A 45 year old man with malignant pleural mesothelioma had symptoms of prolonged cough, heamoptysi... more A 45 year old man with malignant pleural mesothelioma had symptoms of prolonged cough, heamoptysis, breathlessness, weight loss and low grade persistent fever. The high prevalence of pulmonary tuberculosis in this environment, coupled with paucity of radiologists in rural areas to competently review his chest radiograph, resulted in making the wrong diagnosis of pulmonary tuberculosis (PTB) as against malignant pleural mesothelioma. He was also wrongly treated for PTB for a period of nine months before referral to a Teaching Hospital. The correct diagnosis was made in a university teaching hospital where adequate specialized manpower was available. Distant metastasis to the liver was already observed at presentation. He was treated with six courses of chemotherapy (Peclitaxel and Cisplastin) and he did well before he was lost to follow-up. Key words: Malignant pleural mesothelioma, PTB, liver metastasis.

Infantile fibrosarcoma is a rare type of soft tissue sarcoma seen in children usually less than 2... more Infantile fibrosarcoma is a rare type of soft tissue sarcoma seen in children usually less than 2years of age. Few cases of giant infantile fibrosarcoma have been reported in literature. We report a rare case of a giant multi-nodular infantile fibrosarcoma in the left anterolateral chest wall in a 7-year-old boy. The tumour was said to have been recurrent twice for about 6 years. At presentation patient was evaluated and was commenced on 6 cycles of neoadjuvant chemotherapy with vincristine, adriamycin and cyclophosphamide (VAC) regimen to alternate with Ifosfamide/Etoposide (IE) regimen with very good response. Thereafter, had a wide local excision of the tumour and then had 4 more cycles of adjuvant chemotherapy. His 6months follow up showed no evidence of tumour recurrence. Infantile fibrosarcoma is said to be chemo-sensitive tumour with very good response, though surgical excision is the main treatment of choice and overall it is said to have a good prognosis.

BIO Ventures for Global Health (BVGH) launched the African Access Initiative (AAI) to address Africa’s cancer crisis by establishing sustainable access to cancer medicines. Through AAI, BVGH and the Ahmadu Bello University Teaching Hospital (ABUTH) are piloting a first-of-its-kind program focused on improving Nigeria’s access to affordable, gold-standard cancer drugs manufactured by multinational pharmaceutical companies. METHODS BVGH, the Federal Ministry of Health, and Nigerian oncologists held a stakeholder meeting to prioritize cancers and their associated drugs. The cultural habit of application of concoctions made from mud and animal dung to chronic and necrotic wounds in our environment may provide a portal of entry for serious tetanus infection. There is a paucity of published data of tetanus infection arising from a necrotic breast tumor as a possible portal of entry in study area necessitating our report on a 48-year-old woman with extensive and fungating invasive ductal carcinoma of the breast who acquired fatal tetanus infection presumably from application of animal dung to the necrotic chest wall tumor. Patient and caregiver's education, clinical awareness, timely treatment, and proper tetanus vaccination in predisposed patients before surgical excision, debridement, or chemotherapy might be necessary to prevent this catastrophe.

