Background: The ability of statins to reduce LDL-c plays an important role in both primary and se... more Background: The ability of statins to reduce LDL-c plays an important role in both primary and secondary prevention of atherosclerotic cardiovascular diseases. Such treatment can often be costly, but using generic atorvastatin may reduce cost by up to US$2635. In addition, a previous 8-week study found that it exhibited comparable efficacy to the brandname medication. This study aimed to evaluate the efficacy of generic atorvastatin over a longer period of six months in a real-world setting. Methods: This was a retrospective cohort study in adult patients who had received brandname atorvastatin for at least three months and then had switched to generic atorvastatin for at least six months. Lipid and safety profiles were evaluated at six months after switching. Adjusted analyses for age, sex, co-morbid disease, dosage, and indications for statin therapy were also performed. Results: During the study period, there were 488 patients who met the study criteria. The mean (SD) age of the patients was 60.97 (12.26) years, and 48.36% were male (236 patients). At six months, average total cholesterol, HDL-c, and LDL-c were all lower, from 174.43 to 166.15 mg/dL, from 51.64 to 49.51 mg/dL, and from 110.08 to 100.78 mg/dL (p < 0.001), respectively. There were no significant differences in terms of any other laboratory test results. LDL-c exhibited the highest significant reduction at 9.30 mg/dL. Stratified analyses by age, sex, co-morbid disease, dose, and indications for statin therapy revealed similar decreases in HDL-c and LDL-c as in the study population as a whole. Conclusion: Generic atorvastatin resulted in significantly lower LDL-c than name-brand atorvastatin but less of an increase in HDL-c.

Background: Refractory status epilepticus (RSE), defined as status epilepticus that fails to resp... more Background: Refractory status epilepticus (RSE), defined as status epilepticus that fails to respond to first, second and third-line therapy. The RSE is associated with high morbidity and mortality. Treatment guidelines of RSE give a spectrum of options, such as, continuous intravenous (i.v.) midazolam (MDL), or continuous i.v. propofol (PRO) as alternatives to phenobarbital (PB) or continuous i.v. pentobarbital (PTB). Objective: To study the efficacy of very-high-dose phenobarbital (VHDPB) for treatment RSE. Study design: Retrospective study. Material and Method: The authors collected and analyzed data from adult patients who were diagnosed with RSE. Results: The authors present 10 patients with RSE who were treated with VHDPB. All of them were generalized convulsive status epilepticus (GCSE). Ages ranged from 16-86 years old (mean: 43 years). PB dosage ranged 40-140 mg/kg/day (mean: 70 mg/kg/day). The duration of status epilepticus (SE) varied widely, ranged 1-44 days (mean: 7 days). PB level ranged 35.29-218.34 ug/mL (mean 88.1 ug/mL). RSE was controlled by VHDPB 70%, 30% were not controlled. Conclusion: VHDPB were considered as alternative treatment for RSE.

