Objetive: To assess the clinical features of incidental clinically non-functioning pituitary aden... more Objetive: To assess the clinical features of incidental clinically non-functioning pituitary adenoma (NFPA) and to analyze its natural history. Methods: A multicenter retrospective study in patients with NFPA followed-up from 1992 to 2015 was performed. Results: Fifty-seven patients were studied (29 women (50.9%); age 55.8±16.7 years. 43.9% were older than 60 years, 40.3% belonged to the age group of 40-60 years and 15.8% were younger than 40 years. Most patients (n=55, 96.5%) were diagnosed by magnetic resonance imaging (MRI). 71.9% (n=41) were macroadenomas; 2 of them (3.5%) were giant adenomas (≧4 cm). Patients with macroadenomas were older than those with microadenomas (59.5±16.7 vs 46.4±18.1 years, P=0.007). Microadenomas were more common in women (41.3 vs 14.3%, P=0.023). About half of them (n=28; 49.1%) showed suprasellar extension; of these, 19 (33.3%) were accompanied by chiasmatic compression. Invasion of the cavernous sinuses was present in 28.1% (n=16). Hypopituitarism was present in 14 (24.6%) patients. Twenty-four patients (42.1%) underwent surgery. Twenty-six non-operated patients were evaluated after a median follow-up of 15.5 months (interquartile range, 5.7-32.7). No significant changes were found in the maximum tumor diameter at the end of follow-up (1.2±0.6 vs 1.2±0.7 cm; NS). The majority of NFPAs evaluated (n=23, 88.5%) did not show any changes in size. In two cases (7.7%) tumor size decreased and in one patient (3.8%) increased. Conclusion: Incidental NFPA is diagnosed by MRI preferably from the 5th decade of life without sex predilection. Most of them are macroadenomas, more commonly diagnosed in men and at an older age, compared to microadenomas. The suprasellar extension with chiasmatic compression and hypopituitarism are frequent at diagnosis. Most of the non-operated NFPAs remain with stable tumor size over time, being growth an unusual event.

Aim To evaluate the clinical characteristics and therapeutic outcomes of giant non-functioning PA (gNFPA). Patients and Methods A retrospective multicenter study of gNFPA patients diagnosed in a 12-year period was performed. In each patient, clinical data and therapeutic outcomes were registered. Results Forty patients (24 men, age 54.2 ± 16.2 years) were studied. The maximum tumor diameter [median (interquartile range)] was 4.6 cm (4.1–5.1). Women had larger tumors [4.8 cm (4.2–5.4) vs. 4.5 cm (4.0–4.9); p=0.048]. Hypopituitarism [partial (n=22, 55%) or complete (n=9, 22.5%)] at diagnosis was present in 77.5% of the patients. Data regarding the underlying maternal and perinatal risks are scarce. The objective of this nationwide study is to evaluate maternal and perinatal outcomes after BS. We performed a retrospective observational study of 168 pregnancies in 112 women who underwent BS in 10 tertiary hospitals in Spain over a 15-year period. Maternal and perinatal outcomes, including gestational diabetes mellitus (GDM), pregnancy-associated hypertensive disorders (PAHD), pre-term birth cesarean deliveries, small and large for gestational age births (SGA, LGA), still births, and neonatal deaths, were evaluated. Recent evidence has demonstrated their expression in rat and human placenta. Methods In the current study, we have investigated the expression of those neuropeptides in the rat placenta by real-time PCR using a model of maternal food restriction. Results Our results showed that placental-derived neuropeptides were regulated through pregnancy and following food restriction. Conclusion These data could indicate that placental-derived neuropeptides represent a local regulatory circuit that may fine-tune control of energy balance during pregnancy.