There is disproportionate prevalence and poor understanding of CaP in Black men globally. The Prostate Cancer Transatlantic Consortium (CaPTC) has documented significant CaP burden among Nigerian black men in Nigeria and in the Diaspora. Notably, African ancestry is one of the most important risk factors of CaP globally. Increased levels of cortisol, a stress-related catabolic hormone is significantly associated to prostate specific antigen and poor prognosis in prostate cancer patients. Cortisol causes degradation of muscle proteins into amino-acids. Amino acids such as leucine, serves as a source of fuel for aggressive prostate cancer cells in a Warburg effect model through the L-amino acid transporters (LATs). The aim of this study was to assess whether introduction of Docetaxel into the management will im... more The aim of this study was to assess whether introduction of Docetaxel into the management will improve palliation of symptoms, quality of life, response rate and probably survival in breast cancer patients. A total of 19 patients referred from different medical centers in Nigeria were entered into the study. The age range was 25 to 52 years with a median of 34 years. The sex incidence was 18 females, 1 male. There were 7 patients in stage III and the duration of median response was 30 months. There were 12 patients in stage IV. Out of these 3 patients with brain metastasis had median response of 4 months. 4 patients with liver metastasis average duration of response was 22 months. 5 patients with pulmonary metastasis average duration of response was 20 months. In all, 16, out of the 19 patients (84%) had median response duration of 20 30 months. Better response rate could have been in patients with cerebral metastasis but for the fact that Docetaxel does not penetrate brain barrier. Multicentre studies are now needed to reconfirm the superiority of Doxetacel over existing cytotoxic chemotherapeutic drugs currently in use in the management of advanced and metastatic breast cancer in Nigeria.

Patients often present late with unsightly fungating tumors resulting in difficult management and poor outcome. The aim of the study was to determine the clinical profile, diagnostic work-up, treatment and histo-pathological types of orbito-ocular tumors seen at Ahmadu Bello University Teaching Hospital, Shika-Zaria from June 2006 to June 2011. Materials and Methods: A retrospective analysis of all patients who presented to the eye clinic with orbito-ocular tumors during the study period was made. Information extracted incwudes patient demographics, clinical diagnoses, radiological and sonological reports, surgery performed, and histology reports as well as adjuvant chemotherapy and radiotherapy where indicated. Results: A total of 54 patients were managed during the study period. Those without histology reports were excluded from the study. Forty-nine patients were included for analysis. There were 29 male and 20 female patients giving a male to female ratio of 1.8:1. Their age range was 1.5-74 years with a mean of 22 years. Eighteen of the patients had retinoblastoma, eleven had squamous cell carcinoma, two had Kaposi' sarcoma, and one case of nasopharyngeal carcinoma. There were 2 patients with pre-malignant conditions and 10 patients with benign lesions. Thirty-two of the patients had exenterations, sixteen had excisional biopsies, and one had enucleation. Seventy eight percent of the tumors were malignant. Conclusion: Most of the orbito-ocular tumors seen in this study were malignant. Delayed hospital presentation is a characteristic feature. Retinoblastoma and squamous cell carcinoma are the most common orbito-ocular tumors.

Studies have shown that increased intake of dietary fiber which contribute to low fecal pH and also influences the activity of intestinal microbiota, is associated with a lowered risk for CRC. However, whether or not the apparent high dietary fiber consumption by Africans do not longer protects against CRC risk is unknown. This study evaluated dietary fiber intake, fecal fiber components and pH levels in CRC patients. Thirty-five subjects (CRC=21, control=14), mean age 45 years were recruited for the study. This study aimed to understand Nigeria's current RT capacity and the impact of PPPs on RT availability and cost. This study was conducted to determine if prostate cancer status, demographic and health factors a... more This study was conducted to determine if prostate cancer status, demographic and health factors as well as social support, illness perception, financial toxicity and resilience predict the quality of life (QoL) in men and to ascertain the differences between men with and without cancer of the prostate (CaP) in the QoL subscales and significant predictors of QoL. Quality of life, social support, illness perception, financial toxicity and resilience were assessed using the Functional Assessment of Cancer Therapy-Prostate (FACT-P), the Multidimensional Scale of Perceived Social Support (MSPSS), Brief Illness Perception Questionnaire (BriefIPQ), The COmprehensive Score for financial Toxicity (Cost Facit) and the Brief Resilience Scale (BRS) respectively. Each scale was assessed and found to possess acceptable levels of reliability (Alpha =.551 to .899). Demographic and clinical information were also collected include age, marital status, religion, education, income, ethnic group, health insurance, employment status and illnesses respondents are being treated for. Additional data collected from the cancer patients were the cancer stage and time since diagnosis. Total population sampling and opportunistic sampling of all willing men with and without CaP respectively were used in recruiting study participants. T-test, Chi-square, multiple regression and multivariate analyses were used for data analysis. Study participants were 237 men, comprising 139 men with CaP and 98 men without CaP with an age range of 53-81 and mean of 66.5. Majority of the respondents were married, Christians, of the Yoruba ethnic group and without health insurance. The two most common illnesses reported were High Blood Pressure (27.42%) and malaria (21.94%). The cancer stage for 71.2% of the men with CaP was not indicated in their case notes. Majority of the men with prostate cancer were within 1 to 5 years since diagnosis (46.8%). In the present study, a scanning electron microscope (SEM), transmission electron microscope (TEM), and a confocal laser scanning microscope (CLSM) were utilized to investigate the mode of actions of ginger EOs on the cell structure of Xoo. Its clinical spectrum ranges from a benign and locally recurrent form of behaviour to malignant and metastatic forms. The age at risk is between 35 and 45 years. Giant phyllodes tumours are larger than 10cm in diameter. Case Report: Herewith, we present a rare case of giant malignant phyllodes tumour (40x30x20cm) in a young lady who survived 10 years post-treatment in which most literature reported 10-year survival of 23% to 42%. The patient had mastectomy and radiotherapy to the anterior chest wall. Conclusion: A young lady with a giant malignant phyllodes tumour survived 10 years following treatment.

