Independentemente do tipo de psoríase, até 79% dos doentes com esta patologia apresentam atingime... more Independentemente do tipo de psoríase, até 79% dos doentes com esta patologia apresentam atingimento do couro cabeludo, sendo este frequentemente o primeiro local a mostrar sintomas da doença. Para além do prurido, as lesões avermelhadas e escamosas são visíveis e muitas vezes embaraçosas. A terapêutica tópica é geralmente a primeira opção para estes doentes, no entanto a ampla gama de tratamentos disponíveis pode muitas vezes dificultar a escolha e levar a um tratamento potencialmente desadequado. O objetivo desta revisão foi avaliar a eficácia e a segurança dos tratamentos tópicos para a psoríase do couro cabeludo. Foi efetuada uma revisão sistemática de acordo com a metodologia preconizada pela Cochrane, com vista à avaliação da redução da gravidade clínica da psoríase, melhoria da qualidade de vida e efeitos adversos que motivassem a suspensão do tratamento. Para tal, foram incluídos 59 estudos, com um total de 11 561 participantes, tendo sido efetuadas 15 comparações entre os vários fármacos e veículos de aplicação testados. Os resultados obtidos sugerem que os corticosteroides de potência elevada ou muito elevada são mais eficazes que a vitamina D. A combinação de um corticosteroide com a vitamina D tem apenas um benefício marginal em relação à monoterapia com corticosteroide, mas é superior à vitamina D isolada. Dado o perfil de segurança semelhante e o benefício apenas marginal da combinação corticosteroide com vitamina D sobre o corticosteroide isolado, a monoterapia com corticosteroide tópico parece ser aceitável para a terapia a curto prazo da psoríase do couro cabeludo. In addition to being itchy, the red and scaly lesions are usually easy to see, and may be embarrassing. Topical therapy is usually the first line of treatment; however the wide array of available interventions can make the choice difficult, and may even lead to an inadequate treatment. The objective of this review was to evaluate the efficacy and safety of topical treatments for scalp psoriasis. A systematic review was performed according to the methodology recommended by Cochrane in order to evaluate the clinical severity of psoriasis, quality of life, and adverse events that led to treatment discontinuation. To evaluate this, 59 studies were included, with a total of 11 561 participants, and 15 comparisons were made between the various drugs and application vehicles tested. The principal conclusion is that corticosteroids of high or very high potency are more effective than vitamin D. The combination of a corticosteroid with vitamin D has only a marginal benefit over corticosteroid monotherapy, but is superior to vitamin D alone. Given the similar safety profile and only marginal benefit of the combination of corticosteroid with vitamin D over the corticosteroid alone, topical corticosteroid monotherapy appears to be fully acceptable for short-term therapy of scalp psoriasis.

The goals of this study were to determine the number of C. trachomatis infections per year, characterize the demographic traits of the infected population and identify the associated sexually transmitted infections (STI), over the last 11 years, in Lisbon main Hospital Centre. Materials and methods: Laboratorial confirmed C. trachomatis infections diagnosed between January 1st, 2009 and December 31st, 2019 were evaluated. Results: A total of 6404 samples were collected, with 602 being positive for C. trachomatis, corresponding to 550 patients. 60% of diagnoses were made by dermatovenereologists and most cases occurred in males (n = 371; 67.5%), with a median age of 26.1 years (standard deviation 7.98), ranging from 13 to 68 years. More than half the cases (51.3%) occurred below the age of 25 years. In 87 patients (15.8%) there was previous history of STI, in 198 patients (36%) a concomitant STI was present and 43 patients (7.8%) developed a posterior STI. Concerning follow-up, 196 patients (corresponding to 35.6% of total patients) did not show for a re-evaluation after the diagnosis was made. Conclusion: Confirmed cases of C. trachomatis infection have risen along the years, affecting mainly people younger than 25 years. Although traditionally considered treatable, the emergence of Neisseria gonorrhoeae resistance to antimicrobials is currently a serious problem. The goal of this study was to evaluate the incidence and trends of antimicrobial resistance over the last 10 years in N. gonorrhoeae isolates from a Portuguese Centre. Laboratorial confirmed N. gonorrhoeae infections diagnosed between 2009 and 2018 were evaluated. Susceptibilities to penicillin, tetracycline, ciprofloxacin, azithromycin and cefotaxime were studied, along with demographic and clinical characteristics. From 2009 to 2018, 440 cases of N. gonorrhoeae infection were diagnosed in our center, with a significant yearly increase (p<0.05). In the last two decades, 308-nm excimer laser has been increasingly recognized as a therapeutic a... more In the last two decades, 308-nm excimer laser has been increasingly recognized as a therapeutic alternative for several dermatological conditions, being currently FDA approved for the treatment of localized vitiligo and moderately severe localized psoriasis unresponsive to topical treatments. We describe the case of a 17-year-old with a recalcitrant form of dermatitis occupying the entire perioral region, previously unresponsive to several treatments, who was treated with 308-nm excimer laser with an excellent result. 308-nm monochromatic excimer laser has several advantages over other types of phototherapy, including lower UV dose exposure, shorter courses of therapy and a better sparing of adjacent tissue. Although infrequently used, 308-nm excimer laser certainly has a vast potential in Dermatology, particularly regarding recalcitrant and localized inflammatory conditions, such as the one we present.

