A literature review and a case report ଝ Balanopostitis por Actinotignum en niños. Revisión bibliográfica y presentación de un caso Dear Editor: Actinotignum schaalii (A. schaalii, formerly known as Actinobaculum schaalii) is a facultative-anaerobic gram-positive bacillus that was first described in 1997. The Actinotignum genus includes 2 other species: Actinotignum urinale and Actinotignum sanguinis. A. schaalii is the species most frequently associated with infection in humans, 1 especially urinary tract infections (UTIs), with 172 cases described through October 2016, 6 of them in paediatric patients (ages o to 15 years). Although most of the cases of infection by A. schaalii described to date in the literature have occurred in adult patients, the paediatric population seems to be susceptible as well. We present the first case in which A. schaalii was isolated in a paediatric patient with balanoposthitis, who was managed in the outpatient paediatric emergency department of the Hospital Universitario San Cecilio of Granada, Spain. The patient was a boy aged 7 years that presented in the emergency department with inflammation of the penis that had become apparent that day. He did not have urinary symptoms or systemic manifestations. The most relevant finding of the history taking was that the patient had spent the weekend at the beach. He also had a history of phimosis and balanopreputial adhesions treated with topical corticosteroids at age 5 years. He was correctly vaccinated and had no other personal or family history of interest. The physical examination revealed inflammation and erythema in the glans, foreskin and shaft of the penis, with pain on retracting the foreskin and appearance on manipulation of an abundant yellow-green purulent exudate. This paper contributes to the field of physical network interconnection decision support. This ty... more This paper contributes to the field of physical network interconnection decision support. This type of problems are very complex to solve for two main reasons: The computational complexity is high due to its combinatorial nature and to the calculation of parameters to evaluate the solutions. Usually, these parameters are related to the deployment investment, or performance and reliability. Geographical and statistical factors are used for their calculation and are normally considered as constant regardless of the specific deployment place. The main contribution of this work is to consider these factors as variable, and to evaluate their effect. A case study is presented and vectorized maps and image processing is used in order to provide the proper scenario for the evaluation. The objective can be defined as a simple question: Is the interconnection decision different when these factors are constant or variable?

Last year, the American Journal of Infection Control published our retrospective analysis on the ... more Last year, the American Journal of Infection Control published our retrospective analysis on the identification and antibiogram of all bacteria isolated from urine samples with microbiologic confirmation of urinary tract infection in the Microbiology Department of "Virgen de las Nieves" University Hospital Complex (Granada, Spain) between January 2006 and December 2012. 1 Isolates were classified as susceptible, intermediate, or resistant to each antibiotic according to Clinical and Laboratory Standards Institute (CLSI) recommendations before the 2014 criteria. 2 The new interpretive criterion for susceptible-dosedependence introduced by the CLSI in 2014 results in greater cefepime exposure through higher and/or more frequent doses up to the approved maximum dosage. 3 Hamada et al 4 recently compared the cefepime susceptibility of 2,596 and 1,278 nonurine isolates of Enterobacteriaceae and Pseudomonas aeruginosa, respectively, using the CLSI breakpoints before and after 2014. 2,3 As they suggest, the application of new breakpoints appears appropriate not only for Enterobacteriaceae but also for P aeruginosa. Th... more Infectious diarrheas are highly frequent and responsible for a major consumption of resources. This study identified the main diarrhea-causing microorganisms in a health area of Granada (Spain) and determined changes in the epidemiologic pattern over a five-year period. A retrospective study was conducted based on results obtained in the Microbiology Laboratory of Hospital Universitario Virgen de las Nieves (Granada, Spain). Out of the 25,113 stool microbiological and/or parasitological studies ordered, 2,292 microorganisms were identified in 2,152 samples from 1,892 patients. There was a predominance of bacterial diarrheas (50.1 %), mainly caused by Campylobacter spp. (22.2 %), whose frequency increased significantly during the last two years, and by Salmonella spp. (16.4 %), whose frequency remained stable during the whole study period. This technique is simple and detects carbapenemases in 4.5h with high sensitivity and specificity.

Many authors have studied its association with infections. By meta-analysis viruses are the most studied agents, relationship with the Borna virus and human endogenous retrovirus W. Also, C. pneumoniae and C. psittaci DNA in blood are more common in patients. Finally, there is association with parasitism by T. gondii, despite the existence of publication bias. Serologically, in our environment, anti-Toxoplasma IgG may be a risk factor related to schizophrenia, and may have potential value for better diagnosis and prevention.

Two groups of patients were compared in a retrospective study. The results of rectal examination, ultrasound scan, microbiological analysis, and response to different antibiotic therapy regimens were compared. There was a high incidence of perineal-testicular pain and sexual potency reduction; prostate congestion and pain on rectal examination were frequently reported. All the patients had positive microbial cultures, with Gram-negative microorganisms being predominantly isolated. Following the administration of different antibiotic therapy regimens, symptoms either disappeared or diminished, irrespective of whether positive cultures remained. The present study aimed to analyze the pattern of p53 expression and its influence on survival in... more The present study aimed to analyze the pattern of p53 expression and its influence on survival in patients with oral squamous cell carcinoma (OSCC). An immunohistochemical technique with BP53-12 antibody was performed on stored tissue from 78 patients with OSCC (intraoral cancer n=40; lip cancer n=38). The nuclear and cytoplasmic extension of p53 staining was assessed. Clinical and histopathological data were gathered and the patient survival was analyzed. 57.7% (n=45) of the OSCCs expressed p53, with nuclear expression in 52.6% (n=41) of cases and cytoplasmic expression in 24.4% (n=19). The OSCCs with extensive nuclear expression of p53 showed dissociated patterns of invasion of adjacent tissues (p<0.05). A greater extension of cytoplasmic expression of p53 most commonly appeared in tumors that were better-differentiated (p<0.005), more keratinized (p<0.01) and with less nuclear atypia (p<0.05). Los diferentes estudios no han concluido que sea la causa directa de la enfermedad y no excluyen que sea un factor favorecedor de la misma en una parte de la poblacion. Encontrar a la bacteria en la escena del crimen no significa que sea la causa. Es nuestra tarea seguir investigando para descartar la posibilidad de tratar la AT con antimicrobianos. Hasta la fecha se desconocen las caracteristicas exactas de este tratamiento: su naturaleza, duracion y consecuencias. Otro aspecto es que, dada la alta prevalencia de la infeccion, nuestro esfuerzo se debe dirigir hacia la prevencion de la misma. The Alpha-method ELISA for detection of anti-cytomegalovirus (CMV) IgG antibodies (test 1) was co... more The Alpha-method ELISA for detection of anti-cytomegalovirus (CMV) IgG antibodies (test 1) was compared with another ELISA technique MEIA (test 2). Samples (248 sera and 56 cerebrospinal fluid) from patients with suspected CMV infection were investigated. Discordant samples were re-analysed, undiluted, with a latex test. Positive results were considered to be true positives when there was agreement in the results from the two methods. There were fourteen discrepant samples (4.6%). The latex agglutination test confirmed eight positive for test 1 and six positive for test 2. The overall diagnostic yield of tests 1 and 2 was sensitivity, 91% and 100%; specificity, 99% and 96%; positive predictive values, 95% and 85%; and negative predictive values 98% and 100%, respectively.
