small cutaneous horn with such a short duration.

Heat also radiated from our domestic refrigerator. This waste heat from refrigerator will affect the environmental because as heat in the environment will increases it will cause global warming. So it is necessary that a significant effort should be made for conserving energy through waste heat recovery too. So An attempt has been made to utilize waste heat from condenser of refrigerator. This heat can be used for various of domestic and industrial purposes. In minimum constructional, maintenance and running cost, this system is very useful for domestic purpose. It is valuable alternative approach to increase overall efficiency and reuse the waste heat. The study has shown that such a system is technically feasible and economical. This system is a cabin that we are going to install over the head of the simple refrigerator, this cabin will be an arrangement of coils that will work as a heat exchanger for reuse of waste heat. These coils are hot coils of condenser of the refrigerator that will be modified and will put in the cabin. It can serve the purpose of cooking, geysers etc. Besides, the refrigerator may be used as conventional refrigerator. Further COP will also increase. Heat rejection may occur directly to the air in the case of a conventional household domestic refrigerator having aircooled condenser. This system rejected less heat to the environment so it is safer in environmental aspects also.

Background: There is a high risk for healthcare workers (HCWs) to get infected with severe acute ... more Background: There is a high risk for healthcare workers (HCWs) to get infected with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) during the ongoing pandemic, which may last for many months. Inadequate preparedness of the healthcare delivery system, such as shortage of personal protective equipment (PPE), lack of hand hygiene facilities, insufficient manpower capacity building may compromise the safety of HCWs and quality of care. Therefore, it is essential to ensure efficient workplace readiness for the HCWs for their safety and thereby the continued quality of care. Materials and methods: A cross-sectional web-based survey was conducted to assess workplace preparedness at healthcare facilities in northern India, especially the availability of PPE, HCWs capacity building, and mitigation effort taken by institutions. A 20-item structured, prevalidated questionnaire was circulated among nurses, doctors, premedical, and other categories of healthcare workers deployed at different level of healthcare facilities in conveniently selected states of northern India. This web-based survey was conducted during the month of April-June 2020, where 1,218 HCWs voluntarily participated. Results: Out of 1,218 participants, the majority of them reported adequate supply of PPE (804; 66%), availability of sufficient hand hygiene facility (802; 65.8%), organization of capability building and upskilling training program on COVID-19 for them (896; 73.6%), and perceived institute's excellent to good level of support while working with patients of COVID-19 (890; 73.1%). Conclusion: There was provision of adequate PPE, infection control facilities, upskilling of HCWs, and support system for the healthcare warriors in northern India to maintain adequate safety of HCWs and functionality of the healthcare system in this crucial time.

We evaluated the long-term outcome of corneal neurotisation in the treatment of unresponsive unilateral neurotropic keratopathy using surgical variations to mimic and expedient the surgical procedure. Methods A Prospective interventional study involving patients with unilateral neurotrophic keratopathy (NK) who did not respond to medical measures was conducted. The study parameters evaluated were best-corrected visual acuity improvement, ocular surface evaluation parameters [tear break-up time (TBUT), Schirmer's 1, and ocular surface staining scores (corneal and conjunctival staining)], central corneal sensation (Cochet Bonnet esthesiometer), subbasal nerve fiber length (SBNFL), and sub-basal nerve fiber density (SBNFD) determined by central confocal microscopy at recruitment and during follow-up at 1-month, 3-month, 6month, 9-month and 12-month respectively, following corneal neurotization. PURPOSE: The purpose of the study is to evaluate the safety and efficacy of adjunctive use of mit... more PURPOSE: The purpose of the study is to evaluate the safety and efficacy of adjunctive use of mitomycin-C (MMC) using two different concentrations 0.2 mg/ml and 0.4 mg/ml for lacrimal duct probing to treat the nasolacrimal duct obstruction (NLDO) in adults. SUBJECTS AND METHODS: Prospective, an interventional comparative randomized pilot study of lacrimal duct probing conducted in the two study groups 0.02% MMC group (n = 30) and 0.04% MMC group (n = 30) in confirmed primary acquired NLDO of <1-year duration. Patency of lacrimal duct probing confirmed by syringing was compared at 1, 3, and 6-month follow-up in the two study groups, and corresponding subjective improvement of watering was appraised according to Kraft and Crawford grading. RESULTS: Patency of lacrimal duct probing in 0.02% MMC versus 0.04% MMC group was 66.66%/73.33% (P = 0.71) at 1 month, 46.66%/66.66% (P = 0.09) at 3 months and 46.66%/66.66% (P = 0.03) at 6-month follow-up, respectively. Subjective improvement of watering (no watering and mild watering) observed in 0.02% MMC versus 0.04% MMC group was 46.66%/73.33% (P = 0.03), at 1 month, 66.66%/83.33% (P = 0.13) at 3 months and 56.66%/73.33% (P = 0.17) at 6-month follow-up. CONCLUSION: Adult lacrimal duct probing with 0.04% MMC was associated with significant higher objective success rate than adult lacrimal duct probing with 0.02% MMC, without added concurrently side effects.

