Background: This case-control study investigated both the risk factors and outcome of mood disord... more Background: This case-control study investigated both the risk factors and outcome of mood disorders in epilepsy. Methods: For this study, 44 patients with both epilepsy and a mood disorder (study group) were compared with 44 randomly selected patients of epilepsy without a mood disorder (control group). Psychiatric diagnosis was made as per ICD-10 Diagnostic Criteria for Research (ICD-10 DCR). International classification for seizure types (1981) was used for classification of seizure types. Results: Of the patients in the study group, a majority were educated up to at least primary level, had later onset of seizures, longer duration of epilepsy and cluster attacks. The outcome of mood disorders in epilepsy was found good in most. Conclusions: Educated patients who develop epilepsy at a later age and patients with poorly controlled epilepsy are more likely to experience mood disorders. In most patients with epilepsy, mood disorders remit completely; notably, in some patients affective symptoms resolve spontaneously.

Given the paucity of quantitative EEG studies using coherence measures to understand the electrop... more Given the paucity of quantitative EEG studies using coherence measures to understand the electrophysiological functional integrity of sub-cortical structures in obsessive-compulsive disorder, the current study was carried out. We obtained EEG coherence values for 20 adult OCD patients (10 males; 10 females) and 19 appropriately matched healthy controls across delta (0.5-3.5 Hz), theta (4-7.5 Hz), alpha (8-12 Hz), beta1 (12.5-20 Hz) and beta2 (20.5-30 Hz) bands. As coherence between distant brain regions reflects physiological activities at sub-cortical neural networks, we chose EEG channels at four distant brain regions - anterior interhemispheric, posterior interhemispheric, fronto-temporal and fronto-occipital. In comparison to controls we found significant increase of theta band EEG coherence in the fronto-occipital region in OCD patients (P = 0.045) which did not correlate significantly with either medication status or disease severity. This EEG coherence study that suggests hyperactivity at subcortical circuitry in OCD patients is in agreement with existing neuro-imaging findings. Furthermore, this finding provides external validity for sub-cortical dysfunction hypothesis of OCD.

Two cases of florid manic psychoses following head injury are reported along with neurological and neuropsychological investigations. Findings on the Luria Nebraska Neuropsychological Battery (LNNB) suggested residual cognitive deficits, predominantly of right hemisphere. Temporal proximity, clinical neurological findings, EEG changes and deficits on LNNB suggest a causal link between head injury and mania.

Objective: This study was performed to investigate the rate of occurrence, severity, and course o... more Objective: This study was performed to investigate the rate of occurrence, severity, and course of clozapine-induced thrombocytopenia. Patients and Methods: This was a retrospective study of the case records of the 28 patients receiving clozapine therapy. Results: Five patients (17.8%) had had at least one episode of thrombocytopenia. There were no differences in sociodemographic and clinical variables between patients who had and did not have thrombocytopenia. Analysis of the characteristics of thrombocytopenia revealed the episodic nature of the condition. All the thrombocytopenic episodes took place within the therapeutic dose range of clozapine and resolved without clozapine dose reduction. Conclusion: Thrombocytopenia is not an unusual adverse effect of clozapine therapy. A prospective study with is required to confirm these findings.

