La pancreatitis aguda es un proceso inflamatorio con repercusiones sistémicas y locales; la mayor... more La pancreatitis aguda es un proceso inflamatorio con repercusiones sistémicas y locales; la mayoría de los casos son leves con baja tasa de mortalidad, pero el 20% de los pacientes sufren pancreatitis grave cuya tasa de mortalidad puede llegar a ser hasta de un 30%. A lo largo de los años se ha intentado llegar a consensos acerca de esta enfermedad con el fin de orientar a la comunidad médica hacia su mejor entendimiento, clasificación y tratamiento. El más importante de estos ha sido conocido como el Consenso de Atlanta de 1992, vigente por muchos años, pero que está siendo objeto de diferentes propuestas de modificación y actualización, que se discuten en este artículo de revisión.

Using general results on t t -structures, we give a new proof of Rickard’s theorem on equivalence of bounded derived categories of modules. Also, we extend this result to bounded derived categories of quasi-coherent sheaves on separated divisorial schemes obtaining, in particular, Beĭlinson’s equivalences.

El hongo Ganoderma lucidum, en los constituyentes de su biomasa, tiene compuestos con propiedades... more El hongo Ganoderma lucidum, en los constituyentes de su biomasa, tiene compuestos con propiedades benéficas para la salud; es por esto que el conocimiento de las condiciones nutricionales adecuadas para su crecimiento permitirá su producción industrial y a bajo costo. En este trabajo se evaluó a nivel de matraz el efecto de la relación C/N, y la presencia de diferentes fuentes de carbono, nitrógeno y micronutrientes sobre la producción de biomasa. Empleando glucosa y peptona como fuentes de carbono y nitrógeno, respectivamente, se encontró una relación C/N óptima de 16,7:1 para la cual la máxima producción de biomasa fue de 25 g/L. Manteniendo esta relación C/N, y sustituyendo la glucosa por lactosa o harina de cebada y la peptona por extracto de levadura, la producción de biomasa se incrementó a 35 g/L. En presencia de harina de cebada la adición al medio de cultivo de sales de Mg y K, y de tiamina, no generó un mayor incremento en la producción de biomasa. La producción de biomasa de G. lucidum se ve favorecida por la presencia en el medio de cultivo de relaciones C/N cercanas a las reportadas conforme a la composición típica de los hongos, así como por la presencia de sustratos complejos como la harina de cebada que le aportan además de la fuente de carbono micronutrientes necesarios para su desarrollo. Most cases are mild with a low mortality rate, but 20% of patients have severe pancreatitis, with a mortality rate up to 30%. Throughout the years, the medical community has tried to reach a consensus about this disease to better understand, classify and treat it. The most important consensus is known as the Atlanta Consensus of 1992, which has been in use for many years. However, it has recently been the subject of various proposals for change and updating, which are discussed in this review.

We estimated the nationwide prevalence rate of antidepressant drug treatment in Parkinson&amp... more We estimated the nationwide prevalence rate of antidepressant drug treatment in Parkinson's disease (PD) patients. Very few studies exist on the frequency of antidepressant drug treatment in patients with PD. Patients with a PD diagnosis at first hospital contact were identified and followed for up to 6 years. The subsequent probability of antidepressant drug treatment was estimated and compared to a control group of patients with osteoarthritis. The probability of antidepressant drug treatment was 3.98 [95% CI: 3.23-4.91) times higher for PD patients than for controls. Patients with PD have higher rates of actual antidepressant drug treatment than a control group with osteoarthritis. Still, however, undertreatment of depressive states may be the case.

Backround: Coronary artery disease (CAD) is commonly cited as a mechanism underlying diastolic dy... more Backround: Coronary artery disease (CAD) is commonly cited as a mechanism underlying diastolic dysfunction. However, the association of CAD with left ventricular (LV) diastolic dysfunction has not been convincingly demonstrated in asymptomatic patients. The objective of this study was to determine if such a relation exists using coronary artery calcium score (CACS) as a surrogate for coronary atherosclerosis burden. Methods: Individuals ≤ 65 years of age who underwent CACS and echocardiography between September 2006 and September 2007 were included in this retrospective study. Clinical variables were collected from the medical record. Diastolic function classification was based on established echocardiographic criteria previously used in large epidemiologic studies to predict future adverse CV events. Statistical analysis was used to identify predictors of CACS. Results: There were 358 subjects (306 males) aged 58 ± 6 years. Risk factors included hyperlipidemia in 206 (58%), hypertension in 131 (37%), impaired fasting glucose in 78 (22%) and diabetes in 21 (6%). Although left atrial (LA) volume index weakly correlated with CACS (r = 0.24, p < 0.001), there was no relation between CACS and LV diastolic function grade (p = 0.554). After stepwise elimination multivariate analysis, 5 variables independently predicted CACS: age (p < 0.001), hyperlipidemia (p < 0.001), LV posterior wall thickness (p = 0.006), gender (p = 0.024) and LA volume index, (p = 0.029). Conclusions: In asymptomatic patients, CACS does not correlate with LV diastolic function as defined by established echo-Doppler criteria. In the absence of ischemia, postinfarction LV remodeling, or previous coronary artery bypass surgery, CAD does not appear be a cause of LV diastolic dysfunction or diastolic heart failure in asymptomatic patients.