The rest of the ophthalmic examination was unremarkable. MRI showed no extension into orbit or extraocular muscles. An excision biopsy with the amniotic membrane patch was performed. Histopathology confirmed inflammatory myofibroblastic tumour (IMT) with SMA and calponin positivity on immunohistochemistry. Complete surgical excision, amniotic membrane, and oral steroids provided long-term relief from recurrence. The conjunctiva is a rare site for IMT, and IMT should be kept in the differentials for atypical cases of ocular surface neoplasia.

Reaction time is the time taken by an individual to react to a stimulus. It is an indirect index of processing capabilities of the Central Nervous System and is affected by various factors. Aims &amp; Objective: The present study was undertaken to see the relation between Body Mass Index and menstrual abnormalities and effect on Auditory and Visual reaction time in healthy young females (16-19 yrs.) with the help of Audiovisual reaction time apparatus. Method: 90 young female medical and paramedical students from Mahatma Gandhi Medical College, Jaipur, were selected for this study. A questionnaire was given to them regarding the menstrual status and any abnormalities in menstrual cycle. Students were divided into 3 groups according to their body mass index:- Group 1:- Normal(BMI 18.5 – 24.99 kg/m ), Group 2:- Underweight (BMI &lt; 18.5 kg/m ), Group 3:- 2 2 Overweight (BMI &gt; 25 kg/m ). The reaction times were then noted by audiovisual reaction timer. It was observed that students with 2 Results: High or Low BMI were more likely to have an excessive amount of blood ow and long interval between menstrual cycle as compared to normal BMI group . Auditory Reaction Time (ART) and Visual Reaction Time (VRT) were assessed by using one – way ANOVAwith post – hoc Tukey's HSD test. Both Auditory Reaction Time and Visual Reaction Time were prolonged and highly signicant (p&lt;0.0001) in underweight and overweight female groups in comparison to the normal BMI group. Conclusion: Thus Body Mass Index affects the normal menstrual cycle and the Audio-visual reaction time. Adolescent girls should have a healthy and balanced lifestyle to maintain normal BMI and menstrual cycle.</jats:p>

Therefore, through this case report, authors emphasize the importance of performing intraoperative real-time TEE in the mediastinal mass. This imaging technique helps in visualizing the external compression of cardio-vascular structure and aid in their management, obviating the possibility of large vascular structures from being injured by the surgeon during intraoperative manipulation.

