Background: In children, toothbrushing with fluoride toothpaste is the method of choice for prev... more Background: In children, toothbrushing with fluoride toothpaste is the method of choice for preventing dental caries and it is influenced mainly by their mothers.Objective: To provide descriptive information about toothpaste utilization profiles of 6 to 8-year-old Nigerian children.Materials and methods: A cross-section of 130 mothers of 6-8 year-old Nigerians provided information on sociodemographic characteristics, knowledge of toothpaste use, tooth brushing habits and criteria for selecting toothpaste for their child. Their 6-8 year-old children who attended the Dental Centre of the University of Maiduguri Teaching Hospital North-Eastern Nigeria were examined for presence of dental caries and fluorosis. Data analysis included both descriptive and inferential statistics at p 0.05). Females (54.2%) and children of mothers who had no formal education (65.6%) had more fluorosis than their counterparts (p<0.05).Conclusions: Many mothers had poor knowledge score of toothpaste use. Children who had fluorosis were more than those who had dental caries. Introduction: The NCEP III-ATP guidelines provide clear clinical directives for lipid management ... more Introduction: The NCEP III-ATP guidelines provide clear clinical directives for lipid management especially statins therapy in appropriate patient groups. Compliance of primary care physicians with these guidelines especially in ambulatory care settings has been shown to be poor. The compliance of cardiologist to these guidelines is less documented. Methods: A retrospective chart review of 386 patients managed in a large urban cardiology practice was undertaken. Patients with documented contraindications to use of statins were excluded from the study. Only patients with two or more years of follow-up in the practice were included. Demographic variables and medical history including CAD or its equivalent and its major risk factors were identified. The proportion of patients on statins and adequacy of statins therapy were recorded. The lipid profiles of all patients were also analyzed. Results: Fifteen patients with documented contraindications to statins therapy including persistent/severe LFT abnormalities, allergies, and gastrointestinal intolerance were excluded. A total of 371 patients were included in the analysis. The mean age for patients in the study was 65 years (range: 42-84). 236 (64%) were males while 141 (36%) were females. 161 (43%) patients were on statins while 210 (57%) weren't. 88 (62%) of females were on stain compared to 116 (49%) of males (p = 0.001). 68% of patients below the age of 50 yrs were not on statins compared with 55% of those greater than 50 yrs (p = 0.01). 38% of patients on statins therapy had sub-optimal lipid profile despite greater than two years of therapy. No statistically significant differences in race and use of satins were noted. Conclusion: This study demonstrates a higher than expected prevalence of sub-optimal management of dyslipidemia among patients with established coronary heart disease without contraindications to statins managed by cardiologists. Cardiology and primary care practices require similar comprehensive routine lipid management program that is assiduously maintained and evaluated at both in-patient and out patient settings to ensure most patients receive optimal therapy with statins and other lipid lowering agents.

Autoimmune conditions such as Reynaud syndrome, lupus, rheumatoid arthritis, and polymyalgia are more frequent in MDS patients, and activated cytotoxic T cells and elevated levels of tumor necrosis factor alpha (TNF-a) and interferon g (IFNg) 1-4 are found in MDS. Cytokine dysregulation and excessive apoptosis are most notable in early MDS, 3-6 where up-regulation of Fas-receptor and Fas-ligand expression are hallmarks of the disease. 7-11 The concept of using immunosuppressive treatment (IST) for bone marrow (BM) failure originates severe aplastic anemia (SAA) patients. 12 Gluckman and colleagues 13 were the first to show that hematopoietic recovery could occur in SAA following antithymocyte globulin (ATG) treatment alone. As experience and success in IST for SAA accumulated, small numbers of patients with BM failure secondary tp hypoplastic MDS were treated successfully. The response of such patients led to the establishment a clinical trial of IST at the National Institutes of Health in 1994 to evaluate the ability of ATG to improve BM failure in all forms of MDS. 14 This study showed improvement in 30% of patients with MDS after ATG. Background-Hepatitis C virus (HCV) infection may be associated with thrombocytopenia and increase... more Background-Hepatitis C virus (HCV) infection may be associated with thrombocytopenia and increased iron stores in patients receiving medical care. We aimed to determine how often changes in hematologic, iron metabolic and inflammatory markers occur in individuals with undiagnosed HCV in the community. Methods-Inner-city African Americans (n=143) were recruited from the community according to reported ingestion of alcohol. They were divided broadly into those who drank more or less than 56 g alcohol/day as assessed by dietary questionnaire. HCV serology was determined and laboratory values were compared according to HCV seropositivity in analyses that adjusted for alcohol consumption. Results-The prevalence of HCV seropositivity was 23% among men and 29% among women. Levels of hepatocellular enzymes were higher with HCV seropositivity (P <0.0001) but hemoglobin concentrations, white blood cell and platelet counts and serum ferritin concentrations did not differ. The globulin fraction of the serum protein concentration (P=0.002) was increased with HCV seropositivity as expected with chronic inflammation. However, erythrocyte sedimentation rate and serum iron and haptoglobin levels did not differ significantly according to HCV status. Furthermore, multivariate analysis revealed that C-reactive protein was decreased and transferrin concentration was increased with both HCV and alcohol consumption (P<0.014). Conclusions-Previously undiagnosed HCV seropositivity has little effect on the complete blood count and body iron stores but appears to perturb the response to an inflammatory stimulus, causing reduced rather than increased circulating CRP concentrations and increased rather than decreased transferrin concentrations.