The objective of this study is to analyze the trend of malignant tumors in a population of rural Africans attending Ahmadu Bello University Teaching Hospital Zaria, Nigeria with particular emphasis on the first ten most frequently diagnosed cancers over a period of 14 consecutive years (2001-2014). Histologically diagnosed cancers in the various cancer registries of the ABUTH Zaria were included in this study. The ten most commonly presenting cancers were extracted and their demographic characteristics and other relevant clinical data documented using a standardized questionnaire. The data obtained were analyzed using SPSS software version 20.0. There were 6,553 histologically diagnosed cancers over 14 years in Ahmadu Bello Unibersity Teaching Hospital Zaria with the following in descending order: Cervical cancer 1042 (15.90%), Breast cancer 788 (12.02%), Prostate cancer 496 (7.56%), Colorectal cancer 360 (5.49%), Lymphoma 306 (4.66%), Eye/orbit cancer 170 (2.59%), Bladder cancer 169 (2.45%), Nasopharyngeal cancer 139 (2.12%), Ovarian cancers 94 (1.43%), and Liver cancer 52 (0.79%). Cervical cancer tops the list as the most frequently diagnosed cancer however, this should be noted that it is a cancer exclusive to females so also breast cancers though rarely males may present with breast cancer. Prostate cancer is a male exclusive disease therefore the percentage would have been higher if the diagnosis would have excluded the females. Of the ten most frequently diagnosed cancers, liver cancer is the tenth despite the fact that complications of viral hepatitidis are known to facilitate liver carcinogenesis and the prevalence of hepatitis C and B in the Nigerian population is 5 and 20 respectively. Male breast cancer is an uncommon disease accounting for only 1% of all breast cancers. We presen... more Male breast cancer is an uncommon disease accounting for only 1% of all breast cancers. We present the evaluation, treatment and outcome of male patients seen with breast cancer in our institution. Male patients that had histological diagnosis of breast cancer from 2001 to 2010 were retrospectively evaluated. After evaluation patients were treated with modified radical mastectomy. Combination chemotherapy was given to patients with positive axillary lymph nodes. Radiotherapy and hormonal therapy were also employed. There were 57 male patients with breast cancer which accounted for 9% of all breast cancers seen during the study period. Their mean age was 59 ± 2.3 years. The mean tumor diameter was 13 ± 2.5 cm. Fifty three (93%) patients presented with advanced disease including 15 with distant metastasis.