Orbital complications of acute rhinosinusitis may present with painful proptosis and ophthalmople... more Orbital complications of acute rhinosinusitis may present with painful proptosis and ophthalmoplegia. Surgical management, when required comprises of endonasal endoscopic sinus clearance with or without external orbital abscess drainage. External drainage involves blind dissection and carries a risk of iatrogenic injury to periorbital structures. We describe a novel technique of endoscope guided orbital abscess drainage under direct visualisation via the external incision site. Patients with orbital cellulitis secondary to rhinosinusitis and planned for surgical intervention were recruited. After endonasal endoscopic sinus surgery, the orbital abscess cavity was opened and an endoscope was inserted externally. The cavity was examined; loculi were opened under direct visualisation till drainage was complete. This procedure was performed in seven patients with a successful outcome. The mean time to resolution was 1.5 months (36.4 ± 18.2 days). None of the patients had any recurrence or residual disease on follow up. In addition, in two cases with obstructed sinus drainage and “walling off” of frontal sinus, visualisation of the instrument placed in the drained abscess cavity via endo-nasally inserted endoscope confirmed the re-establishment of continuity of sinus opening. This approach may allow the surgeon to drain multiloculated abscess completely under direct visualization while minimising iatrogenic damage to periorbital structures. Real time display using endoscopic camera on the monitor screen also serves as a teaching and training tool during the procedure. Technique utilises the existing endoscopic set-up without the need for additional instrumentation.

He underwent orbital exenteration that was complicated by intraoperative cerebrospinal fluid (CSF) leak from lateral orbital wall. The leak was surgically repaired and the patient recovered well. We postulate the cause of the CSF leak to be twofold: necrotic periorbital tissue due to mucormycosis rendering the thin bones susceptible to damage and second, intraoperative manipulation and dissection at the orbital apex with monopolar cautery and instruments. We describe measures taken to successfully repair the CSF leak and the possible precautions that can be taken to avoid it.

RESULTS: Patency of lacrimal duct probing in 0.02% MMC versus 0.04% MMC group was 66.66%/73.33% (P = 0.71) at 1 month, 46.66%/66.66% (P = 0.09) at 3 months and 46.66%/66.66% (P = 0.03) at 6-month follow-up, respectively. Subjective improvement of watering (no watering and mild watering) observed in 0.02% MMC versus 0.04% MMC group was 46.66%/73.33% (P = 0.03), at 1 month, 66.66%/83.33% (P = 0.13) at 3 months and 56.66%/73.33% (P = 0.17) at 6-month follow-up. CONCLUSION: Adult lacrimal duct probing with 0.04% MMC was associated with significant higher objective success rate than adult lacrimal duct probing with 0.02% MMC, without added concurrently side effects.
A 63-year-male had painless, progressive, yellowish-pink, immobile conjunctival mass with promine... more A 63-year-male had painless, progressive, yellowish-pink, immobile conjunctival mass with prominent feeder vessels in the right eye of 1-year duration. The rest of the ophthalmic examination was unremarkable. MRI showed no extension into orbit or extraocular muscles. An excision biopsy with the amniotic membrane patch was performed. Histopathology confirmed inflammatory myofibroblastic tumour (IMT) with SMA and calponin positivity on immunohistochemistry. Complete surgical excision, amniotic membrane, and oral steroids provided long-term relief from recurrence. The conjunctiva is a rare site for IMT, and IMT should be kept in the differentials for atypical cases of ocular surface neoplasia.