Background: Cutaneous leiomyosarcoma is a rare malignant neoplasm with muscular origin, represent... more Background: Cutaneous leiomyosarcoma is a rare malignant neoplasm with muscular origin, representing 2%-3% of all cutaneous soft tissue sarcomas. Objectives: The aim of this study was to characterize clinicopathological features of patients diagnosed with cutaneous leiomyosarcoma in our center over the last 20-years. Methods: A retrospective study of patients with a histopathological diagnosis of leiomyosarcoma between 1999 and 2018 was conducted. Results: Eleven patients were diagnosed with cutaneous leiomyosarcoma during this period. Most cases occurred in men (n = 7). Age at presentation ranged from 47 to 92 years (mean 64.9 years). Head and neck were the most frequently involved locations (n = 5). Ten leiomyosarcoma were dermal, with one cutaneous metastasis. Immunohistochemical staining was available for 7 patients, demonstrating positivity for smooth muscle actin in all of them. All neoplasms were treated surgically. Mean survival was 32.2-months. Study limitations: This was a retrospective study based on medical and pathological records. Conclusions: Histopathology is essential for the diagnosis of leiomyosarcoma, usually revealing a dermal or subcutaneous lesion composed of intertwined fascicles of smooth muscle fibers. The allergen responsible for most of these cases is isobornyl acrylate, a substance present within the sensor that migrates through the adhesive, thereby reaching the skin. Acquired leukoderma, which may occur in an area previously affected by allergic contact dermatitis, has been described in several medical devices with adhesives. However, until the present, only one case of leukoderma induced by allergic contact dermatitis to FreeStyle Libre® has been described. We report the case of a 41-year-old woman with diabetes mellitus type 1, who developed leukoderma in association with allergic contact dermatitis to this glucose sensor.

(1... more Diabetes mellitus (DM) is a chronic condition the prevalence of which is increasing in Europe. (1) Continuous glucose monitoring systems, such as FreeStyle Libre (Abbott Diabetes Care, UK) have improved DM management. (2) This device is applied to the skin through an adhesive and remains in place for up to 14 days, dynamically measuring interstitial glucose levels. (3) Several cutaneous side effects have been recognized since its introduciton, such as infections, hematomas and irritant or allergic contact dermatitis (ACD). (4,5) Regarding ACD, the most commonly identified culprit allergen is isobornyl acrylate (IBOA; CAS no. 5888-33-5). This study describes cases of contact dermatitis caused by glucose sensors in a tertiary care Portuguese Hospital. Methods Overall, 10 patients with cutaneous reaction to the glucose sensor FreeStyle Libre were included in the study, along with 20 controls. All patients with cutaneous reactions were using 1 st generation Freestyle Libre, the only reimbursed glucose monitoring device available in Portugal. Patients were tested with the Portuguese baseline series, an acrylates and plastic and glues series (Chemotechnique Diagnostics, Vellinge, Sweden), and with IBOA, purchased from Sigma

Over the last few decades, behavioral changes in sexual practices have made oral transmission of ... more Over the last few decades, behavioral changes in sexual practices have made oral transmission of traditional sexually transmissible infections increasingly recognized. Patients harboring a sexually transmissible infection may first present lesions on the oral cavity, as these may be visible and interfere with basic functions such as speech or swallowing. Moreover, the oral cavity may function as a reservoir for future spread of these infections. In order to successfully control this problem, a greater focus on oral sex should be persued, along with promotion of the use of condom and education on safe oral sex practices. Furthermore, examination of the oral cavity should is essential when evaluating any patient suspected of harboring a sexually transmissible infection. In this article, oral transmission of several viral and bacterial infections is reviewed, including human papillomavirus infection, genital herpes, syphilis and gonorrhea, among others.

Gonorrhea is the second most notified sexually transmitted infection in Europe, and its incidence has been increasing in the last years. Although traditionally considered a treatable infection, antimicrobial resistance of Neisseria gonorrhoeaeincludes at present also macrolides, tetracyclines, sulfonamides and trimethoprim combinations, quinolones, and even cephalosporins. These high levels of gonococcal resistance to antimicrobials resulting in untreatable infections in the future may become one of the greatest challenges to the prevention and control of sexually transmitted infections, which may be a significant major public health issue. Excessive solar exposure presents well-recognized risks and side effects, solar radiation being t... more Excessive solar exposure presents well-recognized risks and side effects, solar radiation being the most important environmental factor concerning skin cancer. In the last few years, several connections between solar exposure and prevention and/or treatment of several diseases have been discussed, with studies suggesting that regular solar exposure may be beneficial for conditions such as colorectal, breast, prostate, and pancreatic cancer; non-Hodgkin lymphoma; arterial hypertension; obesity; type 2 diabetes mellitus and metabolic syndrome; nonalcoholic hepatic steatosis; multiple sclerosis; Alzheimer disease; and several psychiatric disturbances. In most cases, UV radiation’s beneficial effects are mediated through vitamin D; however, studies show that in other instances other mediators are responsible for these associations, specifically nitric oxide. Moderation is therefore essential, as a strict strategy of total sun avoidance may be inadequate.