Using the unique Danish psychiatric and somatic health registers, we investigated the rate of sub... more Using the unique Danish psychiatric and somatic health registers, we investigated the rate of subsequent dementia in patients with late-onset acute and transient psychosis. By linkage of the psychiatric and the somatic nationwide registers of all patients with in- or outpatient hospital contact in Denmark, we included all patients with a first ever contact during the period 1 January 1994 to 31 December 2001 with one of the main index diagnoses: late-onset acute and transient psychosis or osteoarthritis. Data on the general population were also included. The first diagnosis of dementia for each individual at discharge or at outpatient contact was established. Poisson regression models were used to compare the cohorts of patients with dementia as the outcome of interest. Using a cut-off age of 60 years, 8062 individuals were included. Significant associations were found between a subsequent diagnosis of dementia and the index diagnosis, age and calendar time. Overall, the rate ratio for developing dementia in late-onset acute and transient psychosis compared to osteoarthritis patients was 10.86 (95% confidence intervals, 8.42 and 14.00, respectively), however, the magnitude of the rate ratio varied according to sex, age, duration since diagnosis and calendar time. Compared to the general population, the rate ratio was 8.12 (95% confidence intervals, 6.77 and 9.74, respectively). The present study has established that subjects with late-onset acute and transient psychosis are at 11 times higher risk of subsequently getting a diagnosis of dementia compared to patients with osteoarthritis, and at 8 times higher risk compared to the general population.

Bursera glabrifolia, a dominant component of the tropical dry forests in southwestern Mexico, is ... more Bursera glabrifolia, a dominant component of the tropical dry forests in southwestern Mexico, is used to illustrate the importance of a broader contextual framework for ethnobotanical studies. The species is currently used as a carving wood to produce small, painted figurines known as alebrijes. Over the past hundred years, however, B. glabrifolia also has been exploited commercially for resin and essential oil, and over-harvesting has pushed the species to the brink of local extinction several times. The destruction of tropical dry forests threatens the ability of the species to recover from the current wave of exploitation. Eight years of study have highlighted two major aspects of the life and times of B. glabrifolia. First, the ethnobotany of the species is extremely complex and, in addition to local artisans and collectors, involves pre-Columbian tributes, resource substitution, NAFTA duty exemptions, and the spread of cattle pastures. Second, a concerted effort to manage the species in its natural habitat would bolster the thriving alebrije market and provide a much-needed incentive for conserving tropical dry forests in Mexico. Vida y tiempos de Bursera glabrifolia (H.B.K.) Engl. en México: Un a parábola para la etnobotánica. Bursera glabrifolia, un componente dominante de los bosques secos tropicales del sureste de México, se utiliza aquí para ilustrar la impotancia de adoptar un marco conceptual amplio en los estudios etnobotánicos. La madera de esta especie se usa actualmente en la zona de Valles Centrales de Oaxaca para elaborar figuras talladas conocidas como alebrijes. Sin embargo, durante los últimos cien años, B. glabrifolia ha sido también explotada por su resina y su aceite essecial, y la sobrexplotación la ha colocado cerca de la extinctión local. La destrucción de los bosques secos tropicales pone en riesgo la capacidad de la especie para recuperarse al ritmo actual de explotación. Ocho años de investigación interdisciplinaria nos han permitado reconocer dos aspectos importantes de la historia de B. glabrifolia. Primero, que la etnobotánica de la especie es extremadamente compleja, ya que además del uso actual por artesanos e colectores locales, involucra tributos prehispánicos, sustitución de especies, exención de impuestos en el marco del TLC y la expansion de la ganadería. Y Segundo, que un esfuerzo concertado por manejar esta especie en su hábitat natural, le daría un impulse al mercado de los alebrijes y significaría un incentivo, muy necesario, para la conservación de los bosques tropicales secos en México.

