Healthcare workers (HCW) have been one of the most affected sectors. Children have also been affected, and although infection generally presents as a mild disease, some have developed the Pediatric Inflammatory Multisystem Syndrome Temporally Associated with SARS-CoV-2 (PIMS-TS). We recruited 190 adults (HCW and cohabitants, April to June 2020) and 57 children (April 2020 to September 2021), of whom 12 developed PIMS-TS, in a hospital-based study in Spain. Surface-exposed proteins of pathogenic bacteria play a critical role during infections . The vast... more Surface-exposed proteins of pathogenic bacteria play a critical role during infections . The vast majority of these molecules are able to trigger strong immune responses. Measuring the humoral immune response against pathogenic bacteria through less-time consuming tests is necessary to reduce the window time for the diagnosis of diseases that may be associated with high morbidity and mortality rates. Due to the multiplex setup, Luminex xMAP® technology allows analysis of immune responses against many antigens in a single assay. Therefore, less volumes of sera samples are needed and inter assay coefficient of variation is much lower in comparison with other immunoassays. With this methodology, the carboxyl groups on the surface of the polystyrene microspheres must first be activated with a carbodiimide derivative prior to coupling antigens . After the antigen is coupled to a microsphere , different microspheres (all having a unique color) can be combined whereafter the presence of specific antibodies directed against the different antigens in sera can be determined simultaneously. The platform here described can also be useful for epidemiological surveillance programs and vaccine studies.
BACKGROUND Visceral leishmaniasis (VL) is an endemic in Southern Europe. However, details regardi... more BACKGROUND Visceral leishmaniasis (VL) is an endemic in Southern Europe. However, details regarding disease burden, clinical presentations, laboratory markers, management and outcome in children are scarce. METHODS Medical records of children (<14 years) admitted with VL to 10 pediatric units in Andalusia (2004-2019) were retrospectively reviewed. VL diagnosis was based on clinical presentation, serology, microscopy and molecular methods. Diagnosis of secondary hemophagocytic lymphohistiocytosis (sHLH) was established using the hemophagocytic lymphohistiocytosis-2004 criteria. RESULTS A total of 127 patients were identified. Median age was 14.5 months; the main clinical presentations were fever and splenomegaly (95.3% each). Cytopenias were the most common laboratory abnormalities. Diagnostics as well as treatment regimens varied over time and the participating centers. Liposomal amphotericin B was prescribed in 97.6%; relapses as well as adverse events were rarely observed (3.1% each). Thirty-seven patients, diagnosed with sHLH required longer hospital admission (P = 0.001), an increased number of platelet (P < 0.006) and red blood cell (P = 0.0001) transfusions and pediatric intensive care unit admission (P = 0.007). Monocytopenia (P = 0.011) and high C-reactive protein levels (P = 0.031), variables not included in the hemophagocytic lymphohistiocytosis-2004 criteria, were associated with sHLH. One patient deceased in the context of the Leishmania infection. CONCLUSIONS We report data on the largest pediatric VL cohort from Europe, commonly associated with sHLH. Raised C-reactive protein levels and monocytopenia appear to be associated with sHLH. The latter may help to identify these patients and to guide decisions regarding need of additional supportive clinical care and immunomodulatory therapies. The observed high rate of heterogeneity in terms of diagnosis and management warrants the establishment of appropriate guidelines.

Methods We designed a quasi-experimental intervention study with an interrupted time-series analysis. A multidisciplinary team conducted a multifaceted educational intervention in our tertiary-care hospital over a 5-year period. The main activity of the program consisted of peer-to-peer educational interviews between counselors and prescribers from all departments to reinforce the principles of the proper use of antibiotics. We assessed antibiotic consumption, incidence density of Candida and multidrug-resistant (MDR) bacteria bloodstream infections (BSIs) and their crude death rate per 1000 occupied bed days (OBDs). Se describe un niño de 5 meses inmunocompetente con meningitis por Haemophilus influenzae tipo f ... more Se describe un niño de 5 meses inmunocompetente con meningitis por Haemophilus influenzae tipo f (Hif). Había recibido previamente dos dosis de vacuna frente a Haemophilus influenzae tipo b (Hib) con polirribosilribitol fosfato conjugado con la proteína de toxoide tetánico (PRP-T). Aunque inicialmente se sospechó un fracaso vacunal a partir de los hallazgos del Gram en líquido cefalorraquídeo, se identificó posteriormente un Hif no cubierto por la vacuna conjugada. Los serotipos no b (Hinb) deberían ser tenidos en cuenta como patógenos potenciales en niños menores de 5 años con enfermedades invasoras. Un programa de vigilancia epidemiológico sería de utilidad para monitorizar posibles incrementos futuros en la frecuencia de estos serotipos como patógenos significativos.

The primary reason for inappropriate use is the insufficient knowledge of the increasingly vast and complex information about the diagnosis and treatment of infectious diseases.