Menstrual problems can affect the normal life of adolescent and young women. Many factors play a role in regulation of normal menstruation like genetic, hormonal factors and Body Mass Index. Reaction time is the time taken by an individual to react to a stimulus. It is an indirect index of processing capabilities of the Central Nervous System and is affected by various factors. Aims &amp; Objective: The present study was undertaken to see the relation between Body Mass Index and menstrual abnormalities and effect on Auditory and Visual reaction time in healthy young females (16-19 yrs.) with the help of Audiovisual reaction time apparatus. Method: 90 young female medical and paramedical students from Mahatma Gandhi Medical College, Jaipur, were selected for this study. A questionnaire was given to them regarding the menstrual status and any abnormalities in menstrual cycle. Students were divided into 3 groups according to their body mass index:- Group 1:- Normal(BMI 18.5 – 24.99 kg/m ), Group 2:- Underweight (BMI &lt; 18.5 kg/m ), Group 3:- 2 2 Overweight (BMI &gt; 25 kg/m ). The reaction times were then noted by audiovisual reaction timer. It was observed that students with 2 Results: High or Low BMI were more likely to have an excessive amount of blood ow and long interval between menstrual cycle as compared to normal BMI group . Auditory Reaction Time (ART) and Visual Reaction Time (VRT) were assessed by using one – way ANOVAwith post – hoc Tukey's HSD test. Both Auditory Reaction Time and Visual Reaction Time were prolonged and highly signicant (p&lt;0.0001) in underweight and overweight female groups in comparison to the normal BMI group. Conclusion: Thus Body Mass Index affects the normal menstrual cycle and the Audio-visual reaction time. Adolescent girls should have a healthy and balanced lifestyle to maintain normal BMI and menstrual cycle.</jats:p>
The authors report a case of Large Anterior mediastinal mass in which Transoesophageal Echocardio... more The authors report a case of Large Anterior mediastinal mass in which Transoesophageal Echocardiography (TEE) revealed a large mass that was compressing the right atrium, SVC, henceforth, TEE helped in guiding aspiration and surgical resection of the cystic mass. Therefore, through this case report, authors emphasize the importance of performing intraoperative real-time TEE in the mediastinal mass. This imaging technique helps in visualizing the external compression of cardio-vascular structure and aid in their management, obviating the possibility of large vascular structures from being injured by the surgeon during intraoperative manipulation.

Orbital complications of acute rhinosinusitis may present with painful proptosis and ophthalmople... more Orbital complications of acute rhinosinusitis may present with painful proptosis and ophthalmoplegia. Surgical management, when required comprises of endonasal endoscopic sinus clearance with or without external orbital abscess drainage. External drainage involves blind dissection and carries a risk of iatrogenic injury to periorbital structures. We describe a novel technique of endoscope guided orbital abscess drainage under direct visualisation via the external incision site. Patients with orbital cellulitis secondary to rhinosinusitis and planned for surgical intervention were recruited. After endonasal endoscopic sinus surgery, the orbital abscess cavity was opened and an endoscope was inserted externally. The cavity was examined; loculi were opened under direct visualisation till drainage was complete. This procedure was performed in seven patients with a successful outcome. The mean time to resolution was 1.5 months (36.4 ± 18.2 days). None of the patients had any recurrence or residual disease on follow up. In addition, in two cases with obstructed sinus drainage and “walling off” of frontal sinus, visualisation of the instrument placed in the drained abscess cavity via endo-nasally inserted endoscope confirmed the re-establishment of continuity of sinus opening. This approach may allow the surgeon to drain multiloculated abscess completely under direct visualization while minimising iatrogenic damage to periorbital structures. Real time display using endoscopic camera on the monitor screen also serves as a teaching and training tool during the procedure. Technique utilises the existing endoscopic set-up without the need for additional instrumentation.
We describe an elderly diabetic patient presenting with sudden onset right-sided proptosis and vi... more We describe an elderly diabetic patient presenting with sudden onset right-sided proptosis and vision loss secondary to rhino-orbital mucormycosis and central retinal vascular occlusion. He underwent orbital exenteration that was complicated by intraoperative cerebrospinal fluid (CSF) leak from lateral orbital wall. The leak was surgically repaired and the patient recovered well. We postulate the cause of the CSF leak to be twofold: necrotic periorbital tissue due to mucormycosis rendering the thin bones susceptible to damage and second, intraoperative manipulation and dissection at the orbital apex with monopolar cautery and instruments. We describe measures taken to successfully repair the CSF leak and the possible precautions that can be taken to avoid it.
