With this observational study we attempted to assess whether nasal cannulas originally used to ad... more With this observational study we attempted to assess whether nasal cannulas originally used to administer high flow could be effectively used as an interface to provide ventilator generated noninvasive respiratory support. Preterm infants whose gestational ages between 26 0/7 and 29 6/7 weeks with respiratory instability who initially received noninvasive respiratory support with binasal prongs and then switched to nasal cannula with attending physician's decision were included. Six infants (27%) needed intubation and mechanical ventilation while getting noninvasive support via nasal cannula, whereas this was not observed during noninvasive ventilation via short binasal prongs (p=0.02). Despite the nasal cannula being easy to use and well tolerated by the preterm infant, it is not as effective as the short binasal prong when it is used as an interface in a mechanical ventilator that provides noninvasive respiratory support other than its own equipment.

The direct antiglobulin titer (DAT) test is regarded as the cornerstone of diagnosis of immune hemolytic disease of the newborn. Methods: Patients with hyperbilirubinemia who were born in our hospital and whose gestational age was over 35 weeks were enrolled. DAT positive and DAT negative patients were compared in terms of light emitting diode (LED) phototherapy efficacy. Results: Seventy-seven cases were DAT-negative and 72 were DAT-positive. No statistically significant differences were found for the duration of phototherapy and hospitalization between the DAT-positive and negative groups. Methods Retrospective chart review of babies born in the Regional Maternity Hospital Limerick bet... more Methods Retrospective chart review of babies born in the Regional Maternity Hospital Limerick between 1 st Jan 2007 and 31st Dec 2011 with ROP requiring treatment. Results During the study period a total of 225 infants with B.wt < 1500g and/or G.A < 32/40 were admitted to the neonatal unit. All these infants were screened for ROP as per unit guidelines. 93.3% (N=210) infants were < 32/40, and 83%(N=187)< 1500g, of these 28%(N=64) infants weighed less than 1Kg. There were 10 infants (4.4%) who developed ROP requiring treatment, 9 of these infants were in the ELBW category, the tenth baby was only 40gs over the criteria for VLBW(B.wt 1040gs). All 10 babies have disease in zone 2.7 stage2. 2stage1 and one had stage 4. Conclusion In our unit 10 infants(4.4%)received treatment for ROP over the 5 years period 2007-2011. Our figures are comparable to those reported by Vermont Oxford Network database 2010.

He was discharged on daily insulin injections subcutaneously. Results On admission glycated hemoglobin(HbA 1 c) was 5.8% (4.3-6.1%), anti-GAD autoantibodies 1.3(ratio>1.1 positive), IA2(tyrosine phosphatase antibodies) 1.2(< 8iu/ml), IAA(anti-insulin antibodies) 1.50(< 1.10 ratio), ICA(anti-islet antibodies) 0.5(< 1.00 ratio), EMA(anti-endomysial antibodies) negative. Past medical and family history were unremarkable. Genetic testing for PNDM failed to detect any mutations in the KCNJ11, ABCC8 and INS genes, as well as the testing for abnormalities in the chromosome 6q for TNDM. Conclusions Genetic testing for NDM can identify PNDM in newborns helping the physician to select the most appropriate therapy. However, 40% of cases are currently without a molecular genetic diagnosis.

We retrospectively reviewed the medical records of all Syrian immigrants from the TurkishSyrian b... more We retrospectively reviewed the medical records of all Syrian immigrants from the TurkishSyrian border who delivered the Zekai Tahir Burak Maternity and Teaching Hospital Neonatal Intensive Care Unit (NICU) in Ankara, Turkey. Between January 2013 and December 2014 a total of 36,346 women gave birth at this center. Of these, 457 women were Syrian immigrants, comprising 1.2 % (457/36,346) of all deliveries. The number of births among Syrian refugees in Turkey appears to be increasing. Further research is needed to understand the relative morbidity of babies born to Syrian refugees compared to the local population, as well as the economic impact on facilities treating these cases.

Recent studies indicate an important role of adipose tissue hormones, or "adipokines", in obesity-associated complications. To investigate the relation of two circulating adipokines (visfatin, adiponectin) with markers of insulin sensitivity and obesity in children, 40 obese children and 40 control children were recruited. Homeostasis model assessment for insulin resistance (HOMA-IR) and visfatin levels (4.99 +/- 2.08 vs. 1.47 vs. 0.7, p < 0.001; 31.3 +/- 11.1 vs. 18.5 +/- 10.7, p < 0.001, respectively) were significantly elevated and adiponectin levels (2.01 +/- 1.02 vs. 12.5 +/- 6.2, p < 0.001) were significantly lower in the obese group. The current guidelines have been proposed keeping the Indian context in mind. We would like to reemphasize that these guidelines do not provide all of the answers but suggest the general course of action that everyone needs to undertake in our day to day practices to improve child nutrition in the Indian subcontinent.

Congenital syphilis is the oldest recognized congenital infection and still represents a serious ... more Congenital syphilis is the oldest recognized congenital infection and still represents a serious healthcare problem in the 21st century. It is important to be fully informed regarding the early diagnosis and treatment of congenital syphilis to prevent its devastating complications leading to death. In this manuscript, we report a newborn infant with unusual clinical findings of congenital syphilis such as a non-fluctuant mass surrounding the left calf. She did not have any additional system involvement such as hepatic or skin involvement or lymph nodes. To our best knowledge, there are only a few case reports presented with isolated bone involvement. This case demonstrates that congenital syphilis should be considered in neonates with bone fractures, lytic bone lesions and periostitis.

