Concerns have been raised that many kidney donors do not receive adequate medical care after nephrectomy. In 2003, our program developed a policy recommending that donors receive medical follow-up by 12 months postnephrectomy. We hypothesized that medically complex donors would have a higher rate of follow-up than other donors. Methods. A retrospective cohort study of 137 live kidney donors from a single center was performed. Donors were considered medically complex if they had hypertension, body mass index of 30 or greater, nephrolithiasis, age 65 years or older, creatinine clearance less than 80 mL/min/1.73 m 2 , or had a first-degree relative with diabetes mellitus. Adequate follow-up was defined as one visit with a nephrologist at our center, or blood pressure, serum creatinine, and urinalysis checked elsewhere. Results. Eighty-three donors (61%) had adequate follow-up, 42 did not, and 12 could not be contacted. At multivariate logistic regression, donors with adequate follow-up were more likely to be medically complex (odds ratio, 2.48; 95% confidence interval 1.18 -5.23; P Ï .02) and older than donors with inadequate follow-up (odds ratio, 1.46 per 10 years of age; 95% confidence interval, 1.01-2.10; P Ï .04). Conclusion. A substantial minority of donors do not receive recommended care by 1 year after nephrectomy.

One specific strategy presented is the use of these non-traditional venues for HIV testing and co-location of services. African American churches can be at the forefront of this endeavour through the provision of HIV testing and linkage to care. However, there are few interventions to promote the churches' involvement in both HIV testing and linkage to care. We conducted 4 focus groups (n = 39 participants), 4 interviews and 116 surveys in a mixed-methods study to examine the feasibility of a church-based HIV testing and linkage to care intervention in Philadelphia, PA, USA. Our objectives were to examine: (1) available assets, (2) challenges and barriers and (3) needs associated with church-based HIV testing and linkage to care. Analyses revealed several factors of importance, including the role of the church as an access point for testing in low-...

A research team composed of nine members of the community along, with the authors of this study, processed a vast range of economic data with the intent of identifying and discussing key pieces of economic information. In the end, water dependent industries in Portage County accounted for about one-quarter of the economic activity in the County.

The first essay examines the impact of health status on the duration of unemployment spells and finds that individuals with impaired health will have significantly longer unemployment spells. These longer unemployment spells will result in the stock of the unemployed being composed of a larger proportion of individuals

Promotion of HIV testing and linkage to care is a national health imperative for this population. As a pillar in the Black community, the Black Church could have a significant impact on the promotion of HIV testing within their churches and surrounding communities. Churches, however, have varied levels of involvement in testing. Furthermore, little is known about how to assess a church's readiness to integrate HIV testing strategies into its mission, much less how to promote this practice among churches. This qualitative study used interviews and focus groups with pastors and church leaders from four churches with varying levels of involvement in HIV testing to identify key stages in the progression of toward church-based HIV testing and linkage to care. Findings showed that churches progressed through levels of readiness, from refusal of the possibility of HIV interventions to full integration of ...

Specifically, there is some evidence that receptive speech functions may be lateralized to the right cerebral hemisphere. To test the notion that this differential specialization may be due to socio-cultural influences, receptive and expressive speech lateralization was examined in a population of acculturated bilingual Native Americans. An analysis of dichotic listening and time-sharing asymmetries indicated that despite evidence for an initial appositional tendency, the Native American subjects were lateralized to the left cerebral hemisphere for both receptive and expressive speech. It is proposed that shifts in the attended perceptual field occur as Native Americans mature perhaps due to an increasing familiarity with a second culture and more propositionally oriented language.
