Chien-Chou Hou, Ph.D., is an Associate Professor in the Department of Medical Sociology & Social Work at Chung Shan Medical University in Taiwan and also serves as the President of the Taiwan Association of Health Care Social Work. Dr. Hou holds a Ph.D. in Social Policy and Social Work and plays a significant role in Taiwan's social work and social policy fields. He serves as the President of the Taiwan Association of Health Care Social Work, Director of the Taiwanese Association of Social Policy, Supervisor of the Taiwan Association of Gerontology, Director of the National Union of Social Workers, and Director of the Taiwan Association of Social Work Education.
Dr. Hou has attended and presented at several conferences recently, including the “19th East Asian Social Policy Research Network 2023 Annual Conference (EASP 2023)” in Sydney, Australia, in 2023; the “Joint Conference on Social Work, Education, and Social Development (SWSD 2024)” in Panama City, Panama, in 2024; and the “20th East Asian Social Policy Research Network 2024 Annual Conference (EASP 2024) & 30th Foundation for International Studies of Social Security (FISS 2024) Joint International Conference” in Kyoto, Japan.

Performance management has considerable influence in today's society and is almost a ubiquitous m... more Performance management has considerable influence in today's society and is almost a ubiquitous management action. Due to the value of new public management, users can raise expectations, check quality, complain to suppliers, etc. Currently, the delivery of public or medical services is almost always under the control of performance management. Therefore, this study intends to explore the sources of performance indicators and the characteristics of performance management practices of medical social workers, as well as the corresponding practices of medical social workers for performance management, and to put forward research suggestions. This study conducted in-depth interviews with 5 senior managers directly under the social work department of the hospital, 5 medical social work supervisors, 5 medical social worker staff, and 3 representatives of medical social work organizations, for a total of 18 research participants.

This study aimed to investigate the responsive strategies of clinical medical social workers toward managerialism based on the interaction between its and medical teams. This study conducted 18 in-depth interviews. The effects of managerialism on clinical medical social workers were derived from cognition and the expectation gap between medical teams and clinical medical social workers. This study generalized the professional responsive strategies of clinical medical social workers as follows: (1) communication and persuade: demonstration of discussions and communication competency to reveal professional functions, (2) support and internal control: stability of internal aspect of social workers to facilitate self-care and growth, and (3) education and spread: making changes to construct a friendly environment. Finally, this study proposes discussions and suggestions and concludes the responsive strategies of medical social work.

[[abstract]]1970年代的石油危機與1980年代後的經濟動盪，形成了管理主義的思潮，其亦影響健康照顧領域，故醫務社會工作也難以迴避。然在績效邏輯、市場原則下，醫務社會工作受影響的面貌... more [[abstract]]1970年代的石油危機與1980年代後的經濟動盪，形成了管理主義的思潮，其亦影響健康照顧領域，故醫務社會工作也難以迴避。然在績效邏輯、市場原則下，醫務社會工作受影響的面貌為何？醫務社會工作又是如何回應？醫務社會工作該如何定位？此些為本研究之目的。本研究採質化研究取向，針對5個研究個案醫院分別訪談醫院主管、社工主管、社工人員3種成員，再加上3位社會工作專業團體成員，共計18位受訪者。本研究發現，管理主義對於醫務社會工作的影響有：（1）財務成本面（如員額控管等影響）、（2）管理制度面（如顧客導向、績效管理等影響）、（3）專業臨床面（如專業者間的期待落差、自由裁量權受限等影響）。醫務社會工作對上述影響之回應策略包括：（1）積極模式：以醫院的一員自居、採取自我績效要求、發揮專業能力、運用管理主義的邏輯、穩固社工內部、拉大參與格局、釐清相同處、種下改變的種子等策略；（2）消極模式：避開壓力感、轉嫁壓力值、合理化必然性等策略。基於研究發現，本研究建議包含：（1）社會工作人力方面（如結合醫院利益考量來倡導醫務社會工作人力之增聘）、（2）專業臨床與醫療團隊方面（研發管理主義下所需的社會工作專業能力）、（3）養成教育方面（如加入管理主義相關單元於醫務社會工作養成教育中）、（4）專業組織與政策倡導方面（如繼續運用專業組織能量來規範醫務社會工作的服務內容）、（5）專業發展與自我定位方面（如決定醫務社會工作未來的發展路線）。[[abstract]]Managerialism had appeared since the Oil Crisis in the1970s and the unstable economy after the 1980s. Managerialism then affected the health care, as well as the medical social work. However, based on the performance logic and the market principle, this study intents to explore what the situation of medical social work is under the impact of managerialism. Also, how should the medical workers cope with it? What are the positions of the medical work? These are the objectives of this research. This dissertation is a qualitative study that researches into five hospitals as the cases. Eighteen interviews were conducted during the research. There were fifteen interviewees from the hospitals, which included the hospital director, the social work director, and the social worker in each hospital. In addition, another three members from the professional social work organizations were chosen to be interviewed. This research shows that managerialism affects medical social work in three dimensions: (1)financial and cost, (2)management strategies, and (3)professional practice. The research concludes the findings with two models with which the medical social workers and social work directors use to react the impact of managerialism. The first model is the Positive Model. It includes identifying oneself as the hospital member, working hard for performance management, performing professional abilities, utilizing managerialism logic, caring social work department, broadening their vision, distinguish the common elements, and cultivating opportunities for change. The other one is the Passive Model. It refers to avoiding the pressure, transferring the pressure, and rationalizing the inevitable situations. According to the research results, this dissertation offers some suggestions in five dimensions: (1)the number of social worker, (2)professional practice and medical teams, (3)education, (4) professional organization and policy advocacy, and (5)professional development and self-positioning.[[note]]博