She was commenced on antiretroviral drugs: Nevirapine, Zidovudine, Lamivudine antiretroviral therapy, and co-trimoxazole but developed generalized maculopapular rash and eye lesions on the 6 th day of treatment. Toxic epidermal necrolysis was suspected. She was managed by a multi-disciplinary team of physicians and in a high dependency care setting and she recovered fully. This case is presented to highlight the possible occurrence of this condition among HIV-positive patients, the challenges of care and the importance for teamwork in such cases.

Treatment with hydroxyurea is associated with a significant decrease in sickle cell complications, hospitalizations and transfusion requirements by about 50% and mortality reduction by 40% in clinical studies. The drug is unfortunately underutilized in sickle cell disease in the United States despite clear efficacy data and management experience. There is no data on the utilization of hydroxyurea in Africa, a part of the world with the highest global burden of sickle cell disease. We prospectively interviewed 206 consecutive adults and pediatric sickle cell patients as part of the Nigerian pulmonary hypertension screening study and reviewed over 1000 patients followed longitudinally at Ahmadu Bello university teaching hospital in Zaria, Nigeria. Biventricular Cardiac Function in Sub-Saharan Africans with Sickle Cell Disease

H... more Background: Various papers on sickle cell disease (SCD) in the USA reveals cardiac dysfunction. However, the highest burden of SCD is in Sub Saharan Africa. Amidst this population, use of hydroxyurea, blood transfusion and iron overload is extremely limited, allowing for a unique assessment of biventricular function. Method: We evaluated 208 hydroxyurea naive consecutive SCD patients aged 10-52 years at steady state and 94 healthy non-matched controls who were studied in Nigeria in a cross-sectional manner. SCD patients were required to have electrophoretic and or liquid chromatography documentation of major sickling phenotypes. Control group was required to have non-sickling phenotype. Cardiac measurements were performed with transthoracic echo (TTE) for both systolic and diastolic functions according to American Society of Echocardiography guidelines. Results: Patients with SCD had significantly higher mean±SD values for tricuspid regurgitant jet velocity than did the controls (2.1±0.6 vs. 1.8±0.5; p<0.001). 25% of SCD patients had elevated systolic pulmonary artery pressure (PAP) as defined by jet velocity ≥2.5 m/s(estimated systolic PAP ≥30 mm Hg) compared to 7% of controls (P<0.001). 4% had jet velocities ≥3.0 (estimated systolic PAP≥41) compared to 0% controls.Compared to controls, SCD patients had significantly higher values of left ventricular (LV) size; there was no qualitative evidence of systolic dysfunction, ejection fraction (EF) ≤0.5). The patients had significant higher values of EF and E/A ratio. In SCD group, there was no clear pattern of worsening diastolic function with increased Tricuspid Regugitant Volume. Also, E/A had a significant positive relationship with jet velocity in bivariate analysis (R=0.20; P=0.013). Conclusions: Overall, this shows that prevalence of pulmonary hypertension in Nigerian patients is independent of concomitant LV systolic or diastolic dysfunction. African SCD patients provide a unique population for evaluation of LV dysfunction from an epidemiological and clinical perspective. We established that while pulmonary hypertension is prevalent in this population the biventricular functions are preserved in contrast to patients in the USA.

We sought to determine the magnitude and spatio-temporal pattern of maternal morbidity in Kano St... more We sought to determine the magnitude and spatio-temporal pattern of maternal morbidity in Kano State, Nigeria. Analysis was on deliveries within the period 1990-1999. We counted 59772 maternal complications among 171621 deliveries, yielding an overall maternal morbidity ratio of 34.8%. We observed yearly variations and increment in the number of complications among mothers (p=0.04). A total of 4154 maternal deaths were registered within the period. The level of maternal deaths correlated significantly with burden of morbidity (r=0.44; p=0.0008). Case fatality rate was 6.9% with temporal fluctuations, and wide variations among the various health institutions. Our study revealed an extremely high morbidity ratio, which indicates that a high proportion of pregnant mothers are in need of essential obstetric care in this population.

Laboratory evidence and clinical evidence suggest that some patients with myelodysplastic syndrom... more Laboratory evidence and clinical evidence suggest that some patients with myelodysplastic syndrome (MDS) have immunologically mediated disease. Autoimmune conditions such as Reynaud syndrome, lupus, rheumatoid arthritis, and polymyalgia are more frequent in MDS patients, and activated cytotoxic T cells and elevated levels of tumor necrosis factor alpha (TNF-) and interferon (IFN) 1-4 are found in MDS. Cytokine dysregulation and excessive apoptosis are most notable in early MDS, 3-6 where up-regulation of Fas-receptor and Fas-ligand expression are hallmarks of the disease. 7-11 The concept of using immunosuppressive treatment (IST) for bone marrow (BM) failure originates severe aplastic anemia (SAA) patients. 12 Gluckman and colleagues 13 were the first to show that hematopoietic recovery could occur in SAA following antithymocyte globulin (ATG) treatment alone. As experience and success in IST for SAA accumulated, small numbers of patients with BM failure secondary tp hypoplastic MDS were treated successfully. The response of such patients led to the establishment a clinical trial of IST at the National Institutes of Health in 1994 to evaluate the ability of ATG to improve BM failure in all forms of MDS. 14 This study showed improvement in 30% of patients with MDS after ATG. Further efforts to identify the profile the characteristics of the IST responder have resulted in better response rates to IST. 15 This article describes the laboratory evidence supporting a role for the immune system in the marrow failure of MDS and clinical trials using IST in these patients.