Most cases from the literature are secondary to fungal sepsis, but few cases of metastatic bacterial endophthalmitis have been reported. In this diabetic patient urine and blood cultures were negative for fungi and positive for Escherichia Coli. The outcome was disappointing, the enucleation of both globes being necessary. The aim of the report is to outline that metastatic endophthalmitis should be counted among the possible complications of bacterial sepsis, especially in diabetics because of their susceptibility to infection. We report herein expression profiles of Lin28 and let-7 members, and related mir-145 and mir-132, in rat testis during postnatal maturation and in models of altered puberty and hormonal deregulation. Neonatal expression of Lin28a and Lin28b was low and rose markedly during the infantile period; yet, expression patterns diverged thereafter, with persistently elevated levels only for Lin28b, which peaked at puberty. Let-7a, let-7b, mir-132 and mir-145 showed profiles opposite to Lin28b. In fact, let-7b and mir-145 were abundant in pachytene spermatocytes, but absent in elongating spermatids, where high expression of Lin28b was previously reported. Our aim was to study circulating fasting ghrelin levels and their response to oral glucose in acromegalic patients and normal control subjects. DESIGN AND PATIENTS 9 acromegalic patients (4 male, 5 female; 59.4+/-3.6 years; 28.6+/-1.0 kg/m2) and 9 age and BMI matched healthy control subjects (4 male, 5 female; 59.1+/-1.4 years; 26.5+/-0.8 kg/m2) were included. We obtained blood samples for glucose, insulin, GH, total ghrelin and acylated ghrelin at times 0, 30, 60, 90 and 120 minutes after 75 g of oral glucose. RESULTS Fasting GH and IGF-I were statistically different between patients and controls: GH (microg/l): 6.7+/-1.4 vs. 0.8+/-0.4, p<0.01; IGF-I (ng/ml): 414+/-75 vs. 86+/-6, p<0.01. Oral glucose (OG) administration affects GH secretion, initially decreasing and subsequently stimulating GH secretion. The aim of this study was to investigate the diagnostic efficacy and safety of a long OG test (LOGT) as a stimulus of GH secretion for the diagnosis of adult GH deficiency (AGHD). Prospective experimental cross-sectional study. The study was conducted at the Endocrinology department of the University Hospital of a Coruña, Spain. We included 60 (40 women) AGHD patients (15) and controls (45) paired 1:3, of similar age, sex and BMI. The area under the curve (AUC) and peak were calculated for GH. The Mann-Whitney test was used to compare the different groups. ROC curve analyses were used. p-Values&lt;0.05 were considered as statistically significant. The intervention consisted of orally administering 75g oral glucose administration; GH was obtained every 30min for a total of 300min. Peak GH area under receiver operating characteristic curve (ROC-AUC) following LOGT. Peak GH (μg/L) levels were lower in the AGHD patients (0.26±0.09) than in the controls (4.00±0.45), p&lt;0.001. After LOGT, with the ROC plot analysis the best peak GH cut-point was 1.0μg/L, with 100% sensitivity, 78% specificity, ROC-AUC of 0.9089 and 81.82% accuracy. There were no relevant adverse events during any of the LOGT. The LOGT could be a cheap, safe, convenient and effective test for the diagnosis of AGHD.

Esta hormona ha sido insuficientemente estudiada en pacientes con enfermedad renal crónica (ERC) avanzada. Objetivo: Realizar un análisis exploratorio de los niveles séricos de irisina en pacientes con diferentes modalidades de tratamiento de la ERC. Método: Según diseño transversal, estimamos niveles de irisina en 95 pacientes con ERC manejados conservadoramente (ERCA), con diálisis peritoneal (DP) o con hemodiálisis, comparándolos con un grupo control de 40 individuos sanos. También investigamos las correlaciones entre irisina sérica y variables demográficas, clínicas, metabólicas y de composición corporal. Resultados: Los niveles de irisina fueron más bajos en cualquier grupo de pacientes que en los controles. El análisis univariante desveló correlaciones moderadas entre irisina, por un lado, y masa grasa (pero no magra), filtrado glomerular (GFR) y albúmina y bicarbonato plasmático, por otro. El análisis multivariante confirmó que los pacientes con ERCA (diferencia 111,1 ng/mL), en DP (25,9 ng/mL) o hemodiálisis (61,4 ng/mL) (todos p < 0,0005) presentaban niveles ajustados más bajos de irisina que los controles. Asimismo, los pacientes en DP presentaban niveles más altos de la hormona que los de hemodiálisis (diferencia 39,4 ng/mL; p = 0,002) o ERCA (24,4 ng/mL; p = 0,036). El análisis multivariante también identificó bicarbonato plasmático (B = 3,90 por mM/L; p = 0,001) y GFR (B = 1,89 por mL/min; p = 0,003) como predictores independientes de los niveles de irisina. Por el contrario, no observamos correlación ajustada entre irisina y marcadores de composición corporal. Conclusiones: Los niveles de irisina son bajos en pacientes con ERC, y muestran correlación consistente con GFR y bicarbonato plasmático. Aim: To evaluate the clinicopathological features and therapeutic outcomes of CP diagnosed in the elderly. Patients and Methods: This was a retrospective, multicenter, national study of CP patients diagnosed over the age of 65 years and surgically treated. Results: From a total of 384 adult CP patients, we selected 53 (13.8%) patients (27 women [50.9%], mean age 72.3 ± 5.1 years [range 65–83 years]) diagnosed after the age of 65 years. The most common clinical symptoms were visual field defects (71.2%) followed by headache (45.3%). The maximum tumor diameter was 2.9 ± 1.1 cm. In most patients, the tumor was suprasellar (96.2%) and mixed (solid-cystic) (58.5%).
