Cryptococcus laurentii infection in a patient with cutaneous leishmaniasis Cryptococcosis is an o... more Cryptococcus laurentii infection in a patient with cutaneous leishmaniasis Cryptococcosis is an opportunistic infection caused by capsulated yeast: Cryptococcus species complex, it can affect lungs in 90%; however, any organ can be involved. 1 It is usually observed in patients with immunosuppression, 2,3 such as AIDS, leukemia, diabetes, connective tissue diseases, transplant recipients, and patients under immunosuppressive therapy with steroids or antibiotics. In AIDS patients, only three cases were due to C. laurentii. 1 Leishmaniasis is a tropical disease caused by an intracellular parasite, transmitted by biting flies of genus Lutzomya in America. 3 According to the WHO data, 60,000 cases per year are documented in Latin America. 3 A 65-year-old patient with no relevant medical data, presented with a 10-month history of skin ulcers. They were irregularly shaped and well delimitated and had sharp borders covered by a thick fibrin layer (Fig. 1a). The patient showed lesions on the left upper extremity, posterior aspect of the trunk and gluteal region. No improvement was observed after self-prescription of unspecified herbs. Complementary studies included complete blood count, urinalysis, coproparasitoscopic test, enzyme-linked immunosorbent assay and VDRL were normal. A biopsy from the main ulcer was performed, (histopathology and culture in Sabouraud dextrose agar). An epithelioid granuloma and histiocytes with intracellular parasites, typified as Leishmania braziliensis was observed (Fig. 1b).
