The oral form is an important clinical presentation of lichen planus. It may precede or accompany the skin lesions, or it may be the only manifestation of the disease. Dermatologists, dentists, otorhinolaryngologists, and other specialists who deal with the oral cavity must be aware of this disease so they can diagnose, treat, and keep these patients under observation.

Cytokeratins are fibrous intermediate-filament protein polymers present in almost all animal cell... more Cytokeratins are fibrous intermediate-filament protein polymers present in almost all animal cells. Their function is related to epithelium structural maintenance, protection from mechanical trauma, and possibly communication between adjacent cells or cytoplasm components. Today there are 20 known cytokeratins, classified according to their molecular weight and pH as type I or acidic (cytokeratins 9-20) and type II or neutral-basic (cytokeratins 1-8). Cytokeratins are always expressed in specific pairs for each type of tissue, composed of one unit of type I and one unit of type II. Primary structural defects of cytokeratins are associated with various keratinization impairments. Two of the better characterized defects are bullous epidermolysis and epidermolytic hyperkeratosis. Anti-cytokeratin monoclonal antibodies are being used for diagnostic purposes to characterize the origin of poorly differentiated tumors and metastatic solid tumors.

Cytokeratins, which are important components of the cytoskeleton, are excellent epithelial differentiation markers used to study neoplastic and inflammatory diseases. To study the profile of cytokeratins in OLP and their possible association with dysplastic alterations, monoclonal antibodies were used for cytokeratins 10, 13, 14 and 19, in 26 samples of OLP. The streptavidin-biotin technique was employed in paraffin-embedded tissue sections. Sample analysis revealed suprabasal expression of cytokeratin 10 in 16/17 samples, 14 of them with reduced expression; suprabasal expression of cytokeratin 13 in 18/23, 16 of them with delay; basal and suprabasal expression of cytokeratin 14 in all samples; and focal basal expression of cytokeratin 19 in 4/21. Lichen sclerosus is a chronic inflammatory disease of unknown origin, which affects mostly women ... more Lichen sclerosus is a chronic inflammatory disease of unknown origin, which affects mostly women in the fifth and sixth decades of life, but can also occur in men and children. The involvement of the oral mucosa alone or together with other forms of presentation is extremely rare, requiring a differential diagnosis with other diseases of the oral cavity, particularly lichen planus. There are less than 30 cases of lichen sclerosus in the oral mucosa described in the literature and there are no reports on malignant transformation so far. We describe a patient with skin, oral and genital lesions of lichen sclerosus.

Expression of cytokeratins 10, 13 and 14 was altered in OLP lesions. The inflammatory process and hyperkeratosis or parakeratosis seem to have interfered with the expression of these CKs. Cytokeratin 19 was expressed in the lesions, in a pattern similar to that mentioned in the literature for the non-keratinized oral mucosa. The presence of mild dysplasia did not change the expression of the cytokeratins studied. No differences in pattern of expression were observed between keratinized and non-keratinized areas in the lesions caused by OLP.

Although many classifications include oral lesions of lichen planus in the category of a premalig... more Although many classifications include oral lesions of lichen planus in the category of a premalignant condition, there is still much discussion about whether the mucous membranes lesions should be characterized as an intrinsically premalignant condition or merely as a facilitator of the action of a carcinogenic factor. The possibility that an epidermoid carcinoma can emerge at the site of lichen planus lesions, mainly in mucous membranes, has been shown; however, several published cases omit information about other potential risk factors. This prevents a complete analysis of the triggering relationship between lichen planus and squamous cell carcinoma. This contribution reviews the literature on this subject. The question of whether oral or vulval lichen planus, or both, are premalignant conditions will only be answered after prospective studies with large samples and extensive follow-up are performed, taking into consideration the great variety of risk factors involved, together with the establishment of a consensus in relation to the points still without agreement.

The harmful effect of these diseases occurs by toxic action of substances in contact with the skin, direct contact with the body of certain aggressors, inoculation of toxic substances through stings or bites, and finally by the agent's penetration of and permanent residence inside the skin. This permanence can be forever, during certain phases of the biologic cycle of the aggressor, or as a way by which the parasite reaches other organs. Parasitic agents may exclusively affect humans or other animals, and stings, bites, or contact with the body or substances cause nonparasitic lesions. This group constitutes a very large subject in dermatology, with a great variety of diseases. Most of the dermatozoonoses have an exclusive regional pattern, and for this reason the present article will emphasize leishmaniasis, besides creeping eruption, myiasis, schistosomiasis, and tungiasis, mainly as they are seen in Brazil.

In this site the lesions are very similar, making the clinical diagnosis difficult; therefore, the definition of the immunohistopathologic characteristics of each one becomes essential for a differential diagnosis. The authors review the clinical-pathological and therapeutic aspect of these oral injuries in order to help in the diagnosis, treatment and prognosis of the oral conditions of those diseases.

The skin is a vital organ for life and, among its many functions, the role as a protective barrie... more The skin is a vital organ for life and, among its many functions, the role as a protective barrier is one of the most important. It is the main boundary between the body and the external environment. As defensive barrier, the epidermis protects internal organs from physical and chemical trauma, microorganism invasion, and ultraviolet radiation. It also acts in the regulation of transepidermal movement of water and electrolytes, and in preventing dehydration, all of which are essential for sustaining life. The main role is allotted to the stratum corneum and to the lipid matrix located in the intercellular space. The occurrence of dysfunction in the epidermal barrier is an important factor in the physiopathogenesis of skin diseases, particularly atopic dermatitis and psoriasis. There are few, but important, systemic changes that influence or are influenced by dysfunctions in the epidermal barrier. We review the effects of some systemic diseases on the maintenance of the skin's homeostasis.

Determinar a prevalência de aneurisma da aorta abdominal na população da cidade de Vitória (ES) e... more Determinar a prevalência de aneurisma da aorta abdominal na população da cidade de Vitória (ES) e associar com possíveis fatores de risco. Métodos: Estudo de prevalência realizado por amostragem aleatória estratificada por faixa etária e classe socioeconômica. Os exames foram realizados nos postos de saúde dos setores censitários do IBGE sorteados. A amostra foi recrutada pela equipe do Programa de Saúde da Família, após uma campanha de conscientização da importância do diagnóstico da doença. O exame foi realizado por ecografistas vasculares, utilizando os aparelhos de ultra-som HDI 5000-ATL e o portátil Sonosite. Resultados: Foram examinadas 834 pessoas no período de dezembro de 2002 a junho de 2003. Dessas, 21 eram portadoras de aneurisma da aorta abdominal (prevalência de 2,5%). Dos casos, 15 (71,4%) eram do sexo masculino, 14 (66,7%) tinham idade entre 65 a 75 anos, 13 (61,9%) eram brancas, 14 (66,7%) eram fumantes ou ex-fumantes, 12 (57,1%) tinham hipertensão arterial. Dos aneurismas encontrados, 20 (95,2%) eram infra-renais, do tipo fusiforme e com diâmetro médio de 3,43 ± 0,57 cm. Quando comparamos os casos com os demais pacientes da amostra, encontramos associação estatisticamente significante com sexo (χ 2 = 13,401; P = 0,000), idade (χ 2 = 11,39; P = 0,036) e tabagismo (χ 2 = 13,984; P = 0,001). Conclusões: A prevalência de aneurisma da aorta abdominal encontrada na população de Vitória (ES) foi de 2,5%. Os autores incentivam o rastreamento do aneurisma da aorta abdominal em grupos considerados de risco: homens acima de 65 anos com história de tabagismo.
