She underwent descending thoracic aortic aneurysm operation 1980. She had graft replacement for a residual thoracoabdominal aortic aneurysm on April 28, 1994. She didn't accept steroid therapy although she had a slight inflammatory reaction on admission. The graft replacement of thoracoabdominal aortic aneurysm was done with an aid of Carmeda closed chest support system bypass and segmental aortic cross clamping. Reattachment of the intercostal and lumbar arteries to the graft was used with button technique or interposition technique. Visceral branches including celiac axis, superior mesenteric and right renal arteries were reconstructed to an opening made in the graft with button technique and reattachment of left renal artery was used with graft interposition during selective perfusion of visceral arteries. She started steroid therapy with 5 mg of predonine to prevent inflammatory reaction postoperatively...
Infective endocarditis (IE) after transcatheter aortic valve replacement (TAVR) is a rare complic... more Infective endocarditis (IE) after transcatheter aortic valve replacement (TAVR) is a rare complication, but has a high mortality. An 86-year-old female with symptomatic severe aortic stenosis underwent TAVR at our hospital and she was discharged without complication after 10 days. She was readmitted with high fever and acute heart failure 1 month later. Blood culture revealed Staphylococcus, and echocardiography showed vegetation on the septal cusp of the tricuspid valve and perforation at the membranous ventricular septum. We decided to perform emergency operation due to active infection and intracardiac complication despite appropriate antibiotic treatment. The infected valve was replaced with a bioprosthetic valve and the right ventricular (RV)-left ventricular (LV) communication was closed with a bovine pericardial patch. The patient received the antibiotics for 6 week and was transferred to the previous facility.
A 54-year-old man with unstable angina and Wolff-Parkinson-White (WPW) syndrome was admitted. Cor... more A 54-year-old man with unstable angina and Wolff-Parkinson-White (WPW) syndrome was admitted. Coronary angiography showed 90% stenosis of the left main trunk and 75% stenosis of the obtuse marginal branch. Coronary artery bypass grafting under cardioplegic arrest was done emergently. The left internal mammary artery graft was anastmosed to the left anterior descending artery, and a saphenous vein graft was used as a sequential bypass graft to the high lateral branch and obtuse marginal branch. Immediately after weaning from cardiopulmonary bypass, paroxysmal supraventricular tachycardia (PSVT) requiring electrical cardioversion was occurred, and catheter ablation was performed on the first postoperative day. There are controversus concerning the strategies of surgical treatment for unstable angina concomitant with WPW syndrome. Coronary bypass operation may trigger PSVT in patients with WPW syndrome. The optimal timing of perioperative catheter ablation needs further discussion. Jpn.
The authors have discussed the significance of the Morlet continuous wavelet transform of bileafl... more The authors have discussed the significance of the Morlet continuous wavelet transform of bileaflet mechanical heart valve (BLMHV) sound for detecting its malfunction: consecutive single patterns on the scalogram alway suggested its malfunction, whereas the tandem pattern with two steepled figures was demonstrated in both normal and malfunctioning valves. Therefore, authors have tried to distinguish this pattern between them by manually calculated multiple scalographic parameters. Although only the sum of wavelet coefficients (SWC) is supposed to be closer to valve sound property than other parameters, its calculation was not available in the original wavelet application. Therefore, the application was customized in the current study to semi-automatically calculate the SWC ratio between two figures for classifying the scalographic pattern of malfunctioning valves, and its efficacy to distinguish valve function was compared to other parameters. Among 155 BLMHVs, 6 valves with consecutive single patterns (type-I) and other 6 with two similar needle-like narrow figures (type-II) were confirmed to be a malfunction by fluoroscopic examination, whereas 14 malfunctioning valves with the tandem pattern which showed a great difference between two figure sizes (type-III) were distinguished from 129 normal valves by the cutoff point of the SWC ratio < 0.482 with the highest AUC (0.960) compared to other parameters by the ROC analysis.

学術情報の電子化が進み、WWW上の学術情報リソースの高い流通性が求められており、これらのリソースに対する索引や目録としてメタデータを付与することが重要な課題となる。本論文では、WWW上の学術情報... more 学術情報の電子化が進み、WWW上の学術情報リソースの高い流通性が求められており、これらのリソースに対する索引や目録としてメタデータを付与することが重要な課題となる。本論文では、WWW上の学術情報文書からメタデータを生成する方法について述べる。本研究では、NII限定子を用いてDublin Core Metadata Element Setを拡張したメタデータセットを用いた。まず固有名詞情報やリンク元のアンカーテキストを手がかりに、大学内のWebページをタイプ判定し、タイプ別にヒューリスティックルールを用いてメタデータの生成する。名古屋大学Webサイト資源に対してメタデータ生成実験を行った結果、本手法の実現可能性を確認した。 By electoronization of scientific information, the distributivity of the scholarly information resource on WWW is strongly required. 症例は 52 歳,男性.頭痛および左頸部痛のため近医に救急搬送された.瞳孔不同および左眼の視野障害を 認め,緊急頭部 MRA および胸部造影 CT を施行し,左総頸動脈の閉塞を伴う Stanfo... more 症例は 52 歳,男性.頭痛および左頸部痛のため近医に救急搬送された.瞳孔不同および左眼の視野障害を 認め,緊急頭部 MRA および胸部造影 CT を施行し,左総頸動脈の閉塞を伴う Stanford A 型大動脈解離を 疑われ,当院に転院した.緊急手術を行ったが,上行大動脈に付着した血栓が左総頸動脈へ連続しており, 左頸動脈血栓除去および上行部分弓部置換術を行った.術後経過は良好で,第 16 病日に独歩退院した.胸 部大動脈内血栓は稀であり,なかでも上行大動脈に存在する頻度は少ないので報告する.日心外会誌 45 巻 1 号:52-56(2016) キーワード:胸部大動脈内血栓;左頸動脈閉塞;上行部分弓部置換術 Fifty-two-year-old man who suffered from headache and left neck pain was brought to a nearby hospital by ambulance. Anisocoria and disorder in the field of view of the left eye were observed. Emergency brain MRA showed obstruction of the left internal carotid artery. The patient was transported to our hospital for emergency surgery for suspected acute type A aortic dissection on CT scan. Operative findings revealed a thrombus attached to the ascending aorta continued to left common carotid artery. Thrombectomy for left carotid artery and partial arch replacement were performed. The patient was discharged in good condition on the 16th postoperative day. We encountered a very rare mural thrombus in the ascending aorta. Jpn. J. Cardiovasc.