<jats:p>Background: Hypertriglyceridemia may contribute to hardening or thickening of the a... more <jats:p>Background: Hypertriglyceridemia may contribute to hardening or thickening of the artery walls which increases the risk of stroke and heart disease. Therefore, the normal level of serum triglyceride can prevent both cardiovascular and cerebrovascular diseases. Gamma-oryzanol, a phytosterol compound presents in rice bran oil, has been shown to exhibit lipid-lowering effects in both animals and human. Objective: To evaluate the effect of gamma-oryzanol-enriched-rice bran oil from Thai brown rice, prepared by innovation technology, on plasma triglyceride (TG) level in patients with hypertriglyceridemia. Materials and Methods: Double-blind randomized clinical trial was carried out in 54 subjects with hypertriglyceridemia. They were divided into two groups (27 patients per group) and treated with RBOh (gamma-oryzanol-enriched rice bran oil, 20,000 ppm of gamma-oryzanol), and RBOn (cooking oil, gamma-oryzanol 5,000 ppm). The treatments were performed for 8 weeks. Physical examinations and laboratory tests were performed at weeks 0 (as baseline), 4th and 8th of the study. Lifestyle modifications were conducted in both groups. Results: Significant reduction of plasma TG levels at the 4th and 8th weeks of the treatment were found in RBOh-treated group with p-value = 0.024 and 0.006, respectively. In RBOn-treated group, significant reduction of plasma TG and increase in HDL levels and was observed at the 4th week, but not the 8th week of the treatment, with p-value = 0.001 and 0.020, respectively. TG/HDL cholesterol ratio was significantly reduce at the 4th and 8th week in RBOh-treated group (p-value = 0.011 and 0.048, respectively) and at the 4th week in RBOn-treated group (p-value = 0.003). No serious side effects were found throughout the study period in any group of treatment. Conclusion: Rice bran oil containing gamma oryzanol could reduce the TG level in hypertriglyceridemic patients. The RBOh, gamma oryzanol-enriched rice bran oil, showed prominent effects in reducing serum TG level and TG/HDL ratio. Therefore, RBOh intakes might be able to help prevent the occurrence of cerebrovascular and cardiovascular disease in the patients with hypertriglyceridemia. The aim of this study was to determine the distribution or prevalence of ABO and Rh blood groups in blood donors between June 2013 to May 2017 (4 years). Method : The study was concluding on 62,201 blood donors. The blood group antigens of ABO and Rh were confirmed by micro-column gel technique. Doubtful case was confirmed by standard conventional tube method. Age, sex, prevalence of ABO and Rh were reported in percentage. Result : Female blood donors were more than male (52.8 and 47.2%, respectively). The blood group A, B, O and AB were 20.3, 34.7, 37.5 and 7.5%, respectively. The prevalence of Rh (D) positive and negative was 99.8 and 0.2%. Blood group frequency with respect to A, B, O and AB were 0.04, 0.05, 0.07 and 0.01%, respectively. They replace 60-70% of plasma in platelet components. So the amount of storage plasma can be decreased. Platelet stored in PAS have been demonstrated to have a lower risk for allergic transfusion reactions and appeared to have equivalent clinical efficacy for controlling bleeding, compared to platelets stored in 100% plasma. We try to bring PAS to replace plasma in making leukocyte poor platelet concentrates (LPPC) compared with conventional methods that use plasma, 1 bag of total buffy coat 4 units. There lead us to implement new method and set Type and Screen channel for un-risk diagnosis. And blood logistics was started at end of 2015 for transfer the RC units to the wards. The diagnosis and hematocrit of patients must be considered for cross match. Objectives: (I) To evaluate transferred to transfused ratio (T/T ratio) in patient's diagnosis, count on transfused units and the result will lead to reduce blood inventory and decrease blood wastage and budget for blood stocking. (II) To understand the diagnosis scope of RBC cross matching and issuing practices and measure efficiency using a novel quality indicator. Methods: Polyphenolic compounds content were determined by HPLC methods and antioxidant activities by DPPH and FRAP assays of the LP extract. The effect of the LP extract at 5 g/kg/day were evaluated in two rat models of IBD that included acetic acid and indomethacin induced IBD. On each day of treatment, changes of body weight, stool consistency and stool blood were scored and expressed as disease activity index (DAI). At the end of the experiments, the animals were euthanized. Colon length and spleen weight were determined, and the degree of inflammation of the colon was scored. Results: Rats in both models of IBD (acetic acid- and indomethacin-induced IBD), exhibited significant increases in DAI, macroscopic inflammation scores and spleen weights, while the lengths of colon were decreased. Compared with the reference clopidogrel product, most commercially available generic clopidogrel products contain different crystalline forms of clopidogrel. This study was aimed to compare the pharmacodynamics of a commonly used generic clopidogrel product in Thailand with the reference clopidogrel product under steady state conditions. A multiple-dose, randomized 2-way crossover study was conducted in 32 healthy male Thai volunteers. The subjects were assigned to receive 75 mg once daily of the test or the reference product for 7 days with a 2-week wash out period. Blood samples were collected on days 1, 5, 6, and 7 prior to drug administration and at 1, 2, 3, 4, 8, 12, and 24 hours after the last dose administered. The antiplatelet aggregation effects of clopidogrel were determined by using two different ex-vivo platelet aggregation tests including the whole blood impedance assay (WBA) and the VerifyNow&lt;sup&gt;®&lt;/sup&gt; P2Y12 assay. Both pharmacodynamic parameters, the maximal antiplatelet effect (E&lt;sub&gt;max&lt;/sub&gt;) and the areas under the antiplatelet effect-time curve (AUEC&lt;sub&gt;0-24h&lt;/sub&gt;), were calculated. Neither the mean values of E&lt;sub&gt;max&lt;/sub&gt; (90.70 ± 15.15 vs. 89.50 ± 10.71% inhibition) nor of AUEC&lt;sub&gt;0-24h&lt;/sub&gt; (1,892.84 ± 657.22 vs. 1,853.58 ± 673.95% inhibition × h) under steady-state conditions obtained using the WBA method of these two clopidogrel products were significantly different. The results obtained using the VerifyNow&lt;sup&gt;®&lt;/sup&gt; P2Y12 assay were consistent with those of the WBA assay. This study clearly demonstrated that ex-vivo antiplatelet aggregation effect under steady-state conditions of the test product was not significantly different from the reference product.

Recently, andrographolide, a main active compound of Andrographispaniculata has been shown to possess hypolipidemic effects in animals. To investigate the TG-lowering effects of A. paniculata extract (APE) in patients with hypertriglyceridemia (TG ≥ 150 mg/dL) using gemfibrozil treatment as the reference. A randomized controlled clinical trial was carried out in sixty subjects with hypertriglyceridemia. They were divided into three groups and treated with low dose of APE (APE-L, andrographolide 71.64-72.36 mg/day), high dose of APE (APE-H, andrographolide 119.64-120.36 mg/day), and gemfibrozil 300 mg/day.
