We describe two cases of Binswanger's disease of pre-senile onset which presented with affective ... more We describe two cases of Binswanger's disease of pre-senile onset which presented with affective and psychotic symptoms well before the appearance of cognitive deterioration and neurological signs, initially evading an accurate diagnosis. Psychiatrists should be aware of white matter disease and its role in the pathogenesis of psychiatric illness. Particular attention should be given to a history of hypertension as a risk factor in the early identification of these cases.

Tests of word and face processing were given to patients with complex partial epilepsy focussed o... more Tests of word and face processing were given to patients with complex partial epilepsy focussed on the left or right temporal lobe, and to non-epileptic control subjects. The left TLE group showed the greatest impairment on object naming and on reading tests, but the right TLE group also showed a lesser impairment relative to the normal control subjects on both tests. The right TLE group was selectively impaired on distinguishing famous from non-famous faces while the left TLE group was impaired at naming famous faces they had successfully recognized as familiar. There was no significant difference between the three groups on recognition memory for words. The implications of the results for theories of the role of the temporal lobes in word and face processing, and the possible neural mechanisms responsible for the deficits in TLE patients, are discussed.

The objective of this study was to determine the extent to which junior doctors assess depression... more The objective of this study was to determine the extent to which junior doctors assess depression in older medical inpatients and how much this could be influenced by a teaching session and the availability of a depression rating scale; also to determine the attitudes to depression of hospital doctors and senior nurses involved in the medical care of older people. Case note scrutiny was used on two occasions before and after a teaching session and distribution of depression rating scales, together with administration of the Depression Attitude Questionnaire. Participants were medical inpatients over 65 years old at a district general hospital and medical and senior nursing staff involved in their assessment and care. No patient had a documented mental state assessment at admission. This was unchanged following intervention. The questionnaire suggested that the problem lay not with knowledge, but with willingness to be involved in managing depression. In conclusion, the management of depression in older medical inpatients was unsatisfactory. The questionnaire suggested that future interventions to improve care should focus on encouraging 'ownership' of depression management rather than providing 'one-off' teaching.

Background: The aim of this survey is to investigate professional attitudes to the presence and v... more Background: The aim of this survey is to investigate professional attitudes to the presence and value of spiritual care from Old Age Psychiatrists.Method: All registered members of the Faculty of the Psychiatry of Old Age in the United Kingdom were asked to complete a 21-question semi-structured questionnaire. The first mail shot took place in 2002 and the second mail shot to non-respondents in 2003. Quantitative and qualitative analyses were carried out on the answers received.Results: The response rate was 46%. The majority of respondents (92%) recognize the importance of spiritual dimensions of care for older people with mental health needs and about a quarter of respondents appear to consider referring patients to the chaplaincy service. Clinicians and other health professionals have a duty to provide teaching to medical students and supporting them to achieve their learning objectives. This paper reviews the literature on workplace learning and discusses the importance of providing a fruitful and welcoming clinical placement to medical students in Old Age Psychiatry. It then describes a local scheme to improve medical students placement and how the changes introduced in this scheme helped to enhance learning and satisfaction of the medical students. There has been increasing concern In recent years that trainee psychiatristsare not receiving ade... more There has been increasing concern In recent years that trainee psychiatristsare not receiving adequate training and are not fully competent in the administration of electroconvulsive therapy (ECT).This has led to the publication of guidelines by the Royal College of Psychiatrists recommending measures to improve supervision and tuition. Thisstudy compares the results of two surveys, five years apart, exploring levels of supervision, satisfaction with training and confidence in the procedure among trainee psychiatrists on the Royal Free Hospital scheme. The findings are discussed in the light of improvements to teaching of ECTin the interim. Continuing problems with supervision are highlighted, and the need for organised training is emphasised.