The aim of the study was to identify factors associated with the level of social well-being for c... more The aim of the study was to identify factors associated with the level of social well-being for cochlear implanted children and to estimate effect-related odds ratios for the children's well-being. Another aim was to analyse associations between speech and language level and the level of social well-being. Data relate to 167 children with cochlear implants. In structural interviews, parents rated their children's level of social well-being regarding the degree of their child's personal-social adjustment. Five different factors were considered. Logistic regression models and proportional odds models were used to analyse the relationship between the considered factors and the assessments. The analyses showed that the communication mode at home was the most highly associated factor. A statistically signifi cant association was found between the level of social well-being and speech understanding, speech production and vocabulary. Children who were exposed to a spoken language had considerably better odds of having a high level of social well-being compared to children with a mixture of spoken language and sign support or sign language. By linkage of the psychiatric and the somatic nationwide registers of all out- and in-patients with hospital contact in Denmark, we included all 60+ patients with first ever from 1 January 1994 to 31 December 2001 with the index main diagnosis: delusional disorder. First contact osteoarthritis patients as well as the general population were used as controls. A total of 1,437 patients with persistent delusional disorder and 7,302 patients with osteoarthritis were included. Median follow-up time until first diagnosis of dementia at discharge was 1.87 and 4.40 years, respectively. The probability of getting a dementia diagnosis was estimated using Poisson regression models with dementia as the outcome of interest. Patients with very late first-contact delusional disorder had an 8.14 (95% CI, 6.51; 10.19) times increased rate of subsequently developing dementia compared with very late first contact osteoarthritis patients. Compared with the general population the rate ratio was 5.49 (95% CI, 4.81; 6.26). Very late first-contact delusional disorder increases the risk of subsequently getting a diagnosis of dementia 5-8 times compared with osteoarthritis patients and the general population.

The aim of the study was to determine the type and magnitude of detectable changes in pig multifo... more The aim of the study was to determine the type and magnitude of detectable changes in pig multifocal electroretinography (mfERG) induced by the vitreoretinal surgical procedures necessary to gain access to the subretinal space. Methods: Twenty pigs underwent posterior segment surgery. Six animals had a vitrectomy (V), six had in addition a retinal bleb detachment (V + B); five had in addition a retinal diathermia on the bleb (V + B + D) and three received a retinotomy in the diathermized retinal area (V + B + D + R). mfERG evaluation was performed at baseline and 1 and 6 weeks postoperatively. Selected eyes were enucleated for histological evaluation. Results: The retinal detachments blebs all reattached spontaneously. All four surgical sequences resulted in slight, non-significant changes in the mfERG peaks. A trend towards an amplitude reduction of the mfERG peaks N1, P1 and N2 were observed within the first postoperative week. After 6 weeks, all amplitudes had normalized. Of the implicit times only that of peak N1 (after retinal diathermia) was prolonged significantly at 1 week (P ¼ 0.037). However, it returned to the preoperative level after 6 weeks. Histologically, the retinal detachment bleb was characterized by transient double layering of the retinal pigment epithelium (RPE) and loss photoreceptor outer segments. Conclusion: Access to the subretinal space in pigs can be gained without permanent detectable changes in the mfERG. A short-term retinal detachment was found to cause only reversible electrophysiological and histological changes in the outer retina, which suggests that this procedure is tolerated well in the porcine retina. The size of the known destructive lesion (retinotomy) was too small to be detected, given the spatial resolution of the mfERG method applied. In the future, the presented protocol can be used to assess the functional outcome of surgery and transplantation in the subretinal space in pigs.

To establish a method allowing multifocal electroretinography (mfERG) recording with simultaneous... more To establish a method allowing multifocal electroretinography (mfERG) recording with simultaneous fundus monitoring on anaesthetized pigs. In addition we characterize the peaks of the porcine mfERG trace, and compare the visual streak area with the optic nerve head, a known nonresponse area. Finally we illustrate the feasibility of the method by performing mfERG after an induced laser burn in the visual streak. Methods: Fifteen pigs underwent mfERG recordings at baseline, and after 1 and 6 weeks of observation. One pig was evaluated before and after retinal diode laser treatment in the visual streak. Results: The porcine mfERG trace appears similar to the human mfERG trace, and can be described by three peaks named N1, P1 and N2. Significantly faster implicit time was found in the visual streak regarding N1 (P < 0.001) than in areas outside the visual streak. Amplitudes of all three peaks were increased in the visual streak (P < 0.005). The laser-treated area was characterized by a response similar to what is found at the location of the optic nerve head. Conclusion: Porcine mfERG is similar in appearance to the human response and can be described by the same three peaks. Significantly higher amplitudes of all three peaks are found in the visual streak when compared to the optic nerve head and inferior retina. We have detected the functional deficit caused by a laser burn at the size of 3 • 3 mm.
