In a hospital environment communication starts at the handover. I am given a written handover with the patient's medical conditions and care needs, their wellness (whether a patient has become unwell or had a crisis overnight and now needs a higher level of care) level of assistance and support needs and type of support required and their preferences (preferred name, dietary choices, social or emotional choices.) The baseline for the patient's needs and preferences are identified, and by reading the patient's notes and their personal care plan further identifies their needs.
As an HCSW on a surgical ward I am given a hand-over every morning for each patient, the medical ... more As an HCSW on a surgical ward I am given a hand-over every morning for each patient, the medical condition they have presented with, whether they are post or pre-op, on-going medical treatment and any other health needs (including social, emotional and/or mental health needs) and a plan of care (for the nursing staff to ensure continuity of care, and to inform them which members of the multidisciplinary team need to be communicated and/or are involved with their the care). A full past medical history will also be included on the hand over, (medical history, social or emotional needs and mental health needs as well as the individuals family/living circumstances, whether they live alone, have carers, or are caring for a family member, whether they are independent and mobile or need assistance, have communication needs etc.)