Objective Our objective was to identify factors associated with sleep pattern in school age child... more Objective Our objective was to identify factors associated with sleep pattern in school age children living in Tehran city. Study Design This was a cross-sectional study of 6-9 year olds children (n=270) who recruited by multistage sampling among primary school in Tehran city. Sleep pattern and efficiency was measured for 7 consecutive days using Actigraph accelerometer. Height and weight were directly measured using standardized equipment. Other factors including age, gender, history of preterm birth, birth weight, playing video game, watching TV, family income and parental educational level gathered by standard questionnaire. Results 7days and weekdays sleep duration was significantly shorter (p<0.05) in older children while there was not the same for weekend sleep duration. After adjusting for age we found no significant associations between measured factors and sleep pattern. There was no significant gender difference for sleep pattern, but girls had better (p<0.05) sleep efficiency during 7 days and weekdays compared to boys. Conclusions Sleep efficiency showed an inverse correlation with sleep duration. No significant relationship was found between sleep efficiency during 7 days, weekdays and weekend with the personal and the environmental factors.

The aim of this study was to evaluate the preventive effect of cytidine 5′-diphosphocholine (CDP-choline) treatment on hyperoxic lung injury in a neonatal rat model. Methods: A total of 30 newborn pups were divided into control, hyperoxia, and hyperoxia + CDP-choline groups. After birth, pups in the control group were kept in room air and received saline injections, whereas those in hyperoxia and hyperoxia + CDP-choline groups were exposed to 95% O 2 and received daily injections of saline and CDP-choline throughout postnatal day 10, respectively. Histopathological scoring, radial alveolar count, lamellar body membrane protein expression, fibrosis, proinflammatory cytokine levels, lung tissue and bronchoalveolar lavage (BAL) fluid phospholipid content, and apoptosis were evaluated. results: Hyperoxia-induced severe lung damage was reduced significantly by CDP-choline treatment. Radial alveolar count and lamellar body membrane protein expression were significantly recovered, and the number of terminal deoxynucleotidyl transferase-mediated deoxyuridine triphosphate nick-end labeling-positive cells, active caspase-3 expression, and tissue proinflammatory cytokine levels were decreased by CDP-choline administration. Lung tissue and BAL phospholipid contents showed significant increases after CDP-choline administration. conclusion: These data show that CDP-choline ameliorates hyperoxic lung injury in a neonatal rat model. It may therefore be suggested that CDP-choline may be a novel therapeutic option for the prevention of BPD.

Conclusions Detection of K. kingae DNA in oropharayngeal swabs from children presenting clinical findings of OAI is highly predictive for K. kingae OAI. This test represents thus a valuable diagnostic tool, which could improve the recognition of OAI in young children.

Background: Citrulline, a nonprotein amino acid, is an intermediate of the urea cycle and synthes... more Background: Citrulline, a nonprotein amino acid, is an intermediate of the urea cycle and synthesized in small intestine. Lower plasma citrulline levels were associated with reduced function of enterocytes. Necrotizing enterocolitis (NEC) causes high morbidity and mortality, and leads impaired intestinal functions. Methods: Plasma citrulline levels of neonates with a gestational age <32 weeks and ≤1,500 gm who developed NEC stage II/III were measured by high-performance liquid chromatography. Results: We enrolled 36 preterm infants including 20 with NEC and 16 controls. Median citrulline levels of NEC and control groups were 8.6 and 20.18 μmol/l (P < 0.05), and cut off level of citrulline was 13.15 μmol/l with a sensitivity of 80% and a specificity of 82%. Median arginine levels of NEC and control groups were 22.02 and 39.89 μmol/l (P < 0.05), and cut off level of arginine was 28.52 μmol/l with a sensitivity of 70% and a specificity of 75%. Blood sampling day, gender, parenteral, and enteral nutrition did not affect the amino acid levels. Conclusion: We found lower plasma citrulline and arginine levels in preterm infants with NEC. Further studies are needed to determine most appropriate levels to predict recovery and prognosis of NEC, and treatment options with these amino acids in preterm infants.

Background: The aim of this study was to investigate any changes in mean platelet volume (MPV) in... more Background: The aim of this study was to investigate any changes in mean platelet volume (MPV) in patients with neonatal sepsis (NS). Methods: Subjects were stratified into two groups: proven sepsis (Group 1a) and clinical sepsis (Group 1b). The control group (Group 2) consisted of healthy newborns matched for gestational age and birth weight. Results: A total of 100 patients with NS (35 with proven sepsis and 65 with clinical sepsis) and 50 healthy controls were enrolled. A comparison of markers of sepsis obtained at baseline revealed white blood cell count (WBC), C-reactive protein (CRP), interleukin-6 (IL-6), and MPV levels to be significantly higher in newborns with sepsis compared to healthy controls (P = 0.01, <0.001, <0.001, and 0.001, respectively). Mean baseline serum levels of CRP and MPV were significantly higher in Group 1a compared to Group 1b (P = 0.003, P = 0.007, respectively), whereas the difference between group with regards to baseline serum levels of IL-6 and platelet count was statistically insignificant (P = 0.14, P = 0.28, respectively). Conclusion: This is the first study to demonstrate a statistically significant difference with regard to baseline MPV values between patients with sepsis (proven or clinical) and healthy controls.
