Case report: In a animal model of regenerative medicine of myocardium we use an anaesthesiological protocol that included Propofol, a benzodiazepine (Midazolam) and an alpha-2 adrenergic agent. Results: In a group of 15 rabbits undergoing cardiac surgery we reported 1 arrhythmic complication during stem cell injection. Discussion: The functional cardio- respiratory depressor activity of Propofol was compensated by alpha-2 adrenergic drugs, avoiding serious complications. We hypothesize that the functional cardio-respiratory depressor of different anaesthesiological drugs can be reversed by the contemporary administration of with adrenergic agents. Background In an experimental model in the rabbit, a myocardial ischemia-reperfusion injury was o... more Background In an experimental model in the rabbit, a myocardial ischemia-reperfusion injury was obtained. Subsequently, the effects of homologous bone marrow stem cell (BMSC) administration were studied. Methods In 21 New Zealand adult rabbits, ischemia/ reperfusion damage was induced by temporary occlusion of the anterior descending coronary artery. Homologous BMSCs were isolated, cultured and re-suspended for injection at the level of the ischemic zone. We evaluated the proangiogenetic effect of intramyocardial injections of BMSC at the peri-infarcted area. Histological evaluations were made after 20 days from the surgical procedure. Results In rabbits treated with intramyocardial BMSC administration, we demonstrated histologically capillary neoangiogenesis, without signs of tissue immunological reaction or of generation of new myocardial cells. On the contrary, only minimal neovascular supply was detected in rabbits treated with intravenous administration of BMSC. Only typical signs of ischemic myocardium injury were observed in the control group. Conclusion These observations suggest that the effect of direct BMSC administration in ischemic myocardium could promote a capillary neoangiogenesis, which helps to prevent ischemic myocardial damage.

Background: Splenic vein obstruction can lead to left-sided portal hypertension, which is a rare ... more Background: Splenic vein obstruction can lead to left-sided portal hypertension, which is a rare segmental portal hypertension condition, often caused by inflammatory or neoplastic disease of the pancreas. Today, adequate study by cross-sectional imaging, e.g., ultrasound and computed tomography, allows the identification of particular features of the venous collateral pathways that favor the development of gastroesophageal varices. Methods: A retrospective study of 15 cases of left-sided portal hypertension secondary to complete thrombosis of the splenic vein was performed, with special attention to the morphodynamic conditions predicting the development of gastroesophageal varices. Results: In patients with left-sided portal hypertension, gastroesophageal varices were greatly favored by two conditions: collateral pathways directed to the gastric fundus and hypertension in the left gastric vein. This last condition typically occurs when the left gastric vein inflows into an already obstructed splenic trunk, or in the case of concomitant portal hypertension. On the contrary, patients with left-sided portal hypertension and collaterals connected with the left renal or adrenal veins have minor risk of gastroesophageal varices. Conclusions: In every case of left-sided portal hypertension, upper digestive endoscopy and close follow-up are recommended. Besides, computed tomography can demonstrate particular conditions directly favoring gastroesophageal varices, and aid in selection of the appropriate therapeutic decisions.

Patients with low rectal tumors undergoing radical sphincter-sparing resection are eligible for inclusion in a randomized multicenter study. Whether two-stage Turnbull-Cutait coloanal anastomosis provides significant benefits over hand-sewn coloanal anastomosis and associated lateral ileostomy in terms of postoperative morbidity is the primary endpoint. In addition, the study aims to assess secondary endpoints such as quality of life, fecal incontinence, and locoregional recurrence of the neoplasm. Patients with adenocarcinoma of the lower rectum diagnosed by rigid proctoscopy, with histological confirmation of malignancy, and who are candidates of rectal removal and coloanal anastomosis will be included in a randomized controlled and multicenter trial. Objective: The most recent molecular acquisitions about the pathogenesis of the thoracic aortic a... more Objective: The most recent molecular acquisitions about the pathogenesis of the thoracic aortic aneurysm permit to better understand also the morphological counterpart of this disease, where atherosclerosis is not the only factor involved. The cascade of pathological processes leading to the development of the thoracic aortic aneurysms is multifactorial. Methods: We have investigated five surgical specimens of aneurysms of the thoracic aorta, collected from patients without Marfan's disease or other related genetic syndromes. Histochemistry for elastic fibers and mucosubstances has been performed, following the standard staining protocols. Results: The elastolysis, which can be considered a starting process, involves all the tunica media and it is followed by the mucoid degeneration characterized by the deposition of basophilic mucopolysaccaridic substance. Finally, the overlap of classic atherosclerotic lesions is observed, inside the whole aortic wall. Conclusions: The development of a thoracic aortic aneurysm is a complex event, where different factors and processes are involved, other than the well-known atherosclerosis, which can finally appear the most evident aspect of the overt disease.

These can develop also in the absence of other symptoms related to acute toxicity, such as neurological symptoms. Considering the absence of specific toxicological tests, the diagnosis of energy drink intoxication still remains clinical.

In-hospital mortality was 52% in group 1 and 62% in group 2. Thromboembolic complications occurred in 24% of group 1 patients and in 27% of group 2 patients. Central venous or pulmonary thromboembolism was more frequent in group 1 (10% as opposed to 2% in group 2, p=0.04). Intestinal thromboembolism was more frequent in group 2 (15% as opposed to 0% in group 1, p=0.03).