Behavioural problems are common in people with an intellectual disability (ID). Psychopharmacolog... more Behavioural problems are common in people with an intellectual disability (ID). Psychopharmacological treatments include several drug groups such as antiepileptics, antidepressants, and benzodiazepines, although the most extensively used are antipsychotics, especially risperidone. However, there is little or no literature on the metabolite of its monthly administration-paliperidone palmitate. The objective of this study was to assess the efficacy and tolerability of paliperidone palmitate when administered monthly to treat the behavioural alterations associated with ID. Methods This was a prospective, observational, open-label, 3-year duration study on adults with intellectual disabilities (DSM-V criteria) and associated behavioural disorders who had paliperidone palmitate added to their usual treatment. Pre-treatment and 6 months after starting the treatment, the participants were given a full blood chemistry panel, and scored on the Aberrant Behaviour Checklist (ABC) and the UKU side effects scales. Results The sample consisted of 34 participants, with a mean age of 40.35 years (18-62), and 64.7% of them had some concomitant medication at baseline. The mean monthly dose of paliperidone palmitate was 116.17 mg (75-200 mg). There were statistically significant improvements in all 5 subscales of the ABC scale (being especially strong for irritability and hyperactivity) and in certain of the metabolic parameters. The treatment was withdrawn in one case due to amenorrhea. Conclusions Paliperidone palmitate was associated with significant improvements in behavioural problems associated with intellectual disability, and was well tolerated. Further studies are necessary to establish its efficacy and tolerability for this specific population.

Efforts have been made to meet the rising demands for innovative therapeutic services that can give autistic children to develop intellectually, socially and to discover their talents & cope with their challenges. One such therapeutic module is "Floor time therapy". It is a component of the comprehensive Developmental, Individual-difference, Relationship-based intervention and assumes that language, cognition, emotional and social skills, are learned through relationships that involve emotionally meaningful exchanges, children vary in their underlying motor and sensory processing capacities, and progress in all areas of development is interrelated. The tools were used Developmental Screening Test, Indian adaptation of Vineland Social Maturing Scale and Indian Scale for Assessment of Autism. The therapeutic potential of Indian ragas has been well documented in the treatment of various psychological problems. Ragas are believed to affect specific chakras or energy centers and create an atmosphere of harmony in the body. The result shows that the potential ragas were played in a pre-planned sequence during the Floor time therapy sessions. Integration of these two therapeutic techniques showed positive results.

This paper report findings from a high resolution (192-channel) EEG study conducted on 15 drug free/naïve consenting schizophrenia patients, in which the specific role of right hemispheric MNS has been explored over 4 weeks of antipsychotic treatment. The authors used mu wave suppression paradigm, and found that baseline right MNS mu suppression correlated negatively with and predicted relative improvement in thought disturbance cluster score of PANSS during first 4 weeks of drug treatment.

Objective: Minor physical anomalies (MPAs) and gamma oscillatory activity have been proposed as a... more Objective: Minor physical anomalies (MPAs) and gamma oscillatory activity have been proposed as associated endophenotypes in schizophrenia. Combining these endophenotypes to create a composite endophenotype may help identify those at risk for schizophrenia better. The present study aims to investigate MPAs and gamma oscillatory activity in schizophrenia patients, their unaffected first degree relatives and healthy controls and appreciate whether they can be used together as a composite endophenotype. Methods: This was a cross sectional family study conducted at a tertiary care mental health setup. Ninety participants including schizophrenia patients, their first degree relatives and controls (thirty each) were assessed for MPAs on the Extended Waldrop Scale. All participants underwent an awake, resting 192channel EEG recording. Spectral power and coherence in 30-100 Hz gamma bands were estimated using Welch's averaged periodogram method. One-way ANOVA, chi square test were used for comparing sociodemographic-clinical variables. MANOVA supplemented by one-way ANOVAs (post hoc Tukey HSD) were done for comparison of spectral measures. Pearson's correlation, step-by-step linear discriminant functional and intra-familial correlation analysis were subsequently performed. Results: An endophenotype pattern of finding was found for MPAs in the craniofacial region, the total number of MPAs, spectral power in right temporal region on all bands and in the right parietal region in 50-70 Hz and 70-100 Hz gamma bands. The three groups were most accurately classified when MPA total score, right temporal 30-50 Hz gamma power and right occipital 'intra hemispheric' 50-70 Hz gamma coherence were considered together than when considered independently. Significant intra familial correlation was seen for MPA total score and right temporal gamma 30-50 Hz power. Conclusion: Composite evaluation of two developmentally linked markers i.e. MPAs and gamma spectral measures may prove useful in categorizing schizophrenia and identifying at-risk individuals.