ABSTRACT Taiwan faces the world’s top aging rate over the next 8 years, making long-term care a p... more ABSTRACT Taiwan faces the world’s top aging rate over the next 8 years, making long-term care a priority for all populations, including the 16 indigenous groups who live primarily in the mountains and have different cultural practices than mainstream Taiwanese people. To examine how home care services are coordinated, managed, and delivered to the indigenous populations, we interviewed 10 public care managers and analyzed the interview content to assess their work experiences with home care workers and indigenous users in 2016. The research revealed four findings: (1) Home care utilization patterns and attitudes were different between the rural indigenous and non-indigenous populations; (2) home care utilization was limited by cultural, economic, and language challenges; (3) home care workers faced cultural and socio-structural challenges; and (4) policy and service provisions were rigid, without the socio-structural flexibility needed to accommodate the indigenous culture. A more efficient model will require the collaboration of public care managers, home care workers, and indigenous families to create a fair-coordinated plan. As Taiwan launches the Ten-year Long-Term Care Plan 2.0, a one-stop service delivery center to integrate care options and provide services for people of indigenous backgrounds is a must.

Methods: We used data contained in the Taiwan Biobank database (2011-2018) and included 19,133 participants having complete information on hypertension, rs762551 polymorphism, coffee intake, etc. The risk of hypertension was determined using multiple logistic regression. Results: Coffee intake was significantly associated with a lower risk of hypertension. The odds ratio (OR), 95% confidence interval (CI), and p-value were 0.877, 0.807-0.954, and 0.0032, respectively. CYP1A2 rs762551 was not significantly associated with the risk of hypertension, but it had a significant interactive association with coffee drinking (p value = 0.0303). After stratification by rs762551 genotypes, the inverse coffee drinking-hypertension association was retained, but significant results were observed only in those with the AC + CC genotype (OR 0.678, 95% CI 0.722-900, p value = 0.0001). According to the combination of coffee drinking and rs762551 genotypes (reference group: no coffee drinking and rs762551 AA), the coffee drinking-AC + CC group had a lower risk of hypertension (OR 0.888, 95% CI 0.789-0.999, p value = 0.0483). Conclusion: Coffee drinking, particularly among individuals with the CYP1A2 rs762551 AC + CC genotype was associated with lower odds of hypertension.

Challenges to Successful Cultural Care for Indigenous People in Indigenous Areas in Taiwan

Cultural care plays a crucial role in the long-term care of indigenous people in order to build c... more Cultural care plays a crucial role in the long-term care of indigenous people in order to build cultural responsiveness to indigenous human dignity. Culturally based care factors are recognized as the main influence on human expressions related to health and well-being. Therefore, long-term care need to be culturally competent so that they can provide care for their receipts with respect to their cultural practices. This study aimed to identify key challenges to successful cultural care for indigenous people in indigenous areas in Taiwan. A qualitative research strategy was applied. We purportedly recruited 13 long-term care professionals from tribes of the indigenous area in Taichung City, Taiwan. Face-to-face interviews were conducted from April to October 2021. Qualitative content analysis was used to analyze the qualitative data. Eight themes related to the challenges emerged: Outmigration, Limitations in transportation and geographic location, Difficulties in regional dialects, Different religious beliefs, Long-term care services that were not provided by organizations in tribes, Organizational opposition and differentiation in indigenous area, Ethnically mixed indigenous areas, and Difference in emphasis on quality. The findings highlight the need to provide enough resources for indigenous areas, strengthen human resources in long-term care, cultivate cultural competencies of the long-term caregiver act, build long-term care service with cultural expectations, establish partnerships with equal rights, build consensus on cultural care and learn from indigenous people.
